That keeps writes available in documented cases, but it does not make the operation free: index creation can add CPU and I/O load, consume storage and transaction-log capacity, and wait on long-running transactions. The safe command depends on your engine, version, edition or managed service, storage engine, table structure, and index type.
Choose the method for your database engine
“Online” and “concurrent” describe availability behavior, not zero impact. Before running a command, identify the exact platform and release, the table’s storage engine and partitioning, and the type of index you plan to create. Then check the documentation for that specific combination; support and syntax are not interchangeable across engines or releases.

There is no universal safe table-size threshold or completion-time estimate established for these methods. Even PostgreSQL’s documentation notes that builds on very large tables can take many hours without defining a general size cutoff. Estimate and test against the actual workload and environment rather than promising a duration or slowdown percentage.
PostgreSQL: build concurrently when writes must continue
Run a concurrent build
For a typical non-partitioned table and index, use:

CREATE INDEX CONCURRENTLY index_name ON table_name (column_name);
A regular CREATE INDEX takes a lock that blocks inserts, updates, and deletes on the table until the build finishes, although reads remain possible. The concurrent form is designed to let those writes proceed, but it takes longer and adds CPU and I/O work because PostgreSQL scans the table twice.

Account for operational restrictions
CREATE INDEX CONCURRENTLYcannot run inside a transaction block.- Only one concurrent index build can run on a table at a time, and schema changes to that table are disallowed while the build is in progress.
- The build waits for transactions that could affect the index. Long-running transactions can therefore delay completion.
- For a unique index, uniqueness enforcement can start before the index is usable and can remain in effect if the build fails.
Handle partitions and failures
PostgreSQL does not directly create a partitioned parent index concurrently. Its documented approach is to build the index concurrently on each partition, then create the partitioned index on the parent non-concurrently; that final step is metadata-only and reduces the parent table’s write-lock interval.
If a concurrent build fails, an invalid index may remain. Queries ignore it, but it can still add write overhead. Check the index’s validity and remove or rebuild it as appropriate, taking unique-index behavior into account. The PostgreSQL documentation also describes build progress through pg_stat_progress_create_index; confirm the view and available fields on your target release.

Add a secondary index

For this documented InnoDB secondary-index operation, the table stays available for reads and writes while the index is created. The operation’s performance, space use, and semantics are subject to its online DDL limitations, and completion waits for transactions accessing the table.
Verify the algorithm and lock behavior
MySQL’s CREATE INDEX also provides ALGORITHM and LOCK clauses to influence copying and concurrency. Do not assume a requested clause is supported for every table and operation. Check the online DDL limitations for the exact release and operation, and verify what the target server actually supports before execution. The cited operation guide covers MySQL 8.4; the separate MySQL 26.7 CREATE INDEX reference documents clause syntax for that reference version, which should not be treated as a substitute for checking the target release.

SQL Server: use online options only where supported
Check support before choosing ONLINE
For an index operation supported by the exact SQL Server edition and index type, use its ONLINE = ON option. Online creation or rebuild still needs short shared or schema-modification lock phases. A long explicit transaction can extend those phases and block other work, so transaction duration matters as well as the online setting.
Control resources and consider resumable work
Online operations maintain source and target structures during the work, which can increase resource consumption for data modifications. Use MAXDOP to cap parallelism when appropriate, after weighing build speed against available resources.
Microsoft documents resumable online create for supported cases in SQL Server 2019 and later, Azure SQL Database, SQL database in Microsoft Fabric, and Azure SQL Managed Instance. Resumable operations can be paused and resumed, but require additional space and have functional limitations. Check the exact platform, edition, operation, and index-type support before relying on resumability; consult Microsoft’s online index operation guidelines.

Production checklist: prepare, monitor, and verify
- Define the target workload. Identify the query or workload the index is meant to help. Validate the proposed key order and whether uniqueness is required against that query pattern; there is no single index-design rule that applies to every workload.
- Check whether the index is justified. Look for an equivalent index before adding another. Indexes consume storage and require ongoing maintenance, so avoid adding speculative indexes solely because a query is slow.
- Assess the actual build conditions. Record the engine and version, edition or managed service, table size and partitioning, index type, available disk and transaction-log capacity, write rate, CPU and I/O headroom, and long-running transactions. These determine whether the documented online path is supported and whether the system can absorb its work.
- Choose timing and resource controls. Prefer a lower-traffic period when possible, and apply platform-supported resource controls. Scheduling the work does not remove its extra CPU, I/O, storage, or transaction impact.
- Watch the build and application together. Monitor build progress, latency, write throughput, lock waits, CPU and I/O, free storage, log growth, and replication lag where relevant. Use metrics and progress views supported by the exact engine and release.
- Define abort and recovery actions in advance. Decide who can stop the operation, how to handle a failed or paused build, and how to inspect and clean up leftover state. For PostgreSQL, include checking for an invalid index and account for uniqueness enforcement; for resumable SQL Server work, check the operation’s resumable state.
- Verify the result. Confirm the index’s validity and metadata, then observe whether the target query’s plan and behavior changed under the real workload. Creating an index does not by itself prove that it improves the query.
