To create a useful customer satisfaction questionnaire, start with the decision you need to make, define which customers and experience you want feedback on, choose a measure that fits, and ask only clear, answerable questions. Pretest the questionnaire before sending it, then interpret results in light of who responded and how the survey was run.

Start with the decision—not a list of questions

Write one sentence stating what decision the results should support. For example: “We want to identify what to improve in the checkout experience for customers who purchased in the past month.” Then define the relevant customer group, product or service, touchpoint, and recall period.

Consider whether a survey is the right tool. Existing operational records may show where customers abandon a process, while interviews may help explain why. Use a questionnaire when you need structured answers from a defined population or want to track a consistent measure over time. The American Association for Public Opinion Research (AAPOR) recommends aligning survey design with the research objective and being transparent about how the sample was built and recruited: AAPOR Best Practices for Survey Research.

Choose a measure that matches what you want to learn

Customer satisfaction, recommendation, and perceived effort are related, but they measure different things. Choose the one tied to the experience or decision under consideration rather than combining them into an undefined “happiness” score.

For a satisfaction item, ask about a defined experience rather than leaving “satisfied” open to interpretation. For example: “How satisfied are you with your recent support interaction?” Qualtrics illustrates a five-point scale anchored from highly unsatisfied to highly satisfied, but that is an example, not a universal requirement. Adapt the wording and options to your audience and purpose: Qualtrics CSAT survey questions and template.

A rating can tell you how a customer responded without revealing why. If an explanation will help you choose an improvement, add one focused follow-up, such as “What most influenced your rating?” or “What one change would improve this experience?” Avoid adding open-text questions simply to make the questionnaire feel more thorough; they take respondents longer to answer and require more work to analyze.

Write clear, neutral questions and usable answer choices

Use familiar language and ask about one idea at a time. State the experience and relevant timeframe, and avoid leading language, assumptions, and double-barreled questions. “How satisfied were you with the speed and friendliness of support?” asks about two attributes in one answer; a low rating would not show which one the respondent meant. Ask about them separately if both matter.

  • Make response choices fit the question and easy to distinguish.
  • Offer “not applicable” or “don’t know” when respondents may not have had the experience or cannot reasonably answer.
  • Provide a skip option for sensitive questions, and do not interpret a blank response as “none.”
  • Use closed-ended choices when you need comparable answers; use open-ended prompts when you need reasons or ideas the available choices may miss.

There is no universal question-count rule established by the cited guidance. Keep the questionnaire as focused as the decision allows, and remove items that will not inform an action. See Pew Research Center’s guidance on writing survey questions and AAPOR’s survey best practices.

Put the questionnaire in a sensible order and choose a suitable channel

Begin with easy, relevant questions and arrange the rest in a natural sequence. Question order and surrounding context can influence answers, so avoid introducing material that could steer responses to later items. Pick a channel and timing that suit the experience being evaluated and the customers’ access. Email, a website, an app, SMS, chat, mail, and in-person delivery are all possible; none is the best choice in every situation. Qualtrics discusses these options in its customer experience survey design guidance.

Explain the survey’s purpose where appropriate. Collect only information needed for the stated use, follow your organization’s policies and applicable privacy laws, and consider whether combinations of demographic details and answers could identify an individual. AAPOR recommends minimizing respondent burden and allowing people to skip sensitive questions.

Pretest the questions, then pilot the whole process

Before launch, ask people similar to the intended respondents to explain what they think each question means and how they chose an answer. This kind of cognitive pretesting can expose unclear terms, missing response options, or assumptions the writer did not notice. Pew Research Center calls pretesting an essential step, particularly when questions are being introduced for the first time: Writing Survey Questions.

Then pilot the full process, not just the questionnaire text. Test recruitment, delivery, survey programming, response handling, data cleaning, and analysis. Revise and test again if a change to the instrument or process could affect results. A polished question cannot compensate for a broken invitation link or a response workflow that loses answers.

Analyze results in context and report what they represent

Set data-cleaning rules before interpreting scores. Check for incomplete or inattentive responses, summarize results at a level relevant to the original decision, and compare customer groups or touchpoints only when the sample supports that analysis. Low response can leave some types of customers underrepresented, so respondents should not automatically be treated as representative of all customers.

When sharing findings, provide enough detail for others to understand what the result does and does not show:

  • The population the questionnaire was intended to represent
  • How the sample was constructed and respondents recruited
  • The response count and survey mode
  • The exact question wording and answer options
  • Any weighting or uncertainty measures used

Keep the method stable when comparing results over time

A change in score may reflect customer sentiment, a change in how the survey was conducted, or both. For a trend comparison, keep wording and framing, question order and context, mode and timing, and sample and recruitment method as consistent as practical. If you change a measure or questionnaire, test the old and new versions side by side where feasible and disclose the change when reporting the results.

Response format itself can affect what people report. In Pew Research Center’s 2008 post-election poll example, 58% selected “the economy” when it appeared among closed-ended choices, compared with 35% who volunteered it in an open-ended version. That illustration concerns response format, not customer satisfaction, and is a reminder that differently framed questions may not produce directly comparable answers.

Example model questionnaire

Adapt this short structure to your audience and objective; it is a starting point, not a validated universal instrument.

  1. Define the experience: “Thinking about your support interaction on [date or timeframe], …”
  2. Ask the core rating: “How satisfied were you with your recent support interaction?” Provide response choices appropriate to the question, such as a clearly labeled satisfaction scale.
  3. Ask one explanatory follow-up if useful: “What most influenced your rating?” Make clear whether an answer is optional.
  4. Ask for a practical improvement if it will inform a decision: “What one change would improve this experience?”
  5. Include only necessary context questions: Ask about customer or experience characteristics only when they support the stated analysis, and provide an appropriate skip or “not applicable” option.

Before sending it, check that every item maps to the objective, respondents can answer from their experience, and response choices cover plausible answers without forcing a guess.
