Microsoft 365 Copilot can produce fluent answers that are inaccurate, incomplete, or out of date. Grounding and citations can help you find the information behind a response, but they do not prove that its claims are supported or that important context has been preserved. For anything consequential, check the original sources and independently verify the details before acting or sharing.
Why can Microsoft 365 Copilot give wrong answers?
Copilot generates a response from the information available to it, rather than guaranteeing that every statement is correct. Microsoft Support warns that output may be inaccurate or out of date, and says a response can sound polished while still being wrong, incomplete, or risky to use. - It may not find relevant information. If Copilot cannot locate sources relevant to your prompt that it is allowed and able to access, it may answer more generally. That can leave out organization-specific details or lead to an answer that does not fit your situation. (Microsoft Support: What information does Copilot use to answer my prompt?)
- Its sources may be stale. An old policy, file, or web page can lead to outdated guidance. The answer may reflect the source it found, not the latest decision or version.
- A summary can lose or add meaning. When synthesizing multiple sources, Copilot can omit a qualification or produce content that is not present in the inputs. - Your prompt or sources may leave out context. If you do not specify the relevant date, audience, exception, or version, the response may make assumptions that change the conclusion.
- The wording may overstate the evidence. A response can sound certain even when its sources support only a narrower claim. Treat confidence in the writing as separate from confidence in the facts.
Microsoft does not provide a general error-rate statistic in the cited guidance that would tell you the chance of error for an individual Copilot response. Accuracy depends on the task and the information Copilot can use; a single percentage should not be assumed.

The source base depends on the Copilot experience, account, settings, permissions, and enabled capabilities. Depending on the experience, grounding may draw on work content such as files, email, chats, and meetings; public web information; or content you attach. A work or school Copilot can reference organizational content only if you are authorized to access it. Web search and other capabilities may also be controlled by an administrator. Microsoft describes Work IQ being off in the relevant experience as meaning Copilot answers without referencing Microsoft 365 work data. Available modes and controls can differ by account type, license, and tenant configuration, so check the options actually shown in your account rather than assuming every user has the same sources. Grounding can make a response more relevant by connecting it to available information. It does not certify that the answer is correct: source availability is conditional, and generated text may still misrepresent, omit, or go beyond its sources.

How to check a Copilot answer
Microsoft Support recommends four checks: Source, Verified, Context, and Resilient. Use them in sequence, with more scrutiny when the answer will inform a decision, go to customers or leadership, combine sources, or guide time-sensitive or difficult-to-reverse work. - Source: Open the inline citations and the Sources list or pane. Trace each important claim to its original file, message, data, or web page. Read the relevant passage and compare it with Copilot’s wording. Check for unsupported additions, distorted wording, and missing qualifications—not just whether the cited source exists. - Verified: Independently confirm the facts that would matter most if wrong, especially names, dates, figures, and recommendations. Use the original authoritative source or another trusted source; do not accept a plausible-sounding detail just because it appears beside a citation.
- Context: Ask what assumptions, exceptions, dependencies, or risks are missing. Check whether including them would change the answer or recommendation. A smooth summary can still omit context that matters to a decision.
- Resilient: Consider alternative interpretations, different audiences, and exceptions that could make the response misleading or incomplete. You can ask Copilot to identify gaps or critique an answer, but treat that critique as another generated response and confirm important findings against trusted sources.
How to reduce errors before prompting
Good source selection makes checking easier, but it cannot guarantee correctness.

- For a current policy, product detail, or decision, name or attach the authoritative, up-to-date file or page.
- When similar documents exist, identify the intended version explicitly and check its date and relevant section.
- State the scope that affects the answer—for example, the applicable audience, time period, or scenario—and ask Copilot to flag assumptions, exceptions, and uncertainty.
- Review whether the response actually used the sources you intended. Access, permissions, settings, and enabled capabilities can affect what it can reference.
These steps help direct Copilot to relevant evidence; they do not replace reviewing that evidence yourself.

Do not act on or forward a material claim until you have checked it against a trusted source. If the response contains an error, Microsoft advises submitting feedback that describes the issue. Microsoft also describes evaluating groundedness, response quality, and citation relevance, with unsupported claims, missing or irrelevant citations, and inconsistent results among possible suboptimal outcomes.
