Check important claims against sources suited to the subject, and get qualified professional advice for medical, legal, or financial decisions.
Why can ChatGPT be wrong?
ChatGPT is a language model: it generates text by drawing on patterns learned during training and predicting likely continuations. A likely continuation is not necessarily a factually accurate one. OpenAI describes a plausible but false statement from a language model as a hallucination, and notes that errors can arise even in answers to apparently straightforward questions. OpenAI’s explanation of language-model hallucinations covers both the mechanism and the role of evaluation incentives.
This does not mean every answer is a database lookup—or that ChatGPT is deliberately lying. The model can produce natural-sounding text without having established that each detail is true. Arbitrary, uncommon, or highly specific details can be especially difficult to infer from language patterns.
Questions leave room for assumptions
A broad or ambiguous question may have several reasonable interpretations. If the answer depends on a date, location, definition, or other constraint that was not supplied, ChatGPT may fill in the gap with an assumption. The result can be coherent but not relevant to the situation you meant.

Some details are easy to state incorrectly
Errors can include incorrect definitions, dates, figures, and other facts, as well as invented quotations, studies, or citations. Complex questions can also invite overconfident answers that conceal uncertainty. OpenAI’s Help Center discusses these kinds of misleading outputs in “Does ChatGPT tell the truth?”
Information can be out of date
ChatGPT may not have the latest information about a changing policy, product, event, or other current detail. Search or deep research features may help it consult newer material, depending on the product and plan, but using such a feature does not establish that the answer interpreted its sources correctly. Open the cited material and check the specific claim yourself.

Evaluation can reward guessing
OpenAI argues that evaluations focused mainly on exact answers can reward a guess when it happens to be right while giving little credit for an honest “I don’t know.” That incentive can favor confident answers over appropriate uncertainty. A request for clarification or an explicit statement that the answer cannot be established may be more useful than a confident unsupported claim.
How to reduce the chance of relying on a wrong answer

Make the question specific
Include the relevant date, location, definition, and constraints. If more than one interpretation is possible, ask ChatGPT to list its assumptions or ask you a clarifying question before answering. Clearer wording reduces ambiguity; it does not guarantee accuracy.

Ask it to separate claims from inferences
Request the key factual claims, what evidence supports each one, what is uncertain, and what information is missing. Treat the result as a guide to what to investigate, not as proof that the evidence is real or has been interpreted correctly. Citations can be fabricated or fail to support the accompanying statement.

Use current-information features when timing matters
If the answer depends on recent developments, use search or deep research if the feature is available to you. Then open each relevant citation and confirm that the source directly supports the detail in question. A search tool can help locate recent evidence; verifying that evidence is a separate step.

Verify consequential details independently
Check names, dates, figures, quotations, and citations against reliable sources, preferably primary sources such as the relevant agency, organization, or original document. Match the source to the claim: a source can be credible yet still not support the precise detail ChatGPT stated.

Ask for uncertainty, not confidence
Try asking, “What can’t you establish from the available information?” or “What assumptions could change this answer?” This gives the model room to flag missing context or uncertainty instead of forcing a definitive response.

Get expert review for high-stakes decisions
For medical, legal, or financial matters, use ChatGPT to prepare questions or understand general concepts, not to replace a qualified professional. OpenAI Academy’s guidance on responsible and safe AI use recommends checking critical facts and consulting qualified experts for consequential decisions; ChatGPT is not a licensed professional.

Report incorrect or unsafe replies
OpenAI recommends using the thumbs-down or report option to flag an incorrect or unsafe answer. Feedback may help the provider improve its systems, but it does not automatically correct the reply you already received. Verify or replace that answer before relying on it.
How much should you trust a ChatGPT answer?
Trust should depend on the claim, its freshness, the evidence behind it, and the consequences of being wrong—not on how polished the response sounds. Use this check before acting:
- Is the claim time-sensitive? If it concerns something that may have changed, find a current source and check its date.
- Does the source support the exact detail? Open the citation and compare its contents with the answer, rather than relying on the presence of a link.
- Is the question ambiguous? Clarify the interpretation and assumptions before treating the answer as applicable.
- What happens if the answer is wrong? The greater the consequence, the more important independent verification and qualified human review become.
OpenAI has reported a historical comparison in which GPT-4 was 40% more likely than GPT-3.5 to produce factual content. That figure comes from OpenAI’s 2023-era safety overview; it is a comparison between those models, not a current error rate for ChatGPT or a guarantee about any individual answer. It should not be used to infer that a particular response is correct. A universal, directly comparable error rate for ChatGPT is not established by that comparison.
