The accountable authority accepts cybersecurity risk. In the NIST Risk Management Framework (RMF), that authority is the authorizing official: a senior official or executive with responsibility and accountability for operating the system. NIST identifies this person as the only organizational official who can accept security and privacy risk to organizational operations, assets and individuals.
Under ISO/IEC 27001:2022, the equivalent decision maker is the risk owner. Clause 6.1.3 requires the organization to obtain the risk owner’s approval of the information-security risk-treatment plan and acceptance of residual risk. Security staff, privacy officers, IT managers and vulnerability finders can assess, advise and administer the paperwork, but they do not automatically possess acceptance authority.
Who is allowed to accept a cyber risk?
Acceptance is a governance decision, not a technical task. The person who accepts risk must have authority over the mission, service, process or information affected and must be accountable for the consequences if the risk materializes.
NIST: the authorizing official
NIST Special Publication 800-37 Revision 2 uses the term authorizing official. It states that “the authorizing official is the only organizational official who can accept the security and privacy risk to organizational operations, organizational assets, and individuals.” The official approves or denies the authorization package after reviewing the system’s risk determination, planned responses and supporting evidence.

An authorizing official is normally a senior executive, mission owner or business leader rather than the person who configured a control. The role can be supported by an authorizing official designated representative, system owner, chief information security officer (CISO), privacy official or risk executive, but delegation of analysis is not the same as transferring accountability. Your governance documents should identify who may sign, what authority was delegated and any limits on that delegation.
ISO/IEC 27001: the risk owner
ISO/IEC 27001:2022 frames the decision through the risk owner. The risk owner is accountable for managing a particular information-security risk and for deciding whether the residual risk is acceptable after treatment. Clause 6.1.3 requires documented approval of the treatment plan and acceptance of residual risks by the risk owners.
ISO does not prescribe one universal job title. An organization may assign ownership to a product executive, process owner, data owner, service owner or another manager with real authority over the affected objective. The assignment should be explicit in the information-security management system (ISMS), risk methodology or responsibility matrix.
What authority means in practice
Authority should cover the business impact described in the risk statement. A manager who controls an application but cannot approve downtime, regulatory exposure, customer commitments or budget may be able to own remediation tasks without being able to accept the resulting risk. Where several business units are affected, the organization should name a single accountable owner and record the consulted stakeholders or require joint approval under its policy.

Can the CISO, IT manager or person who found the vulnerability sign?
Job title alone does not settle the question. A CISO may sign an acceptance when the organization has formally appointed the CISO as the risk owner or authorizing official for that scope and the CISO has authority over the affected mission or service. In many organizations, however, the CISO is an independent adviser or control owner. In that arrangement, the CISO can assess severity, recommend treatment, verify controls and document dissent, but the business or mission authority must make the acceptance decision.
An IT manager faces the same test. If the manager owns the service and can commit the resources and business outcomes at stake, policy may permit acceptance. If the manager only operates infrastructure on behalf of a business owner, the manager should prepare the record and obtain the business owner’s approval.
The discoverer of a vulnerability, penetration tester or security analyst should not accept the risk merely because that person created the ticket. Separating assessment from acceptance reduces conflicts of interest and makes the decision auditable.
What a defensible risk-acceptance record must contain
Keep one durable record for each material risk or decision and link it to the broader authorization package or ISMS evidence set. A ticket can be the front end, but the retained record must preserve the decision and its context even after the ticketing system changes.

| Record element | What to document | Why it matters |
|---|---|---|
| Scope and context | System, service, process, data, environment, business owner and assessment date. | Defines what the decision covers and prevents an acceptance from being misapplied to another asset or environment. |
| Risk statement | Threat or event, vulnerability or condition, affected asset, plausible consequence and impacted stakeholders. | Connects the decision to a concrete scenario instead of a vague label such as “high vulnerability.” |
| Assessment method | Likelihood and impact scales, assumptions, confidence level, risk criteria and tolerance threshold. | Allows another reviewer to reproduce the rating and understand uncertainty. |
| Accountability | Named risk owner or authorizing official, delegated representatives and consulted security, privacy, legal and business roles. | Shows who was accountable and who provided specialist advice. |
| Treatment decision | Treat, transfer, avoid or accept; selected controls; control status; action owner; milestones; dependencies and budget assumptions. | Records the chosen response and the work required to reach the intended residual-risk level. |
| Residual risk | Post-treatment likelihood, impact, rating, rationale and comparison with the acceptance criteria. | Demonstrates that the approver accepted the remaining exposure, not an unexamined initial score. |
| Decision evidence | Explicit accept or deny decision, approver identity and role, signature or authenticated approval, decision date and conditions. | Provides proof that an authorized person made a deliberate decision. |
| Review controls | Expiry or review date, monitoring metrics, reassessment triggers, change history and links to assessment reports, plans of action and milestones, policies, contracts and test evidence. | Keeps acceptance from becoming a permanent waiver after circumstances change. |
Write the risk as a scenario
Use a sentence that identifies cause, asset and consequence: for example, “An unpatched internet-facing service could be exploited to expose customer records, causing service interruption, notification costs and privacy harm.” Add the affected population and any regulatory, contractual or safety implications. A scanner finding or control gap can be attached as evidence, but it is not a complete risk statement by itself.
Make the rating reproducible
State the scales used by your policy, such as the definitions for low, moderate and high impact, and record the assumptions behind likelihood. Note confidence when evidence is incomplete. The acceptance threshold must be visible: an approver should be able to see whether the residual rating is within tolerance, above tolerance with an exception, or unresolved pending treatment.
Separate planned controls from operating controls
List controls that are already implemented separately from controls that are planned. For each planned action, identify an owner, due date, dependency and funding assumption. Do not calculate residual risk as though a future control were already operating. Attach test results, configuration evidence or assessment reports that support the control-status claim.

How to run the acceptance workflow
- Set criteria first. Approve the likelihood and impact scales, risk appetite, tolerance thresholds and escalation rules before scoring individual risks.
- Describe and assess the scenario. Record the asset, threat, weakness, consequence, affected stakeholders, likelihood, impact, uncertainty and evidence. Identify whether privacy, legal, safety or contractual obligations change the analysis.
- Assign the accountable owner. Select a risk owner or authorizing official who controls the affected objective and can commit resources or accept the consequences. Record any formal delegate and the boundaries of the delegation.
- Choose treatment. Decide whether to treat, transfer, avoid or accept the risk. Document selected controls, compensating measures, action owners, milestones and reasons for exclusions. In an ISO/IEC 27001 ISMS, align control choices and exclusions with the Statement of Applicability.
- Recalculate residual risk. Rate the exposure after the planned or implemented treatment, identify remaining uncertainty and compare the result with the approved acceptance criteria.
- Obtain explicit approval. Capture an accept or deny decision from the authorized official or risk owner, with identity, role, date, authenticated signature and any conditions. A meeting note is useful evidence only when it clearly records the decision and approver.
- Track conditions and actions. Convert deadlines, compensating controls and dependencies into monitored actions. Link the record to plans of action and milestones, tickets, test evidence and relevant contracts or policies.
- Monitor and reopen. Set a review or expiry date, define metrics and specify trigger events. Reassess or reauthorize when the system, threat, controls, law, contract, business process or impact changes materially.
NIST RMF and ISO/IEC 27001 compared
| Comparison axis | NIST RMF | ISO/IEC 27001 |
|---|---|---|
| Authority | Authorizing official, the accountable official who accepts security and privacy risk. | Risk owner, who approves the treatment plan and accepts residual information-security risk. |
| Evidence package | Authorization package containing the risk determination, risk responses and authorization approval or denial. | Risk-treatment process, Statement of Applicability, treatment plan, risk-owner approval and retained documented information. |
| Lifecycle | Authorization decision supported by continuous-monitoring and review triggers; federal programs often use ongoing authorization practices. | ISMS process with continual improvement, monitoring and reassessment when risk or context changes. |
| Scope | Federal-style RMF terminology and role structure, adaptable outside government. | Organization-adapted ISMS requirements applicable across sectors and environments. |
| Auditability | Traceable owner, criteria, analysis, controls, residual risk, approval and authorization status. | Traceable owner, criteria, treatment plan, Statement of Applicability, approval and retained evidence. |
The frameworks use different labels but converge on the same control principle: the person accountable for the affected objective must make or approve the residual-risk decision, and the record must show how that decision was reached.

When an acceptance expires or must be revisited
An acceptance is not a lifetime exemption. Define a fixed review or expiry date when the risk is time-bound, and define event-based triggers for every material risk. Reopen the decision after a major architecture or data change, a new threat or vulnerability, a failed control test, a security or privacy incident, a change in law or contract, a change in business criticality, or the completion or cancellation of a treatment action.
Monitoring metrics should be specific enough to detect deterioration. Examples include unresolved vulnerability age, control-test failure rate, privileged-access exceptions, incident frequency, patch coverage or service exposure. Record the review outcome, updated rating, new conditions and the next approval date in the same history.
How to prove acceptance to an auditor
- Show the policy or methodology that defines risk criteria, tolerance and acceptance authority.
- Provide the dated risk record with the scenario, rating, assumptions, treatment and residual-risk rationale.
- Demonstrate that the named approver’s role and delegation were valid when the decision was made.
- Produce authenticated approval and any conditions, deadlines or compensating controls.
- Trace each claimed control to assessment, implementation and testing evidence.
- Show review history, monitoring results and evidence that material changes triggered reassessment.
- For ISO/IEC 27001, cross-reference the treatment plan, Statement of Applicability and retained documented information.
- For NIST RMF, cross-reference the authorization package, risk determination, responses and authorization status.
Common governance failures and fixes
“The CISO approved it” without authority evidence
Fix the record by identifying whether the CISO was the designated risk owner or authorizing official. If not, obtain approval from the accountable business or mission authority and retain the CISO’s recommendation separately.
A vulnerability ticket is treated as acceptance
A ticket may show discovery and remediation work, but it usually lacks impact criteria, residual-risk analysis, approval conditions and review triggers. Link the ticket to a complete risk decision record.

Acceptance is issued for an entire system forever
Narrow the scope to the documented scenario, environment and period. Add expiry, metrics and event triggers so a changed system or threat cannot inherit an outdated decision.
Planned remediation is counted as completed
Mark planned controls as planned, recalculate residual risk conservatively and require evidence before changing control status. If the interim exposure exceeds tolerance, escalate it rather than silently carrying the original acceptance.
Several owners are listed with no accountable decision maker
Name one accountable owner or document the required joint-approval rule. List other parties as consulted, responsible for actions or informed, using a responsibility matrix that matches policy.

Illustrative record structure
A concise record might contain these fields in order: “Customer portal—production environment”; assessment date and business owner; a scenario describing exploitation of an unpatched component; likelihood, impact, confidence and tolerance; the designated risk owner; treatment actions with owners and dates; residual rating after compensating controls; an explicit conditional acceptance; authenticated approval and date; a 90-day review; metrics for patch age and intrusion alerts; and links to the assessment report, test evidence, policy and action plan. The example is a format, not a universal rating or approval period: your criteria and authority determine those values.
