Production AI needs four control layers working together: limits that contain traffic and spend, logs that make every decision traceable without exposing unnecessary data, fallbacks that degrade safely instead of retrying forever, and human approval before an action could cause material harm. Set each control from measured workload, provider quotas, service objectives, data sensitivity, and business risk; there is no responsible universal number for requests per minute, token budgets, retention, fallback rates, or human-review percentages.

Start with a control baseline

Write the production policy before tuning individual values. Assign an owner for each setting, record the reason for the value, define an alert threshold, and document how a change is tested and approved. Revisit the policy after a model, prompt, retrieval index, tool, safety policy, traffic pattern, provider, or geographic deployment changes.

Control What it must protect How to choose the setting What to record
Rate and concurrency limits Availability, provider quota, cost, and abuse exposure Measured peak demand, token usage, latency objective, quota, and risk tolerance Scope, limit, burst behavior, queue policy, owner, alert, and change procedure
Spend and token budgets Unexpected bills and runaway workflows Approved budget, expected workload, maximum task cost, and reset period Budget scope, accounting method, enforcement action, and escalation contact
Operational logging Debuggability, safety evidence, and accountability Minimum fields needed for operations and audits, with privacy and retention constraints Schema, redaction rules, access roles, encryption, retention, and deletion process
Fallback and circuit breaker Reliability without unsafe or uncontrolled degradation Tested failure rates, deadlines, equivalent-model capability, and business impact Error classes, retry policy, routes, open/close conditions, and degraded response
Human approval Irreversible, high-impact, safety-sensitive, or externally visible outcomes Risk tier, legal and policy obligations, uncertainty, and reversibility Trigger, reviewer role, evidence shown, decision, timestamp, and escalation

The National Institute of Standards and Technology (NIST) organizes implementation as Govern, Map, Measure, and Manage. Its AI Risk Management Framework is voluntary and was released on January 26, 2023. The NIST AI RMF Playbook provides implementation suggestions under those four functions.

Configure layered rate limits and budgets

Limit more than requests per minute

A single global requests-per-minute limit is easy to bypass and can let one tenant consume capacity needed by everyone else. Apply independent controls at the levels that match your architecture:

  • Per tenant or organization, so one customer cannot exhaust shared capacity.
  • Per user or session, to contain abusive or malfunctioning clients.
  • Per API key or credential, to detect leaked keys and enforce contractual quotas.
  • Per model and provider route, because quotas, prices, context limits, and latency differ.
  • Globally, to protect the service and keep an emergency reserve for operations.

Track token rate as well as request rate whenever the provider exposes token accounting. Ten short prompts and one very long context are not equivalent workloads. Count input and output tokens separately if that is how billing and capacity are calculated, and make the accounting definition part of the policy.

Add concurrency caps and queueing

Concurrency limits protect downstream systems from a burst that technically stays under a per-minute quota. Set caps for each tenant, route, and the whole service. Decide what happens when a cap is reached: reject immediately with a retryable response, queue work for a bounded period, or switch to a pre-approved reduced-capability path. A queue needs a maximum depth and age; otherwise it becomes an unbounded memory and latency problem. Return a clear timeout or overload result when the bound is reached.

Use budgets as hard stops

Enforce a token or monetary budget at the scope where runaway behavior would occur: request, workflow, user, tenant, model route, and billing period. A workflow budget should include retries, tool calls, retrieval expansions, and fallback calls, not only the first model request. When a hard budget is reached, stop generation or require approval rather than silently continuing. Emit an event so the owner can distinguish an intentional budget stop from a provider failure.

Choose values from measurements, not a universal default

Collect demand and latency data under normal peaks, known launches, batch jobs, and abuse scenarios. Compare the observed load with provider quotas and your latency objective. Then set a sustained limit, a burst allowance if needed, a concurrency cap, and an alert level below the point of customer impact. There is no authoritative universal numeric default for these values. Publish the chosen numbers, the measurement period, the assumptions, the owner, and the review date.

Back off without creating a retry storm

Retry only errors that are plausibly transient, such as transport interruptions or temporary capacity responses. Use exponential backoff with jitter, a maximum attempt count, and an overall deadline. The deadline must include time spent waiting, not merely model execution time. Do not retry policy refusals, malformed requests, invalid credentials, unsupported parameters, or application bugs; fix or route those conditions instead. A rate limiter should reduce undesirable actions and give monitoring time to detect a problem before significant damage occurs, as described in OWASP guidance.

Expose safe overload behavior

Return a machine-readable error class, a retry-after indication when appropriate, and a human-readable message that does not disclose secrets. Keep overload responses consistent across providers so clients do not learn provider-specific behavior. For internal jobs, persist the work item and resume only within its deadline; for interactive requests, prefer a bounded queue or a clear failure over an indefinite spinner.

Design privacy-aware, useful production logs

Separate operational metadata from sensitive content

Most incidents can be investigated from metadata. Store prompts, completions, uploaded files, and retrieved text separately from the operational event stream. Encrypt both in transit and at rest, restrict access by role, and require an explicit reason to view sensitive payloads. Redact credentials, authentication tokens, payment data, health information, and other regulated fields before they reach general-purpose logs. Define retention and deletion rules for each data class; an audit record can outlive a raw prompt, but neither should be retained indefinitely without a documented purpose.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Use a consistent event schema

For each model invocation or material workflow step, capture the following fields where applicable:

  • Request and trace identifiers, timestamp, environment, and service version.
  • A pseudonymous tenant or user identifier rather than a direct identity when operations do not require the identity.
  • Model name, model version, provider, route, and the policy or guardrail version applied.
  • Input and output token counts, latency components, status, timeout state, and normalized error class.
  • Safety, moderation, and policy results, including which rule caused a block or escalation.
  • Tool calls, tool outcomes, authorization context, and whether a side effect was attempted.
  • Retrieved-document identifiers or hashes, index version, and source metadata needed to verify provenance.
  • Fallback reason, original route, selected route, and the outcome after fallback.
  • Human-review trigger, reviewer decision, edits, escalation, and decision timestamp.
  • Cost estimate and the accounting basis used to calculate it.

Keep a correlation identifier across retrieval, model calls, tools, fallback attempts, and reviewer actions. This makes it possible to reconstruct one user-visible outcome without copying the full payload into every subsystem.

Monitor signals that indicate loss of control

Alert on error rate, latency, queue age, concurrency saturation, token and spend velocity, refusal and policy-block rates, fallback frequency, review backlog, and unusual changes in output or retrieval behavior. Establish separate thresholds for warning and response. A sudden increase in refusals can indicate a policy or prompt change; a sudden decrease can indicate a guardrail failure. Drift alerts should lead to a defined investigation, not an automatic rollback without checking whether traffic or user mix changed.

Review provenance and guardrails

NIST’s Generative AI Profile, NIST AI 600-1, published July 26, 2024, emphasizes reliability validation, provenance, source and citation verification, and regular review of guardrails. Use the logged model, retrieval, policy, and tool versions to reproduce a decision. If the system operates outside tested conditions, document the limitation and route the case to a safer mode or a human.

Build bounded fallbacks and failure handling

Classify the failure before choosing an action

Normalize provider and application errors into classes with different handling:

Error class Typical examples Permitted response
Transient transport or capacity Connection reset, temporary overload, gateway timeout Bounded retry with jitter, then an approved alternate route or degraded response
Provider quota or account limit Rate or quota exhaustion Respect the reset or route within an approved budget; do not retry rapidly
Policy or safety refusal Model blocks the requested content or action Do not bypass with another model unless that route has explicitly equivalent policy controls; explain the safe alternative
Invalid request Bad schema, unsupported parameter, context overflow Correct, truncate under a documented rule, or return a validation error
Application defect Serialization bug, authorization error, tool integration failure Stop the workflow, page the owner when material, and preserve diagnostic context

Retry only inside a deadline

For a retryable class, apply capped exponential backoff with jitter, a maximum attempt count, and one end-to-end deadline. Include all attempts in the request and spend budget. A retry that would exceed the user-visible deadline or workflow budget must be skipped. Never turn a refusal or invalid request into a retry loop.

Route only to a genuinely approved alternative

A fallback model is not automatically equivalent. Before production use, verify its data-access permissions, geographic processing constraints, safety and policy behavior, output format, tool permissions, quality for the task, and cost. Keep a route-specific capability declaration so the orchestrator can reject a fallback that cannot safely perform the requested operation. If the task requires a capability the alternate route lacks, choose a reduced-capability response instead.

Provide an honest degraded mode

When no safe model path remains, return a useful bounded result: retrieval-only facts with provenance, a deterministic template, a saved draft that requires approval, or a human handoff. Tell the caller what was not completed and whether retrying later is appropriate. Do not claim an action occurred when the tool call was never confirmed.

Use a circuit breaker and emit one complete event

Open a circuit when failures exceed a threshold demonstrated in testing for that route, then stop sending traffic for a defined cool-down. Probe cautiously before closing it. The threshold and cool-down are workload-specific; no universal values are established. Every fallback event should include the original error class, selected route, reason for selection, attempt count, latency, budget impact, and final outcome. This record supports incident review and prevents silent degradation from becoming normal.

Put human approval where consequences require it

Define risk tiers and triggers

Require an approval gate before an action is irreversible, high-impact, safety-sensitive, financial, legal, employment-related, or externally visible. Examples include sending a public or customer-facing communication, changing an account or entitlement, executing a transaction, making a consequential eligibility recommendation, deleting data, or calling a tool that changes a real system. A low-risk draft can often be automated; the same text sent to customers needs a different control.

Risk tier Typical treatment Minimum evidence for the reviewer
Low and reversible Automated execution with monitoring and an undo path Policy result, basic trace, and confirmation of scope
Moderate or externally visible Single human approval before release Proposed output, sources, uncertainty, tool plan, and policy checks
High-impact or irreversible Named qualified reviewer; dual control where required Complete evidence, alternatives, known limitations, authorization, and explicit consequence acknowledgement

OWASP LLM06:2025 recommends human-in-the-loop control so a human approves high-impact actions before they are taken, including manual review before externally visible communications.

Make the review decision meaningful

Show the reviewer the exact proposed action, affected records or recipients, supporting sources, retrieval dates or identifiers, policy and safety results, model and prompt versions, uncertainty indicators, and any fallback that occurred. Do not bury the evidence in a raw transcript. Give the reviewer distinct choices to approve, reject, edit, request more evidence, or escalate. A timeout must have a safe default, normally no execution.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Assign authority and expertise

Specify which role may approve each risk tier and what expertise is required. Separate the person who designs a workflow from the person who approves a consequential action when practical. Use dual control for especially consequential operations: two independent approvals, each recorded separately. If a reviewer lacks the required context, the interface should make escalation easier than an uninformed approval.

Preserve an immutable audit record

Record the recommendation shown, evidence and sources, policy results, reviewer identity or pseudonym, decision, edits, reason, timestamp, and resulting tool outcome. Protect the record from alteration and link it to the originating trace. NIST AI 600-1 also calls for documenting human domain knowledge used to improve system performance and for ongoing review of validity, provenance, and guardrails.

Operate the settings as a continuous lifecycle

Govern

Name accountable owners for limits, logging, routing, tools, and review queues. Define acceptable use, data handling, approval authority, incident severity, rollback authority, and change-control requirements. Keep a current inventory of providers and sub-processors so an ownership or geographic change does not bypass review.

Map

Document models, versions, providers, data classes, users, retrieval sources, tools, downstream systems, geographic boundaries, and failure modes. Map which outputs are merely informational and which can trigger side effects. Identify where a fallback changes quality, policy, data access, or legal exposure.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Measure

Test normal and stress conditions for quality, safety, latency, cost, refusal, fallback, queue behavior, and reviewer outcomes. Include provider outages, malformed inputs, prompt-injection attempts, oversized context, tool failures, and exhausted budgets. Validate that logs contain enough evidence to reconstruct an incident while redaction still works.

Manage

Remediate incidents, roll back unsafe changes, tune thresholds from observed data, close unused routes, and reapprove material modifications. Trigger a review after a model or provider change, prompt or policy update, retrieval-index change, new tool, traffic or user-population shift, abnormal drift, or repeated human override. Keep the previous configuration available for a controlled rollback.

Production readiness checklist

  • Limits exist at tenant, user, key, model, provider, and global scopes where applicable.
  • Both request and token rates are measured when token accounting is available.
  • Concurrency, queue depth, queue age, retry count, deadline, and spend are bounded.
  • Retryable and non-retryable errors are classified before routing decisions.
  • Fallback routes have documented capability, policy, data, geographic, quality, and cost checks.
  • A circuit breaker prevents repeated calls to a failing route.
  • Logs include trace, route, version, policy, token, latency, error, tool, provenance, fallback, review, and cost fields without unnecessary sensitive payloads.
  • Encryption, role-based access, retention, deletion, and redaction are tested.
  • Alerts cover reliability, latency, spend, refusal, policy, fallback, queue, review, and drift anomalies.
  • Human gates cover irreversible, high-impact, safety-sensitive, financial, legal, employment, and externally visible actions.
  • Reviewers see evidence, uncertainty, sources, and consequences, with approve, reject, edit, timeout, and escalation paths.
  • Every material change has an owner, test evidence, approval, and rollback plan.

What a defensible configuration looks like

A defensible production setup is not the one with the most restrictive numbers or the most verbose logs. It is the one whose limits are measurable, whose logs are useful without being indiscriminate, whose fallbacks preserve policy and capability boundaries, and whose human gates match the consequences of the action. Record why each value was chosen, test it under failure, and treat every material system change as a reason to re-evaluate the controls.
