Require human approval when a repair could delete data, weaken security, disrupt important services, affect an uncertain scope, or compromise incident evidence. An AI agent’s confidence is useful monitoring information; it is not permission.
Which server repairs are safe to automate?
Unattended repair is most defensible when the failure and fix are familiar, the action has a narrow scope, and the system can verify the outcome and stop if it makes matters worse. The service owner—not the agent—should approve the action class and set its limits in advance.
- Known procedure: The repair addresses a repeatable failure with a defined, tested response.
- Limited authority: The action targets a specific host, service, or rollout slice, using only the permissions it needs.
- Clear conditions: Deterministic rules specify when the action is allowed, what it can affect, and when that permission expires.
- Observable result: A health check can establish whether the repair worked, and a failed check halts further action.
- Recovery and interruption: Operators can contain or undo the change promptly and pause the automation.
- Acceptable consequences: The action will not expand privileges, erase evidence, disrupt a critical service, or make recovery materially harder.
Examples that may qualify after local validation include restarting one unhealthy stateless worker, retrying a known idempotent operation, or rolling back a recent deployment when predefined health thresholds fail. These are engineering examples, not universally approved actions; each service owner must accept the specific failure modes and blast radius.
When should a person approve the repair first?
Put a human checkpoint before actions with substantial, uncertain, or hard-to-reverse consequences. Australian Signals Directorate and partner agencies advise human approval for high-impact containment, disruption, recovery, or communications decisions, and specifically identify destructive or irreversible recovery such as system restoration or data rollback as requiring approval: Opportunities for AI in cyber defence.

- Deleting, overwriting, or restoring important data, or changing backups.
- Changing identities, credentials, permissions, firewall rules, network egress, or other security boundaries.
- Restarting, isolating, rebuilding, or rolling back a critical service when customers could be affected or an outage could result.
- Changing multiple hosts, regions, shared dependencies, or systems whose scope is unclear.
- Altering or suppressing logs and other evidence that may matter to incident response.
- Acting on an uncertain diagnosis, conflicting telemetry, a failed policy check, or an unfamiliar incident pattern.
Guidance on agentic systems likewise says the designers or operators should define when approval is needed, rather than allowing the agent to set its own threshold. It highlights review when mistakes could be costly, including system resets, network egress changes, or deletion of critical records: Careful adoption of agentic AI services.
How to move from recommendations to autonomous repair
Increase autonomy in controlled stages, not all at once. Australian guidance recommends phased deployment, progressively increasing access and autonomy, ongoing evaluation, and fail-safe escalation when an agent is uncertain. Microsoft also recommends clear boundaries, least privilege, approval for high-risk or irreversible actions, and a system-level way to pause or stop execution.

- Observe: Let the system diagnose and explain, but make no changes.
- Recommend: Have it propose the action, target, expected result, and rollback plan for an operator to assess.
- Approve each action: Require a person to authorize execution, then compare the result with defined health signals.
- Automate one narrow class: Permit a specific, pre-approved reversible action on limited targets under deterministic rules and monitoring.
- Expand only with evidence: Increase the action set or scope when continued evaluation shows safeguards work; retain the ability to withdraw permissions.
For production deployments, use progressive exposure: start with a limited rollout, check service health, and make each next phase conditional on those checks. Microsoft’s Azure Well-Architected guidance says to halt deployment and initiate recovery when issues are detected: Architecture strategies for safe deployment practices.
What controls should be in place before enabling repair?
- Explicit action policy: Maintain an allowlist specifying the permitted action, target, triggering conditions, limits, and expiration. Deterministic controls should block actions outside it. See Microsoft Learn’s guidance on reducing autonomous agentic AI risk.
- Least privilege: Give the repair identity only the tools and permissions needed for its approved task; control privileged calls separately.
- Operator-owned approval thresholds: Designers or operators define which actions need approval. The agent must not be able to waive the gate based on its own confidence.
- Defined health gates: Decide what “healthy” means before rollout, check between phases, and stop when problems are detected.
- Stop and recovery mechanisms: Provide a reliable pause or stop control and a tested way to recover from an unsuccessful change.
- Auditable records: Record the action, tools used, key decision inputs, any approval, and outcome so operators can reconstruct what happened.
- Engineering review: Apply normal change controls, testing, and traceability. The UK Home Office engineering standard requires qualified human review and approval before production for its departmental work; it is not a universal rule for every organization. How to assess a proposed repair
Use these questions to compare repair options or decide whether a particular action belongs on an automation allowlist. A high-impact answer, uncertain diagnosis, broad scope, or weak recovery path is a reason to add a human gate.

- Impact: What is the worst credible effect on availability, security, data, customers, or incident evidence?
- Reversibility: Can the change be undone reliably, and has the recovery method been tested?
- Scope: Is it limited to one instance, or could it cascade through shared infrastructure?
- Diagnostic certainty: Are the triggering signals clear and corroborated, or could bad telemetry or an attack provoke the action?
- Privilege: What access does execution require, and can it be narrowed or time-limited?
- Observability: Can monitoring detect a failed repair or a worsening service?
- Accountability: Is a named owner responsible for authorizing the action class and reviewing outcomes?
Common mistakes that make server repair automation risky
- Assuming past success grants blanket authority: Approval should cover named actions, conditions, and targets; reconsider it after failures or scope changes.
- Letting the agent choose its own approval threshold: The operator or system designer must decide where the gate applies.
- Using broad administrator credentials for convenience: Narrow permissions limit the damage a mistaken or compromised agent can do.
- Automating rollback without defining a safe state: A rollback can itself be disruptive or destructive. Specify its conditions and validate the resulting state.
- Skipping logs for routine fixes: Records support audits, incident response, and learning when a familiar action behaves unexpectedly.
- Expanding autonomy after a demo: Base expansion on continued operational evaluation, and reduce access when failures show safeguards are insufficient.
Meaningful oversight is not just a person nominally “in the loop.” Microsoft Learn describes it as giving users meaningful control to guide, correct, and interrupt autonomous behavior, especially when input is ambiguous, actions are high impact, or adversarial manipulation is possible.
