Get emergency help for back pain with new bladder or bowel problems, numbness around the genitals or anus, significant symptoms in both legs, chest pain, changes in sexual function or sensation, or pain after a serious accident. In the UK, the NHS advises calling 999 or going to A&E for these symptoms. Other symptoms call for urgent advice or a GP appointment, depending on how severe and persistent they are.
The routes and numbers below follow UK NHS guidance. MedlinePlus is a US federal health-information source; if you are outside the UK, use your local emergency and urgent-care services.
When should I go to A&E for back pain?
Under NHS guidance, call 999 or go to A&E if back pain occurs with any of the following:
- Pain, tingling, weakness or numbness in both legs.
- Loss of feeling around your genitals or anus.
- New difficulty urinating, loss of bladder control, or loss of bowel control.
- A change in sexual sensation or function.
- Chest pain.
- Pain that began after a serious accident.
The NHS says: “Do not drive to A&E. Ask someone to drive you or call 999 and ask for an ambulance.” NHS sciatica guidance includes this advice, and the NHS back-pain guidance lists the emergency warning signs.

When does back pain need urgent advice?
Contact NHS 111 or ask for an urgent GP appointment if your back pain comes with feeling hot, cold or shivery, or generally unwell. Seek urgent advice as well if severe pain starts suddenly or gets worse quickly. These symptoms warrant prompt assessment; they do not, on their own, establish a particular diagnosis. NHS guidance

Arrange a GP appointment if back pain has not improved after a few weeks of home treatment, interferes with everyday activities, or feels worrying or difficult to cope with. The NHS also advises contacting a GP about pain accompanied by any of these features:

- A lump or swelling, or a change in the shape of your back.
- Pain that does not improve with rest or is worse at night.
- Pain made worse by sneezing, coughing or straining.
- Pain in the upper back between the shoulders.

Is back pain with numbness an emergency?
It depends on where the numbness is and whether other symptoms are present. Numbness or weakness affecting both legs, or loss of feeling around the genitals or anus, appears on the NHS emergency list. New bladder or bowel dysfunction alongside back pain also needs emergency assessment under that guidance.

Cambridge University Hospitals NHS Foundation Trust identifies fever, recent infection, intravenous drug use, weight loss, night pain and a history of cancer as information important to tell clinicians. MedlinePlus also advises prompt contact for severe trauma, unexplained fever, cancer history, loss of urine or stool control, weakness or numbness, walking or balance problems, and pain lasting longer than four weeks.

No. Warning signs are prompts for assessment, not diagnoses. The NHS says serious causes of back pain are very rare. The NHS England National Low Back and Radicular Pain Pathway describes red flags as symptoms or signs associated with increased risk of serious underlying disease, while noting concerns about how accurately they identify it. It identifies suspected spinal cord neurology, impending cauda equina syndrome, major motor radiculopathy and suspected spinal infection as emergency spinal referral concerns; an isolated symptom does not prove any of these conditions.

What can I do while monitoring uncomplicated back pain?
If none of the warning signs above apply, NHS guidance says most back pain improves within a few weeks. It recommends staying active and continuing daily activities as you are able. An ice pack wrapped in cloth or a heat pack may help with pain, swelling, stiffness or muscle spasm. Ask a pharmacist or GP whether pain relief is suitable for you.
