Neither WhatsApp nor SMS is proven to work better for every clinic. The studies available here test different patients, message designs, schedules and outcomes; they do not provide a direct comparison between the two channels. For a clinic, the practical choice is the channel patients can receive and act on, measured in its own workflow.
What the studies show about WhatsApp
A randomized trial in hypertension follow-up
A 2024 randomized controlled trial recruited 388 patients with uncontrolled blood pressure from outpatient wards at two public sub-district hospitals in Punjab, India. Patients in the intervention group received WhatsApp messages three days and one day before a physician-requested follow-up visit. The messages combined a standard reminder with statements intended to debunk misconceptions; the control group received usual care without messages.
Attendance was 21.8% in the WhatsApp group and 19.6% in controls. The estimated effect was 2.2 percentage points (p=0.603), so the trial found no evidence that this enhanced intervention improved attendance. It compared WhatsApp with usual care, not SMS, and its result applies to this intervention and patient setting. Favaretti et al. (2024)
A large Buenos Aires analysis
A 2026 medRxiv preprint analyzed 475,214 eligible first appointments in the Autonomous City of Buenos Aires public health system, using data from June 2023 to May 2024. Its target-trial emulation compared WhatsApp reminders around 72 hours and/or 24 hours before visits with no reminder. The analysis reported lower no-shows for all active WhatsApp schedules; the largest estimated reduction was for a single reminder around 24 hours before the visit. The no-reminder comparison arm had a reported baseline no-show risk of 34.6%.

This is a large observational emulation, not a randomized trial comparing WhatsApp with SMS. It does not establish that WhatsApp beats SMS or that a 24-hour reminder is best for clinics elsewhere. Sanchez et al. (medRxiv, 2026)
What the studies show about SMS
Wording can affect results
In the first of two randomized trials at one NHS Trust in England, Hallsworth and colleagues tested SMS reminder wording. A message that stated the health system’s cost of a missed appointment had an 8.4% did-not-attend (DNA) rate, compared with 11.1% for the existing reminder. This shows that wording mattered in that study; it does not compare SMS with WhatsApp. Hallsworth et al. (2015)

A randomized study across 14 Clalit Health Services hospitals included 161,587 members receiving one of nine SMS formats. The strongest tested frame had a 14.2% no-show rate, compared with 21.1% for the control format. The finding concerns message framing within that health system, not SMS performance against WhatsApp. Huf et al. (2020)
Reminder cadence can matter
A pragmatic randomized study at Kaiser Permanente Washington compared one with two text reminders for primary care and mental health visits. The additional reminder was associated with a modest reduction in no-shows for both groups. It suggests that schedule and targeting can affect SMS outcomes, but does not identify a better channel. Why the figures cannot be ranked as a channel contest
The studies differ in population, health system, appointment type, reminder content, timing, baseline risk, comparator and outcome. Some report attendance; others report no-shows or cancellations. Those measures are not interchangeable, and a lower percentage in one study cannot be attributed to its messaging channel when the study conditions also differ.
The evidence supports a narrower conclusion: reminder design and delivery can matter, but it does not establish whether WhatsApp or SMS is more effective for clinics overall.

How a clinic can choose and evaluate a channel
Treat this as a local workflow decision rather than a universal technology choice. If both channels are feasible, pilot them with comparable appointment groups. Keep wording and timing as consistent as possible so that the channel is the main difference being assessed. If only one channel is practical, choose the one with better demonstrated reach and workflow fit among your patients, then measure its results.
- Patient reach and preference: Find out which channel patients use, can access and want for routine reminders rather than assuming one is more popular.
- Delivery and follow-up: Track delivery status and whether patients can confirm, reply, reschedule or cancel. Decide who will handle replies and how quickly.
- Timing and cadence: Test lead times that fit your appointments and patients. The schedules studied do not make any single timing universally optimal.
- Message clarity and access: Include the date, time, clinic identity and a simple route to change or cancel an appointment. Consider language needs and readable wording; the SMS trials show that framing can affect outcomes.
- Privacy and preferences: Respect patients’ communication preferences, limit sensitive details in reminders, and review local rules and messaging-provider terms.
- Operational fit and cost: Compare integration with clinic systems, staff effort, reply handling, delivery management and local costs. The evidence here establishes no universal cost or vendor advantage for either channel.
- Outcome definitions: Record attendance, advance cancellations, late cancellations and no-shows separately. These outcomes should not be collapsed into a single rate.
For a useful pilot, compare outcomes across similar appointment types and time periods, and include delivery, replies, patient complaints and staff effort alongside attendance. A channel that generates responses is not necessarily helpful if the clinic cannot process them.

Privacy: what U.S. HIPAA guidance says
The U.S. Department of Health and Human Services (HHS) says, “The HIPAA Privacy Rule permits health care providers to communicate with patients regarding their health care.” HHS also states that appointment reminders are part of treatment under HIPAA and may be made without separate authorization. This is U.S.-specific HIPAA guidance, not a general legal rule for every country or every messaging provider. Clinics should consider applicable local requirements and vendor terms, and avoid including more sensitive information than a reminder needs.
