The main established medicine for preventing light-triggered symptoms of erythropoietic protoporphyria (EPP) is afamelanotide, sold as SCENESSE. It is a 16 mg implant inserted under the skin by a trained healthcare professional every two months. Treatment also depends on managing light exposure: SCENESSE does not replace protective clothing, sunlight avoidance, or other precautions. For an active episode, cooling may ease symptoms, but there is no established medicine that reliably relieves EPP pain.
How EPP treatment is approached
EPP treatment has two distinct aims: reducing the chance of a phototoxic reaction and managing symptoms if one occurs. Afamelanotide is the principal evidence-supported drug for prevention in adults. Light-aware routines and physical barriers remain important whether or not someone uses the medicine. Options such as gradual light exposure or narrowband UVB have less certain evidence and should be discussed with a specialist.
Afamelanotide (SCENESSE): the main established drug option
Afamelanotide is a long-acting analogue of alpha-melanocyte-stimulating hormone. By activating the melanocortin-1 receptor, it increases production of eumelanin, the pigment that helps protect skin from light. A 2023 consensus guideline recommends it to prevent phototoxic symptoms and reports increased pain-free sun exposure and improved quality of life in adults.
Indication and schedule
In the United States, the FDA indication is to increase pain-free light exposure in adults with a history of phototoxic reactions from EPP. Each implant contains 16 mg and is inserted subcutaneously every two months by a healthcare professional trained and proficient in the procedure. Regulatory wording differs by region. The EMA describes SCENESSE as preventing phototoxicity in adults with EPP and discusses seasonal timing. Approval, access, where implants are administered, and reimbursement depend on the jurisdiction; check the local product information and discuss suitability with an EPP specialist.
What the trial result means
In a six-month study of 93 patients summarized by the European Medicines Agency, participants receiving SCENESSE recorded a mean 116 hours of direct sunlight exposure without pain, compared with 61 hours for placebo. The measure covered exposure between 10 a.m. and 6 p.m. These are group study results, not a prediction of an individual’s response. Safety and monitoring
The FDA label warns about hypersensitivity, including serious reactions, and notes that treatment can darken existing moles and freckles. It recommends regular full-body skin examinations. Ask the treating clinician how personal medical history, other medicines, skin changes, and monitoring should factor into treatment.
Light protection and practical changes
Reducing exposure is a core part of prevention. Consensus guidance recommends avoiding sunlight when possible and wearing opaque clothing. Practical measures can make this easier at home, in a car, at school, and at work:

- Consider car-window tinting to reduce light exposure.
- Discuss school or workplace accommodations and education about EPP.
- For sensitivity to indoor lighting, consider indoor lights that minimize blue light.
- For prolonged surgery, the guideline lists light filters limiting transmission in the 340–470 nm range as an option.
Does sunscreen help with EPP?
Sunscreen is not a substitute for avoiding light or covering the skin. Some patients report modest benefit from tinted or broad-spectrum products, including formulations with zinc oxide or titanium dioxide, which can block some far-UVA wavelengths involved in protoporphyrin activation. The consensus guideline says non-tinted or non-broad-spectrum sunscreen is not useful for preventing protoporphyria phototoxic symptoms.
Alternatives if afamelanotide is unavailable or unsuitable
A specialist may consider limited natural sunlight exposure increased gradually, or low-dose narrowband UVB phototherapy increased slowly, with the aim of encouraging the skin to build melanin. These are lower-certainty approaches: the consensus guideline says their effectiveness is unclear and not well established. They should not be treated as equivalent in evidence to afamelanotide or attempted without clinical guidance.

What to do during a phototoxic episode
Cold compresses or cooling devices may help manage symptoms during an episode. The consensus guideline reports no evidence of benefit for narcotic analgesics, oral or topical corticosteroids, antihistamines, acetaminophen, or NSAIDs for EPP phototoxic symptoms. Pain treatment remains an unmet need, so contact the treating team for advice tailored to the episode and the person’s circumstances.

Consensus guidance does not recommend the following for preventing phototoxic symptoms because evidence of benefit is insufficient or unclear: beta-carotene, cysteine, cimetidine, isoniazid, warfarin, quinacrine, oral zinc, N-acetylcysteine, vitamin C, omega-3 fatty acids, oral adenosine monophosphate, canthaxanthine, terfenadine, inosine, DTT and glycerol, pyridoxine, or hydroxyethylrutosides. They should not be presented as established EPP treatments.

Investigational treatment
Dersimelagon, an oral MC1R agonist, was under investigation for preventing protoporphyria phototoxicity in the 2023 consensus guideline. That guideline reported a completed Phase 2 study and an ongoing Phase 3 study at the time. Those details do not establish the trial’s current status or later authorization.
Choosing a treatment plan
The right plan depends on the goal—prevention or relief during an episode—as well as local authorization and access, treatment burden, monitoring, and individual circumstances. A clinician familiar with EPP can help weigh these factors and make sure light-protection measures fit everyday settings.
