Newer prescription obesity medicines can help some adults lose weight, but eligibility, likely benefit, side effects, cost, and the prospect of long-term use all matter. In the United States, semaglutide and tirzepatide are among the main newer options; older medicines remain available, and retatrutide is not an FDA-approved treatment. Medication is one part of care—not a substitute for a clinician’s guidance, nutrition support, physical activity, or other appropriate treatment.

Who may be considered for weight-management medication?

The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) describes common adult criteria for prescription weight-management medication as a body mass index (BMI) of at least 30, or at least 27 with a weight-related health problem. These criteria are a starting point, not a prescription: a high BMI alone does not mean medication is suitable for everyone.

A clinician can consider the person’s health conditions, other medicines, family medical history, likely benefits and side effects, cost, and goals. Tell the clinician about prescription and over-the-counter medicines as well as herbal products. Weight-management medicines should not be used during pregnancy or when planning pregnancy, according to NIDDK.

Which treatments are available, and what is new?

NIDDK’s U.S. overview lists six FDA-approved medicines for long-term weight management: orlistat, phentermine-topiramate, naltrexone-bupropion, liraglutide, semaglutide, and tirzepatide. It also identifies setmelanotide for a limited group of rare genetic disorders. These options differ in how they work, their risks, and their suitability for a particular person; the list is not a substitute for checking current prescribing information.

The FDA identifies Saxenda (liraglutide), Wegovy (semaglutide), and Zepbound (tirzepatide) as GLP-1-based medicines approved for obesity or overweight. GLP-1 medicines mimic a gut hormone involved in post-meal blood glucose, appetite, and food intake. Tirzepatide acts on both GIP and GLP-1 receptors.

Tirzepatide An FDA-approved medicine for obesity or overweight; it acts on GIP and GLP-1 receptors. ACP’s April 2026 living guideline suggests tirzepatide, alongside semaglutide, as a first-line pharmacologic option for nonpregnant adults with obesity, together with lifestyle modification.
Other established medicines NIDDK lists orlistat, phentermine-topiramate, naltrexone-bupropion, and liraglutide for long-term weight management; setmelanotide is for a limited group with rare genetic disorders. Whether an option fits depends on the person’s health, other medicines, risks, preferences, and access.
Retatrutide A triple agonist of GIP, GLP-1, and glucagon receptors studied in a phase 3 trial published September 29, 2026. It is not an FDA-approved medicine on the evidence described here and cannot be used in compounding under federal law.

On March 19, 2026, the FDA announced approval of Wegovy HD, a 7.2 mg semaglutide injection, for weight loss and long-term maintenance in certain adults with obesity or overweight plus a weight-related condition. The agency said supporting clinical data showed additional average weight reduction over previously approved doses, with a safety profile consistent with known semaglutide effects. Among participants with obesity and type 2 diabetes, the higher dose had similar HbA1c lowering to lower doses. These findings do not establish that the higher dose is the best choice for every patient.

What benefit can medication add?

NIDDK reports that adults using prescription medicines with a lifestyle program lost, on average, 3% to 12% more of their starting body weight after one year than adults in a lifestyle program without medication. That broad estimate combines different medicines and people; it is not a head-to-head comparison of today’s individual treatments, and an individual’s result may differ.

In April 2026, the American College of Physicians’ living guideline suggested semaglutide or tirzepatide as first-line pharmacologic options alongside lifestyle modification for nonpregnant adults with obesity. The guideline also lists lower-certainty second- and third-line options. Its recommendation is not a ranking that overrides a person’s medical history or preferences.

When discussing options, ask how evidence applies to your circumstances and consider:

  • Whether the medicine is indicated for your health context and whether any contraindication or warning applies.
  • Expected benefit in a comparable population and over a comparable period, without treating results from separate trials as direct comparisons.
  • How the medicine is taken, its side effects, and its effect on relevant health conditions.
  • Cost, insurance coverage, access, and whether the treatment fits your goals and preferences.
  • What follow-up and broader lifestyle or behavior support would be part of the plan.

What side effects and warnings matter?

Wegovy HD and semaglutide

The FDA’s March 2026 Wegovy HD announcement lists nausea, vomiting, diarrhea, constipation, and abdominal pain among common adverse reactions. Altered skin sensation occurred more often with the higher dose; the FDA said it generally resolved spontaneously or with dose reduction and that the agency was investigating further.

Wegovy HD carries a boxed warning about potential thyroid C-cell tumors based on findings in rodents. It should not be used by people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN 2). This is a product-specific warning; other medicines have their own prescribing information, side effects, contraindications, and warnings.

Suicidal thoughts and behavior: what the FDA review found

On January 13, 2026, the FDA said its comprehensive review found no increased risk of suicidal ideation or behavior associated with GLP-1 receptor agonists and requested removal of that warning from the labels of Saxenda, Wegovy, and Zepbound. The agency’s review included a meta-analysis of 91 placebo-controlled trials involving 107,910 patients and a retrospective Sentinel cohort of 2,243,138 users. This finding addresses a specific reported outcome; it does not mean every possible risk has been ruled out or replace the warnings for an individual product.

How long might treatment continue?

Duration depends on whether the medicine is helping and whether serious side effects occur. NIDDK says some people may be advised to continue indefinitely and that people probably regain some weight after stopping. Do not start, change, or stop treatment on your own: discuss the benefits, harms, and next steps with the prescribing clinician.

Medication works best as part of a broader plan. NIDDK says these medicines do not replace physical activity or healthy eating habits. A clinician can help determine what lifestyle, behavior, or other support is suitable alongside a prescription.

What should patients know about unapproved and compounded products?

FDA says unapproved versions of semaglutide and tirzepatide are not reviewed by the agency for safety, effectiveness, or quality before marketing. Compounded medicines may be appropriate only when a patient’s medical need cannot be met by an FDA-approved drug or the approved drug is not commercially available; compounded medicines remain unapproved. They should not be treated as equivalent substitutes for approved products.

The FDA flags dosing errors, warm shipments, fraudulent labels, and multidose vials as concerns with unapproved GLP-1 products. It also says retatrutide and cagrilintide cannot be used in compounding under federal law. Do not rely on products sold online as “research” products as a way to obtain treatment.

Are devices another kind of obesity treatment?

FDA-regulated weight-management devices include an oral, removable palatal space-occupying device and an ingested transient space-occupying material. These are clinical interventions, not casual consumer accessories. FDA notes that some devices require assessment for eating disorders. Liquid-filled gastric balloons have been associated with serious complications, including death, perforation, pancreatitis, and spontaneous hyperinflation; anyone considering a device should discuss its risks and follow-up with a qualified clinician.

How to make a treatment decision with a clinician

  1. Review eligibility and health context. Discuss BMI alongside weight-related conditions, pregnancy plans, personal and family history, and other medicines.
  2. Compare relevant options. Ask about expected benefit, route and dosing, side effects, contraindications, and effects on other health conditions. Avoid comparing percentages taken from separate trials as if they were a direct test between drugs.
  3. Discuss practical access. Check the likely cost and insurance coverage for the specific treatment and ask what happens if access changes.
  4. Agree on follow-up. Establish how benefit and side effects will be reviewed, what lifestyle or behavior support is appropriate, and how to handle concerns or a possible treatment change.
