There is no universal field list: choose events and context according to the system’s purpose, risks, deployment setting, and applicable rules. For EU AI Act-covered high-risk systems, the Act adds specific logging and retention requirements; those requirements do not automatically apply to every AI system.
What an AI safety audit log needs to show
A useful log is more than a record of model inputs and outputs. It is a traceable account of system behavior that supports investigation of safety issues, operational monitoring, and documentation of decisions and responses. A reviewer should be able to connect an event to the system state at the time and understand what actions came before and after it.
Start by defining the questions the log must answer: what happened, when, to which system, under what relevant conditions, and what followed? The answer will differ for a medical-support tool, a customer-service assistant, and an automated decision system. The fields below are practical design options, not a universal legal checklist.
Core event and system context
- Event identity: a stable event identifier, event type, timestamp, and a correlation, session, or request identifier where needed to connect related records.
- System state: the AI system and the relevant model, configuration, policy, and deployment versions. Without this context, an output may be difficult to interpret after the system changes.
- Trigger and input: the action that initiated the event and the relevant input, or a protected reference to it. Keep only what is needed for the audit purpose; redact or minimize personal and sensitive data when full payloads are unnecessary.
- Output and status: the result, decision, confidence or status fields that matter to the use case, including how unavailable, incomplete, or failed results are represented.
Events that materially shape behavior
- Dependencies: tool calls, external data sources, safety filters, policy checks, and system errors when they affect the behavior under review.
- Human actions: review, approval, override, escalation, and incident-response steps, with an actor identity or role only where lawful and necessary.
- Changes: model, prompt, configuration, access, or logging-rule changes that could affect system behavior or how a record should be interpreted.
Choose event types from the system’s risks and intended use rather than logging indiscriminately. NIST assessment procedures treat defined event types—and review and update of those selections—as relevant controls. Revisit the schema when the system, deployment context, or risks change. NIST SP 800-171A Rev. 3 provides assessment procedures for audit-record generation, content, retention, timestamps, review, and failure response.

What the EU AI Act requires—and what it does not
The EU AI Act’s Article 12 requires high-risk AI systems within the Act’s scope to technically allow automatic recording of events over the system’s lifetime. Its purpose is traceability appropriate to the intended use, including identifying risk situations, supporting post-market monitoring, and monitoring system operation. This is not a field-by-field schema for every AI system. Regulation (EU) 2024/1689, Article 12 is the primary source; the cited EUR-Lex text is consolidated through 2026-07-27.
Article 12 specifies a narrower minimum for certain remote biometric identification systems: logs must include the use’s start and end times, the reference database checked, the input data that produced a match, and the identification of people involved in verification. Do not treat those special fields as the general minimum for all AI systems.

The NIST AI Risk Management Framework is voluntary, not an AI logging law. NIST describes it as intended to improve the incorporation of trustworthiness considerations into the design, development, use, and evaluation of AI products, services, and systems. How long to keep AI audit logs
Set retention by record category and purpose, then check the jurisdictions, sector rules, data types, and organizational role that apply. The available facts do not establish one general retention period for AI logs outside the EU AI Act provision below; a definitive schedule for another jurisdiction requires local legal and regulatory analysis.

Under Article 19 of the EU AI Act, providers of covered high-risk AI systems must retain automatically generated logs that are under their control for a period appropriate to the system’s intended purpose and for at least six months, unless applicable Union or national law provides otherwise, particularly data-protection law. This six-month minimum is scoped to those logs and systems; it should not be applied automatically to every AI deployment. Regulation (EU) 2024/1689, Article 19
Do not confuse log retention with provider documentation retention. Article 18’s ten-year requirement concerns specified technical and compliance documentation, while Article 19 separately addresses automatically generated logs. Make the schedule workable
- Define a period for each record category and the purpose it serves.
- Account for applicable privacy limits, sector obligations, and legal holds; do not retain sensitive payloads merely because storage is available.
- Specify when records are deleted or placed on hold, who can authorize exceptions, and how deletion is verified.
- Document which requirements are legal obligations and which are internal risk-control decisions.
Protect the log and test its operation
A complete-looking record is not useful if it can be silently altered, cannot be connected to the right system version, or was never generated because a pipeline failed. Define controls for who may write, read, export, and administer logs, and protect records against unauthorized change or deletion. Monitor pipeline health, detect gaps or dropped events, and alert on logging failures, clock problems, and storage limits.
Use a consistent timestamp convention, such as UTC, and preserve enough information to correlate records across services. Define how reviewers search, analyze, and export evidence for an incident, and what response is required when logging stops or becomes unreliable. NIST SP 800-171A Rev. 3 identifies audit-record generation, policy-consistent retention, timestamp generation, review and analysis, and response to logging failures as assessment concerns. Those procedures are useful design guidance, not AI-specific legal obligations.

How to evaluate a logging design or platform
When choosing an implementation, compare it against the system’s documented needs rather than a generic feature checklist.

These are decision criteria, not a ranking of products. A strong design is one that reliably produces the records needed for the system’s risks while limiting unnecessary data collection and supporting applicable retention rules.
