Stop further execution, contain the capability involved, and establish whether the call actually caused a side effect. A blocked proposal is different from an executed write, payment, administrative change, or message sent outside your organization. Use your organization’s incident plan and applicable sector- or jurisdiction-specific escalation process; there is no universal reporting deadline for every agent incident.

First determine what happened

Classify the call by execution status before deciding how far to contain or recover. A model’s later warning or refusal does not reverse an action that already ran.

  • Proposed: The agent requested a tool action, but the execution component did not run it.
  • Denied: A policy, approval gate, or tool rejected the call before it caused an effect.
  • Executed: The tool ran. Determine what changed, who or what was affected, and whether the action triggered further activity.

Then consider the action’s impact and reversibility (for example, read versus write, destructive, financial, administrative, or externally visible), the authority exposed (one tool versus a broader identity or credential), and any evidence of downstream effects. These distinctions help determine whether to pause a run, isolate an integration, revoke credentials, remediate a change, or escalate further. OWASP’s AI Agent Security Cheat Sheet recommends auditing tool attempts and outcomes.

Immediate response checklist

  1. Stop further execution. Pause or terminate the active run at the application or orchestration control point. If your system has an emergency stop, use it as directed by your runbook. OWASP recommends interruption and rollback controls, and the U.S. Department of Energy’s GEAR guidance calls for a stop, rollback, and incident-response plan. DOE GEAR: AI Security and Safety
  2. Contain the capability involved. Disable or isolate the affected tool, service, job, or connected equipment as appropriate. Revoke credentials that may have been exposed. If the agent’s permissions extend beyond the affected tool, consider whether the wider identity or execution boundary also needs containment. OWASP’s prompt-injection guidance covers controls for agent capabilities and tool use.
  3. Block a repeat at the execution boundary. Enforce authorization in the component that executes the action, not only in the model’s instructions. Deny unknown tools and invalid or unapproved calls by default; scope permissions to the task and actor, and use separate read and write credentials where possible. An agent request is not proof that the initiating user is authorized. See OWASP’s agent security controls and its AI Agent and MCP general controls.
  4. Preserve a useful timeline. Record the agent and session identifiers, tool and target, normalized parameters, timestamp, effective permissions, approval decision, relevant inputs and outputs, and downstream actions. Retain request and response records under your organization’s data-handling policy. Avoid copying secrets or sensitive information into plain-text logs; the audit trail must not create another exposure. OWASP’s LLM06:2025 Excessive Agency guidance discusses logging and controls.
  5. Assess impact and scope. Identify affected records, users, systems, and external recipients. Look for repeated calls, chained actions, and possible credential exposure. The exact assessment depends on the application and your incident plan; the available guidance supports auditing attempts and outcomes, not one universal impact procedure.
  6. Review authorization and the failure path. Check the initiating actor and session, tool and operation allowlists, argument validation, delegated identity, approval binding, credential scope, and whether untrusted external content or a manipulated tool description influenced the call. Do not treat a confirmation flag or the model’s confidence as authorization.
  7. Recover deliberately. Use the established recovery process to roll back or remediate changes. Restore tool access only after addressing the failed policy or execution control, then monitor for recurrence. OWASP recommends repeatable abuse-case and regression testing after material changes to prompts, tools, memory, retrieval, policies, or providers. OWASP LLM06:2025
  8. Escalate according to actual impact. Follow your internal security, privacy, safety, and service-owner procedures. External reporting obligations depend on jurisdiction and sector; the sources do not establish one regulator or deadline for every incident.

For high-impact actions, bind approval to the exact call

Destructive, financial, administrative, or externally visible actions warrant independent policy checks and approval tied to the specific action. Where appropriate, use replay protection and short-lived authorization. A generic “confirmed” state is not enough if it is not bound to the current actor and exact tool call. OWASP’s AI Agent Security Cheat Sheet states: “A user_confirmed flag is insufficient: the component must verify that the approval belongs to the current actor and exact tool call, remains valid, and has not already been consumed.”

After containment, verify the control that failed

Before restoring autonomy, test the relevant boundary with repeatable abuse cases and regression checks. OWASP recommends retaining evidence of the tested agent version, model provider, tool policy, retrieval configuration, abuse cases, and observed approval, denial, timeout, or circuit-breaker behavior. This makes it possible to verify that the change prevents the unsafe call without silently removing necessary controls elsewhere.

Why untrusted content matters

An unsafe call can result from weak authorization or from an agent being steered by untrusted input. NIST describes the security boundary at issue: “AI agent hijacking is the latest incarnation of an age-old computer security problem that arises when a system lacks a clear separation between trusted internal instructions and untrusted external data”. The statement appears in NIST’s January 2025 technical blog, “Strengthening AI Agent Hijacking Evaluations.” Treat content from external sources as data, not authority to expand what the agent may do.
