Difficulty balancing on one leg by itself does not identify a health problem. If the trouble is sudden, seek emergency help; if it is recurring, worsening, or causing falls, contact a health professional. Avoid testing or practicing unsupported if you feel unsteady.

First, check whether the problem is sudden

Sudden trouble walking, dizziness, loss of balance, or lack of coordination can be a stroke symptom. In the United States, call 9-1-1 immediately if this happens, particularly if there is also sudden vision trouble, numbness or weakness on one side, confusion, difficulty speaking, or a severe unexplained headache. When to contact a health professional

Arrange a conversation with a clinician if your balance has changed, the difficulty keeps recurring, you feel dizzy or unsteady, or you have fallen. A single-leg stand is not a diagnosis: balance draws on multiple systems, and difficulty can have many possible contributors, including medicines, inner-ear problems, vision, nerves, circulation, strength, or other health conditions. A clinician may ask when the problem began, what it feels like, whether you have fallen, and what medicines or health conditions may be relevant. They can examine you and choose tests based on what they suspect. Possible balance evaluations include hearing, blood, imaging, eye-movement, or movement-response tests; not everyone needs the same tests. MedlinePlus: Balance Tests

Describe what you are experiencing

Rather than focusing only on how long you can stand on one leg, note what happens in everyday situations. The National Institute on Aging suggests questions such as “Do I feel unsteady?” and “Do I lose my balance and fall?” You can also describe whether you feel dizzy or as if the room is spinning, or feel as if you are moving while sitting or standing still. These sensations and a difficulty with a single-leg pose are not interchangeable, but clear details can help a clinician understand the change. Do not self-test or exercise unsupported if you are unsteady

If you feel unsteady, do not keep trying to stand on one leg or practice standing balance without support. If balance exercises may be appropriate for you, ask a health care provider what to do and start slowly. Reduce fall hazards while you seek advice

Practical measures can make moving around safer, but they do not replace assessment of a new or ongoing balance problem. The National Institute on Aging suggests nonskid, rubber-soled, low-heeled shoes and avoiding walking in the dark. Handrails, a cane, or a walker may help some people; whether an aid is suitable depends on individual needs. If you are unsure what support to use, discuss it with a health professional. Falls are a particular concern for older adults: the National Institute on Aging reports that more than one in four people age 65 or older fall each year.
