An AI agent audit trail should let an independent reviewer reconstruct a task from its start through the agent’s decisions, tool calls, and effects in external systems. Record who or what initiated the task, which agent and components acted, what data and tools they touched, what authorization and approval applied, what succeeded or failed, and what changed. A transcript of the agent’s final response alone cannot show that chain.
What to record for each meaningful event
Use a shared task, session, or workflow identifier to connect events across agents and services. For each significant step, capture the following information:
- Event and time: Event type, precise timestamp, duration where relevant, and the correlation ID. Preserve event ordering across services, with enough timestamp precision to interpret sequence.
- Actor and attribution: The requesting person or upstream service, agent identity and instance, relevant model or deployment version, and tool or service identity. Also record the downstream principal or credential context used for an action. Distinguish the agent from the human or service whose authority it is using.
- Location and target: The source system, destination or target resource, tool name, and relevant object or data location.
- Action and context: Normalized action parameters, the input or retrieved context needed to understand the action, its result, and relevant agent or workflow state changes. Capture enough to investigate without indiscriminately retaining credentials, secrets, or personal data.
- Authorization and control: The policy or permission rule evaluated; whether the action was allowed, denied, or held for approval; the reason; and approval identity and time. For action-bound approval, include the approved target, normalized parameters, and expiry. Record blocked attempts as well as executed actions.
- Outcome and errors: Whether the action succeeded or failed, its downstream effect, relevant errors or exceptions, and the status of any recovery, rollback, or compensation.
- Evidence handling: The record schema and version, integrity or tamper-evidence metadata, retention class, access history for sensitive records, and references to related evidence. Keep the authoritative audit store isolated from the untrusted agent runtime, and define what the system does if logging is unavailable.
This is a practical record checklist, not a universal prescribed schema. NIST SP 800-171 Rev. 3 sets a general audit-record baseline of event type, time, location, source, outcome, and associated identities; it also addresses retention under policy, response to logging failures, review and correlation, and preservation of original content and time ordering. It is not an AI-agent-specific standard. NIST SP 800-171 Rev. 3 was published in May 2024.
Why a transcript is not an audit trail
A transcript can show what an agent said, but it may not establish which tool it called, what authority the call used, what data it accessed, whether a policy check permitted it, or what changed in a connected system. A useful trail links initiation, agent and tool activity, authorization evidence, and external effects into a sequence that can be independently examined.

OWASP’s AI Agent Security Cheat Sheet calls for “clear audit trails of agent decisions and actions.” Its guidance also recommends action-bound approvals for high-impact operations, independent validation by a policy or execution component, and fail-closed behavior when audit logging fails. OWASP AI Agent Security Cheat Sheet does not prescribe a universal field schema.
Extend coverage across the agent workflow
The Cyber Security Agency of Singapore’s 2026 Securing Agentic AI addendum describes monitoring across models, databases and files, memory, agents, tools, MCP interactions, agent communications, and external actions. Its suggested log information includes actions, inputs and outputs, state changes, errors, timestamps, duration, and workflow identifiers. It also advises considering privacy rules when recording inputs. Securing Agentic AI is informational community-driven guidance, not a mandatory, prescriptive, or exhaustive standard; its version history lists a public-consultation release dated 2025-10-22 and version 1.0 as TBA.

For a multi-step or multi-agent workflow, the practical implication is to connect records across components rather than treating each tool call as an isolated event. Capture enough context to explain what happened at each handoff, while limiting sensitive information to what is necessary for investigation and accountability.
Protect the audit evidence and plan for logging failures
- Separate evidence from execution: The agent runtime should not be able to rewrite or erase the authoritative audit records it produces.
- Control access: Restrict access to sensitive audit data and retain access history where appropriate.
- Set retention deliberately: Apply a defined retention policy that reflects legal, privacy, security, operational, and incident-response requirements. The cited guidance does not establish one universal retention period.
- Define failure behavior: Decide and document what happens when the logging system is unavailable. OWASP recommends fail-closed behavior; NIST SP 800-171 Rev. 3 says the response to audit-logging failure should be defined by the organization.
- Review and correlate: Ensure records can be searched and connected across services to support investigation, not just collected.
There is no universal rule in the cited guidance to store every full prompt or all retrieved content. Logging those materials may help explain an event, but can also capture personal data, secrets, or sensitive business information; make the choice under the organization’s applicable requirements.

How to evaluate tracing and logging tools
Assess implementation choices against the full evidence chain, not just whether a product can display a trace. Compare coverage, attribution, controls, and record handling separately:

| Identity and authorization | Whether the record preserves the agent identity, downstream principal, policy decision, reason, and approval evidence. |
| Correlation and reconstruction | Whether shared identifiers, timestamps, ordering, inputs or context, outcomes, errors, and state changes let a reviewer follow the workflow. |
| Integrity and isolation | Whether authoritative records are protected from changes by the agent runtime and include suitable integrity metadata. |
| Sensitive-data handling | Whether you can limit captured content and control access to sensitive records. |
| Retention and export | Whether retention can follow your policy and records can be preserved or exported for review. |
| Failure and review | Whether logging failures are surfaced and the system supports the required response, review, and correlation workflows. |
Tracing products can improve visibility without enforcing authorization or providing durable, tamper-resistant audit storage. The Singapore addendum names examples including Langfuse, LangSmith, OpenLLMetry, Helicone, and cloud-provider monitoring tools, but does not rank them or establish that each meets every audit requirement. Evaluate the functions you need rather than treating a trace view as proof of a complete audit trail.

What the guidance does—and does not—establish
NIST’s SP 800-92, Guide to Computer Security Log Management, published on 2006-09-13, is general enterprise log-management background rather than an AI-agent-specific manual. NIST’s AI Risk Management Framework, released on 2023-01-26, provides voluntary governance context; NIST says AI RMF 1.0 is being revised. Neither source supplies a mandatory, universal agent-audit schema.
The cited sources do not establish an ideal number of audit fields, a universal retention duration, or a benchmark proving one audit design is more effective than another. Treat the checklist as a way to build a reviewable evidence chain, then set field-level and retention choices according to your system’s risks and applicable obligations.
