NEPSI—the National ePrescribing Patient Safety Initiative—was a private-sector coalition announced in January 2007 to give U.S. physicians no-cost access to electronic prescribing. Its named service was Allscripts’ web-based eRx NOW. Launch coverage described free physician access, online delivery, drug-interaction checking and pharmacy connectivity, but the historical sources reviewed here do not establish whether NEPSI or eRx NOW remains available today.

What was NEPSI?

NEPSI was announced as a national effort to expand electronic prescribing among physicians. The coalition presented e-prescribing as both a patient-safety intervention and a way to accelerate broader health-information-technology adoption.

Allscripts supplied the web-based eRx NOW application. Wolters Kluwer Health was identified as the source of drug-interaction checking, while SureScripts supplied electronic connections to pharmacies, according to January 2007 launch reporting in Fierce Healthcare.

Allscripts chief executive Glen Tullman described the central objective this way: “our first objective is to equip every physician in the United States with electronic prescribing software that is absolutely free of charge.”

What free software did the initiative offer?

Allscripts eRx NOW

eRx NOW was described as an online service rather than software that physicians installed locally. A March 2007 Clinician.com news brief reported that it required no download or new hardware, involved minimal training and stored information remotely in a secure location. Those are launch-era descriptions of the program, not findings from independent testing.

Safety and pharmacy functions

The planned service combined prescription entry with medication-safety and transmission functions:

  • Drug-interaction checking: provided through Wolters Kluwer Health.
  • Electronic pharmacy connectivity: provided through SureScripts.
  • Web access: intended to avoid a local software download and additional hardware, as reported at the time.

The sources do not provide a complete feature list, clinical-workflow specification, security audit or performance evaluation.

Who was behind the free e-prescribing initiative?

The counts of regional supporters were not consistent. One 2007 account named 13 organizations, while the AHRQ evaluation report published in April 2007 listed 12 regional healthcare organizations. These figures describe the organizations identified by each source during the launch period, not a verified total for the initiative’s entire life.

How was the initiative funded?

MDedge reported an estimated $100 million in private-stakeholder funding over the first five years. The physician-facing software was presented as free. The estimate was a launch-era projection; it does not establish the coalition’s eventual spending, later financing or any current pricing.

The offer was therefore “free” from the participating physician’s point of view, while companies and health organizations were expected to support the initiative collectively.

Why did NEPSI focus on medication safety?

The safety case relied on a historical estimate attributed to the Institute of Medicine. In a 2008 announcement, the Centers for Medicare & Medicaid Services said the IOM had reported in 1996 that more than 1.5 million Americans, including an estimated 530,000 Medicare beneficiaries, were injured each year by drug errors.

That figure is a 1996 estimate quoted by CMS in 2008, not a current national measurement. It explains the rationale used to promote electronic prescribing at the time: replacing handwritten or disconnected processes with electronic checks and direct pharmacy transmission.

Family physician Nancy Dickey, then president of the Health Science Center at Texas A&M University, summarized the concern in the launch coverage: “As a practicing physician, I find that unacceptable.”

What did “free” mean for doctors?

In the launch announcements, “free” referred to physician access to the e-prescribing software. It did not mean that every surrounding clinical, hardware, connectivity or organizational cost was eliminated. The coverage specifically emphasized no new hardware and no local download, but it did not publish a complete cost schedule for practices, pharmacies or other participants.

Nor did the reports establish eligibility rules, prescribing-volume limits, support terms, integration with existing electronic health records or handling of controlled substances.

Was NEPSI or eRx NOW still available?

The 2007 launch articles, the April 2007 AHRQ evaluation report and the 2008 CMS announcement document the initiative’s announcement and early context. They do not establish whether NEPSI or eRx NOW continued operating, what service replaced it, or what terms applied after the rollout period.

Consequently, NEPSI should be understood as a historical launch-era coalition promise—not as a currently confirmed free e-prescribing product. A practice evaluating software today would need current documentation for interoperability, controlled-substance prescribing, pharmacy coverage, medication history, interaction checking, security and compliance, eligibility, support and total cost.
