It can carry cancer-driving genes and regulatory DNA, and its unusual inheritance during cell division may help tumour cells vary and evolve. ecDNA is a feature of tumour biology—not a cancer diagnosis or, by itself, a treatment decision.
What ecDNA is—and what it is not
Most of a cell’s DNA is packaged into chromosomes. Cancer-associated ecDNA consists of circular DNA elements found separately from those chromosomes, in the cell nucleus. These elements can carry oncogenes—genes that promote cell growth when overactive—as well as DNA sequences that regulate gene activity. A 2024 study describes individual ecDNA elements as typically greater than 500 kilobases, but that is not a size that applies to every element. The study’s description of ecDNA provides this context.

Extrachromosomal circular DNA is a broader category; not every circular DNA molecule outside chromosomes should be treated as the cancer-associated ecDNA discussed in cancer research. The term ecDNA here refers to the large, gene-containing elements studied in tumours.
How ecDNA can change tumour cells
It can increase the number of cancer-driving genes
When ecDNA carries an oncogene, a tumour cell may have many copies of that gene. More copies can contribute to increased gene activity, although the effect depends on the gene and the tumour’s biology.

It can be passed unevenly as cells divide
Chromosomes are distributed through an organized process when a cell divides. ecDNA is inherited outside the chromosomes and can be distributed unevenly between daughter cells. As a result, cells within the same tumour may end up with different amounts of ecDNA and different levels of the genes it carries.
Its circular structure can affect gene regulation
ecDNA can bring genes and regulatory sequences into interactions that would not occur in the same way on a chromosome. ecDNA hubs—groupings of ecDNA molecules—are one proposed way these elements can influence gene activity. These mechanisms, along with differences in copy number, can give tumour cells ways to change as a cancer evolves. How common is ecDNA in tumours?
In a 2024 analysis of 14,778 patients and 39 tumour types from the Genomics England 100,000 Genomes Project, researchers reported ecDNA in 17.1% of tumour samples. That percentage describes the samples and methods in that study; it is not a prevalence estimate for every cancer population or every individual cancer type. Differences in which tumours are studied and how ecDNA is detected can affect comparisons between studies. The 2024 study reports its cohort and findings.
What ecDNA findings may mean clinically
In that same cohort, ecDNA detection was associated with tumour stage, metastases, prior treatment, and shorter overall survival. These are group-level associations, not proof that ecDNA caused each outcome or a prediction of what will happen to a particular patient. The analysis also reported ecDNA carrying immunomodulatory and inflammatory genes, and an association between those elements and reduced tumour T-cell infiltration. That finding is a research result, not an established biomarker for choosing immunotherapy or another treatment.

Can ecDNA make cancer resistant to treatment?
ecDNA may contribute to tumour evolution and treatment resistance by allowing some cells to carry or express different amounts of cancer-related genes. The 2024 cohort analysis found ecDNA more often after targeted and cytotoxic treatment, but that association does not establish that treatment created ecDNA or that ecDNA explains resistance in every case. Cancer response depends on many features beyond ecDNA.

The sources cited here do not establish a universal clinical standard for testing tumours for ecDNA, nor do they show that ecDNA alone determines therapy. A tumour’s molecular findings must be interpreted in the context of its cancer type, other test results, and the patient’s clinical situation by an oncology team.
The National Cancer Institute reported on December 5, 2024, that BBI-355—a research strategy intended to exploit a vulnerability in ecDNA-containing cancer cells—was being tested in a small human clinical trial. That dated report describes an experimental approach at that time; it does not establish the trial’s current status or an approved standard treatment. Read the NCI’s December 5, 2024 report.
