An AI refusal is not a safety certificate. It is a response produced for one prompt, in one context, by one model version—and the same system may refuse a harmless request, answer a dangerous one, or sound uncertain while still misleading you. The practical rule is simple: judge an AI by calibrated performance across both unsafe and benign requests, not by how often it declines.
Why a refusal can create false confidence
People often treat “I can’t help with that” as evidence that an AI understood the request, recognized the risk and applied dependable judgment. Research suggests that confidence can be misplaced. In a November 2024 interactive, incentivized experiment, Klingbeil, Grützner and Schreck found that participants were more likely to follow advice when they knew it came from AI—even when that advice conflicted with contextual information and with the participants’ own assessment. The study also found that overreliance could harm third parties. Its abstract does not establish a universal effect size, so the result should not be read as a population-wide probability.
A refusal demonstrates only that the model produced refusal behavior under the wording and surrounding material it received. It does not prove stable understanding of intent, reliable knowledge of policy, or good judgment in a new situation.

Safety evaluation has two boundaries to measure:
- Under-refusal: the model answers when it should have declined, potentially providing unsafe or disallowed assistance.
- Over-refusal: the model declines a benign or otherwise appropriate request, reducing usefulness and blocking legitimate work.
Both failures matter to users. Under-refusal creates direct safety risk; over-refusal can prevent translation, summarization, education, research or support that should be allowed. A system that refuses everything can look safe in a narrow score while being unusable.

OpenAI’s Operator system card illustrates the needed reporting pattern by publishing separate results for unsafe responses and over-refusal on its standard and challenging evaluation sets. Those rates describe those test sets, not the chance of a refusal in everyday use.
What studies show about context and task
Refusal behavior changes when the task, prompt, model family or retrieved documents change. The COVER study, published in the Findings of ACL 2025, found notable variation across those conditions. In its tested material, translation and summarization were especially prone to over-refusal. A model evaluated only with direct question-and-answer prompts could therefore appear well calibrated while failing on ordinary document tasks.

Guardrails can also trade safety for utility. Anthropic’s Constitutional Classifiers prototype resisted thousands of hours of human red teaming, but its report also describes high over-refusal and substantial compute overhead. Stronger blocking is not automatically better if it rejects large amounts of legitimate work or makes a system too expensive to operate.

Benchmarks sample a defined risk taxonomy. SORRY-Bench, presented at ICLR 2025, contains 44 potentially unsafe topics and 440 class-balanced unsafe instructions. Those figures describe the benchmark’s design—not all harmful requests, all languages, or all real-world contexts. A high score means strong performance on that collection of examples under its grading method.

The OpenAI–Anthropic joint evaluation likewise reports model-specific differences in refusal and hallucination outcomes for selected tests. Comparisons are meaningful only with the model version, test conditions and task definitions preserved. A refusal percentage from one evaluation cannot be ranked directly against a number produced by another evaluation with different prompts or graders.
How to evaluate an AI system responsibly
Measure both sides of the boundary
Include disallowed prompts that should be refused and benign or borderline prompts that should receive useful answers. Report unsafe compliance and over-refusal as separate metrics. Do not reward a model simply for abstaining more often.

Vary wording and surrounding context
Use paraphrases, adversarial phrasing, quoted material, retrieved documents and multi-turn conversations. Test translation, summarization and transformation tasks as well as direct questions, because the COVER findings show that task format can change refusal behavior.
Record what was actually tested
Every result should identify the model and version, test date, task and domain coverage, geography when supplied, and whether judgments came from human reviewers, automated graders or both. Without those details, two refusal rates are not comparable.

Use balanced, fine-grained test sets
A taxonomy such as SORRY-Bench can reduce the distortion caused by topic imbalance, but it still cannot represent every risk category or real-world intent. Supplement benchmark prompts with organization-specific scenarios and benign controls.

How an AI expresses uncertainty can change how people rely on it. In a preregistered Microsoft Research experiment with 404 participants, first-person uncertainty wording in a medical-question task reduced confidence and agreement and improved accuracy in that experimental setting. The researchers also found that overreliance was reduced, not eliminated.
Uncertainty wording should therefore be evaluated as a calibration aid, not treated as proof that an answer is safe or correct. A cautious tone can coexist with a factual error, and a confident refusal can still be triggered for the wrong reason.
What users should do when an AI refuses
- Ask what category of risk or limitation caused the refusal, without requesting prohibited details.
- Reframe a legitimate goal narrowly—for example, request a high-level explanation, safety guidance or a benign transformation rather than operational instructions.
- Check whether the refusal is consistent across a small set of clearly benign paraphrases. Inconsistency is evidence that wording or context is driving the result.
- For medical, legal, financial or safety-critical decisions, verify the answer with authoritative human or institutional sources instead of treating either compliance or refusal as a verdict.
Why refusal style affects user perception
A 2025 EMNLP Findings study involving 480 participants and 3,840 query-response pairs compared refusal strategies and user perceptions. In that study, partial compliance produced more than 50% fewer negative perceptions than flat refusals. That is a study-specific finding, not a universal preference rate. It suggests that a useful explanation, safe alternative or limited answer may preserve trust better than an abrupt block—provided the response does not disclose unsafe instructions.
The practical standard for calibrated trust
Trust an AI system proportionally to demonstrated performance on the task in front of you. Look for evidence that it refuses unsafe requests, answers appropriate ones, remains stable under paraphrase and context changes, and communicates uncertainty without inviting blind reliance. Until those dimensions are measured together, “the AI said no” is only an observation about one response—not evidence that the system reliably knows when it should.
