Vestibular rehabilitation can reduce dizziness, imbalance, gaze instability and motion sensitivity in people whose symptoms involve the vestibular system. For VR-related symptoms, the safest approach is not to “push through” nausea or keep increasing headset time. A clinician first determines whether you have vestibular hypofunction, visual motion sensitivity, another balance disorder or a non-vestibular cause, then selects and progresses exercises. Common components are gaze stabilization while moving the head, carefully dosed habituation to symptom-provoking motion, and balance or walking practice. VR may deliver some exercises, but it is an adjunct—not a diagnosis, cure or substitute for individualized therapy.
What vestibular rehabilitation does for VR-related dizziness
Vestibular rehabilitation is an exercise-based treatment for impairments that arise when the balance system in the inner ear, eyes and brain does not work together effectively. Adults with unilateral or bilateral vestibular hypofunction may have dizziness, blurred vision during head movement, unsteadiness, motion sensitivity or difficulty walking in busy environments. The American Physical Therapy Association (APTA) clinical practice guideline gives a strong recommendation for offering vestibular rehabilitation to appropriate adults whose vestibular deficit causes activity limitations or participation restrictions.
VR can expose these problems because a headset presents moving visual scenes while your body may be stationary. Head turns, simulated acceleration, visual flow and rapid scene changes can provoke dizziness, nausea, headache, imbalance or sleepiness in susceptible users. A short-lived, mild response is different from persistent worsening or a new neurologic symptom. Rehabilitation aims to improve the underlying response and your functional confidence, not to make you tolerate unlimited headset time.
First decide whether self-directed exercise is appropriate
Get an assessment when symptoms persist or recur
Arrange an evaluation with a vestibular physical therapist, audiologist or physician if dizziness continues after removing the headset, returns with ordinary daily activities, causes falls, or repeatedly prevents you from using VR. Assessment can distinguish vestibular hypofunction from visual motion sensitivity, migraine-related dizziness, medication effects, eye problems, ear disease and other causes. The diagnosis determines which movements are useful and how quickly to progress.

Stop and seek medical care for warning signs
Do not use VR exposure as a home test when you have new weakness, numbness, trouble speaking, fainting, a severe or unusual headache, new hearing loss, double vision, chest pain, repeated vomiting or an inability to walk safely. Stop the exercise and seek prompt clinical advice if symptoms are severe, continue to escalate after the session, or do not settle with rest. A fall, head injury or sudden change in hearing also warrants medical evaluation rather than another exercise session.
Set up a safe practice area
- Exercise near a stable counter, rail or corner, with a clear floor and another adult available when balance is challenged.
- Remove the headset immediately if you become disoriented; keep a chair nearby so you can sit without turning quickly.
- Do not practice standing balance on an unstable surface, on stairs or while alone until a clinician has judged it safe.
- Record the movement, duration, symptom intensity and recovery time. This gives your therapist useful information for adjusting the dose.
The core exercise categories
1. Gaze stabilization with head movement
Gaze-stability exercises train you to keep a visual target clear while the head moves. A typical starting position is seated, although a therapist may progress you to standing and walking.
- Place a letter or other high-contrast target at eye level at a comfortable distance.
- Keep your eyes on the target and turn your head slowly left and right, staying within a range that does not produce sharp pain or loss of balance.
- Pause when the target becomes persistently blurred or symptoms become more than manageable. Allow them to settle before repeating.
- With professional guidance, add up-and-down head movements, increase speed, change from sitting to standing, move the target farther away, or use a busier background.
The APTA guideline supports gaze-stability work as a vestibular rehabilitation component and cautions that isolated voluntary eye movements without head movement are not a substitute for gaze-stability training. The goal is a clear target during controlled head motion, not maximal speed or deliberate symptom provocation.

Habituation uses repeated, carefully selected exposure to a movement or visual scene that provokes manageable symptoms. The nervous system is given repeated opportunities to reduce its response, with recovery between repetitions. Examples include turning in bed, bending, looking up, walking while turning the head, or viewing a slowly moving visual scene.

- Identify one activity that reliably causes mild symptoms but does not create dangerous imbalance.
- Repeat it in a controlled position for a short period, then rest until symptoms return close to baseline.
- Repeat on a regular schedule rather than performing one long, exhausting session.
- Increase only one variable at a time: duration, speed, visual complexity, distance, or body position.
VR or optokinetic displays can serve as higher-technology habituation tools under clinical supervision. They are not automatically better than real-world movements. If a virtual scene produces severe nausea, prolonged headache or unsafe disorientation, select a less provocative task or stop and consult your clinician.
3. Balance and gait training
Balance work teaches the brain to use vestibular, visual and body-sensation information together. A program may progress from a stable stance to narrower stance, weight shifting, head turns, walking, turning and more complex environments.
- Begin with feet comfortably apart and both hands close to a support surface.
- Practice even weight distribution, then small weight shifts forward, backward and side to side.
- Progress to feet closer together, semi-tandem and tandem stance only when the earlier position is steady.
- Add slow head turns, stepping, direction changes and walking while looking at a target as your therapist recommends.
Challenging balance exercises can increase fall risk. A therapist may use a gait belt, parallel bars or another person for guarding. Do not add a foam pad, eyes-closed condition or rapid head movement simply because an easier version feels comfortable.

Where Cawthorne-Cooksey exercises fit
Cawthorne-Cooksey exercises are a historical generic collection involving eye, head, position-change and balance movements. They can provide ideas for basic activity, but they were not designed to match every diagnosis or modern dosing principle. An individualized program is generally preferable, particularly when VR symptoms are severe, one-sided vestibular loss is suspected, or balance is poor.

How to dose and progress the exercises
There is no universal VR-dizziness prescription. Published protocols differ in diagnosis, exercise content, supervision and outcome measurement. Examples include clinic visits once or twice per week combined with daily home exercises, and a 15-minute VR balance protocol performed five times weekly for six weeks. These are study protocols, not instructions that apply to every headset user.
Use a symptom-and-recovery rule: mild, temporary dizziness may be an expected training response, but symptoms should remain manageable and return toward baseline after the session. A therapist can set the acceptable intensity and recovery window for you. Increase one demand at a time and hold the new level for several sessions before progressing. If recovery takes longer, reduce speed, duration, visual complexity or postural challenge rather than abandoning all movement.
| Training variable | Lower challenge | Higher challenge | Progress only when |
|---|---|---|---|
| Body position | Seated | Standing, walking or turning | You can maintain safe posture and recover promptly |
| Head movement | Small, slow turns | Faster or larger turns; vertical motion | The target stays readable and balance remains controlled |
| Visual scene | Plain, stable background | Moving or visually busy environment | Symptoms stay manageable and do not linger |
| Support | Hands near a counter or therapist guarding | Less hand support or dual-task walking | You have no near-falls and can stop safely |
Using a VR headset as part of therapy
VR can present repeatable visual motion, balance games or simulated environments, which may help a clinician deliver habituation or balance practice. However, studies use different headsets, software, diagnoses, session lengths and comparison treatments. There is no established universal consumer headset, accessory or balance board for treating VR-related dizziness. Buying a device cannot replace examination, diagnosis or progression by a qualified professional.

What the evidence says about VR-based vestibular rehabilitation
Systematic review findings from 2021
Xie and colleagues screened 382 unique records and included 10 studies: six randomized controlled trials and four other studies. Sample sizes ranged from 13 to 70 participants. Diagnoses and interventions varied, and comparators included supervised vestibular rehabilitation and independent Cawthorne-Cooksey exercises. Because of this heterogeneity and limited clinical outcomes, the authors described the evidence for VR-based vestibular rehabilitation as preliminary.
Short-term Dizziness Handicap Inventory result
A 2021 Scientific Reports systematic review and meta-analysis found a standardized mean difference of −1.13 in Dizziness Handicap Inventory scores (95% confidence interval −1.74 to −0.52) at zero to three months for VR or augmented reality added to rehabilitation compared with controls. The review rated risk of bias as high or of some concern, so the numerical effect should not be treated as a guaranteed benefit for an individual. Two studies reporting side effects found that symptoms had decreased by week four, but that finding also comes from limited, varied evidence.
Later synthesis
A 2023 meta-analysis reported potential improvements in Dizziness Handicap Inventory, Vertigo Symptom Scale–Short Form, visual-analogue outcomes and posturography. It also stated that the optimal VR parameters, effective treatment duration and side-effect profile remain uncertain. Together, these reviews support VR as a possible adjunct while leaving the best device, software, dose and patient-selection criteria unresolved.

Dizziness appears during gaze stabilization
Slow the head movement, shorten the interval, sit down or move the target closer. Check that you are moving your head rather than only your eyes. If the target remains blurred or symptoms do not settle, stop and ask your therapist to reassess the exercise.

Nausea or headache follows a headset session
Remove the headset, rest in a quiet environment and note how long recovery takes. At the next supervised session, reduce visual motion, session length or field complexity. Persistent vomiting, severe headache or symptoms that continue well after the session require medical advice.
You feel unsteady during balance practice
Return to a wider stance, restore hand support and have someone guard you. Do not practice on foam or with your eyes closed until a professional has confirmed that the progression is appropriate. Near-falls are a signal to reduce difficulty, not to repeat the same challenge more forcefully.
Symptoms are worse the next day
Pause progression and contact the clinician who prescribed the program. A delayed flare can mean the dose, visual complexity or balance demand is too high, or that another condition needs assessment. Do not interpret prolonged worsening as proof that the exercise is “working.”
A practical decision path
- Screen: Check for warning signs, falls, hearing changes and persistent symptoms. Seek medical evaluation when present.
- Diagnose: Obtain a vestibular and medical assessment if VR repeatedly provokes dizziness or limits daily activity.
- Start simply: Use clinician-selected gaze stabilization, habituation or supported balance work at a tolerable level.
- Track recovery: Record what you did, symptom severity and how long it took to return toward baseline.
- Progress gradually: Change one variable at a time, with supervision for standing, walking and visually complex tasks.
- Add VR selectively: Use a headset only when its scene and dose have a clear therapeutic purpose and stop criteria.
Bottom line for people who get dizzy in VR
Vestibular rehabilitation can be useful when VR exposes a treatable vestibular or motion-sensitivity problem, but it is not a universal cure for cybersickness. The most defensible program combines a correct diagnosis, head-movement gaze stabilization, graded habituation and safe balance or gait practice. Mild symptoms may occur temporarily; severe, persistent or neurologically unusual symptoms are reasons to stop and seek care. Let a vestibular professional determine whether VR belongs in your treatment and how quickly to advance it.
