Neither underwire nor wireless bras are shown to be better for breast health or breast cancer risk. Choose a bra that fits, feels comfortable, and suits what you’ll be doing. Underwire bras are not harmful when they fit well, according to NSW Government guidance; the American Cancer Society says available scientific and clinical data do not show that wearing a bra changes breast cancer risk.
Underwire vs. wireless bras: what the evidence can tell you
Underwire is a construction choice, not a reliable measure of a bra’s health effects, comfort, or support. The available guidance does not establish that one type is universally more comfortable or supportive. Fit, individual preference, and the activity you have in mind are more useful ways to choose.
The American Cancer Society says there is no scientific or clinical data showing that wearing a bra changes breast cancer risk. Its guidance includes evidence from a 2014 study of more than 1,500 women. That statement concerns cancer risk; it does not mean a poorly fitting bra cannot cause discomfort.
How to choose between underwire and wireless
Start with comfort and fit
Assess how the bra feels during the hours you intend to wear it. A size label alone cannot establish that a bra fits comfortably. Underwire ends, seams, or other parts of a poorly fitting bra may contribute to discomfort. NSW Government mastalgia guidance says underwire bras are not harmful to the breasts as long as they fit well.

Match the bra to the activity
Everyday wear and running or other movement can call for different support. If your concern is discomfort during exercise, consider a well-fitting sports bra designed to support movement rather than treating the underwire question as the whole answer. Results depend on the wearer and the particular design.
Use personal preference, not an assumed health advantage
The cited health guidance does not require everyone to choose either construction. If an underwire or wireless bra feels comfortable and works for your needs, these sources provide no basis for switching to the other type to prevent breast cancer or for a general health benefit.

What studies say about support and discomfort
A small study of garments and chronic back pain
A 2024 study assessed 24 larger-breasted women with chronic nonspecific back pain. It compared participants’ usual bras with a new professionally fitted traditional bra and a particular alternative garment, with assessments immediately and after four weeks of wear where applicable. Everyone failed the study’s fit assessment in their own usual bra; 87.5% failed in the standard professionally fitted bra, compared with 4.2% in the alternative garment. Both new garments provided symptomatic relief after four weeks, and the alternative improved more outcome measures.
This was a small study of people who already had chronic back pain, not a general comparison of all underwire and wireless bras. It does not show that wireless bras as a class outperform underwire bras or that a particular garment will relieve back pain for other wearers. The study also discusses uncertainty about whether breast size or bra fit contributes to chronic back pain.

Sports bras and movement
In a 2010 treadmill study involving 20 women with large breasts, an experimental sports bra designed for greater breast elevation and compression produced less exercise-induced breast and bra discomfort than a standard encapsulation sports bra or a placebo. The result applies to that small study and those designs; it is not a universal product ranking.
MedlinePlus lists a well-fitting supportive bra, such as a sports bra, as one possible home-care step for breast pain. A sports bra may help some people during activity, but fit and individual response matter, and no bra is guaranteed to relieve pain.

When breast pain needs medical attention
Breast pain is common and usually is not a sign of cancer, according to the American Cancer Society. An ill-fitting bra is one possible contributor to pain or discomfort. Do not assume that changing bras explains or resolves pain that is persistent or unusual.
The American Cancer Society advises contacting a health professional about constant breast pain or pain that comes and goes at times other than the menstrual cycle. A bra change is not a diagnosis or a substitute for medical assessment.
