Yes—one finger-prick drop can support quantitative blood-chemistry testing, but the drop is only the specimen. A lancet and collection card do not analyze blood. They capture a dried blood spot or fixed-volume microsample that a qualified laboratory later extracts and measures with a validated assay. Accuracy therefore depends on the specific analyte, collection device, drying and shipping conditions, extraction method, and comparison range used by that laboratory.

Some workflows have matched venous results closely for selected analytes. Others can produce biased values when hematocrit, incomplete absorption, evaporation, or matrix effects are not controlled. The safe standard is not “finger-prick equals venous”; it is “validated for this analyte and this workflow.”

What “one drop of blood” actually means

A finger stick produces capillary blood from the small vessels in the fingertip. The drop is transferred to one of several collection formats:

  • Conventional dried-blood-spot (DBS) card: blood spreads through filter paper and dries into a spot.
  • Fixed-volume capillary device: a capillary channel fills with a defined amount of blood.
  • Volumetric absorptive microsampling (VAMS) tip: an absorbent tip is designed to take up a controlled volume.

After drying, the specimen is mailed or delivered to a laboratory. The laboratory punches or removes the sample, elutes or extracts the blood, and measures selected compounds using methods such as capillary electrophoresis, immunoassay, chromatography, or mass spectrometry. The collection device is not a miniature chemistry analyzer.

The final test determines how the sample must be collected and stored. Materials, preservatives, or drying conditions that are acceptable for one assay can make another specimen unusable. A laboratory must therefore validate the complete chain from finger stick to reported result.

How the laboratory workflow works

  1. Prepare the site. The fingertip is cleaned and allowed to dry. Residual alcohol or lotion can dilute the drop or interfere with an assay.
  2. Create capillary flow. A sterile lancet makes a shallow puncture. Excessive squeezing can add interstitial fluid and change the specimen composition.
  3. Fill the collection medium. The drop is placed on a marked DBS circle, into a capillary, or onto a volumetric tip. Fixed-volume formats are intended to reduce variation in the amount submitted.
  4. Dry completely. DBS samples are normally dried in the open air under the kit’s specified conditions. Sealing a damp card can promote diffusion, degradation, or microbial growth.
  5. Package and ship. The dried specimen is protected from moisture and sent according to the laboratory’s instructions. Stability depends on analyte, packaging, temperature, and transit time.
  6. Extract and measure. The laboratory removes the blood from the paper or tip and runs a validated analytical method. Internal controls and calibration establish whether the run is acceptable.
  7. Interpret against the right reference. Results must use reference intervals and conversion factors validated for that specimen type, not automatically the intervals printed for venous plasma.

The FDA-recognized CLSI NBS01 standard describes collection sites, equipment, and technique for acceptable dried-blood-spot specimens used in newborn screening. Its principles illustrate why collection quality is part of the measurement, not an afterthought.

How accurate is a finger-prick result compared with a venous draw?

Accuracy is analyte- and method-specific. Capillary blood and venous blood have different cellular and chemical characteristics, and a dried sample introduces additional steps that can affect concentration. Hematocrit (the fraction of blood occupied by cells), drying, extraction recovery, and matrix effects are common sources of bias.

Evidence from validated workflows

Dvořák and Kubáň (2024) used hemaPEN samples containing 2.74 µL of capillary blood per spot in an automated capillary-electrophoresis workflow for creatinine. Their calibration range was 25–500 µM, the limit of detection was 5.0 µM, throughput exceeded 12 dried spots per hour, and repeatability was reported at no more than 7.1% for peak areas. Within their study conditions, samples aged for 60 days and samples spanning 20–70% hematocrit did not materially change the creatinine results.

Crawford and colleagues (2020) reported mean biases below 3% for five analytes after intrinsic normalization and end-to-end verification against matched venous samples. That result supports the particular collection, normalization, extraction, and measurement process they tested; it is not a universal accuracy claim for every finger-stick test.

Why a venous conversion cannot be assumed

The 2026 International Association of Therapeutic Drug Monitoring and Clinical Toxicology (IATDMCT) guideline states that converting capillary measurements to plasma concentrations requires comprehensive clinical validation, case-by-case evaluation, and an independent set of paired capillary and venous samples. A laboratory that has not completed that work should not present a capillary number as interchangeable with a standard venous-plasma result.

What can a dried blood spot test measure?

A dried spot can support measurement of an analyte when the laboratory has demonstrated acceptable collection, stability, extraction recovery, precision, and clinical interpretation for that analyte. The menu is therefore determined by validated assays, not by the existence of a collection card.

Creatinine as a quantified example

The hemaPEN capillary-electrophoresis workflow described above measured creatinine from 2.74 µL spots. Its 25–500 µM calibration range and 5.0 µM detection limit show that very small volumes can produce quantitative data when the method is engineered and validated for the task.

PFAS as a matrix-and-device example

A 2025 comparison of home-sampling and postage conditions evaluated 49 PFAS compounds. Mitra met that comparison’s recovery and precision criteria for 45 of 49 compounds, hemaPEN for 37 of 49, and conventional DBS for 16 of 49. These are performance figures under that study’s conditions, not universal product specifications or a guarantee that every PFAS assay will behave the same way.

Other analytes

Laboratories may offer additional chemistry, hormone, medication, or exposure tests, but each requires its own validation. Ask for the exact specimen type, analyte, reference interval, stability evidence, and method before assuming that a test listed as “blood chemistry” is equivalent to a venous panel.

Collection methods compared

Method Strength Main limitation Questions to ask the laboratory
Conventional dried-blood-spot card Low cost, simple storage, and easy mailing Spot size, hematocrit, and nonuniform distribution can bias quantitation How is spot volume estimated? Has hematocrit sensitivity been tested? How is extraction standardized?
Mitra VAMS Fixed absorbed volume and practical remote collection The US product is research-use-only and requires validation for the intended test Is the device permitted for this use in my country? Is the assay validated on that tip and extraction protocol?
hemaPEN Four fixed-volume capillary samples from one source Specialized device with territory-dependent regulatory status What volume is collected per sample, and can replicates or repeat testing be performed?
HemaXis DB10 Microfluidic volume control and protected dried spots Certified-laboratory analysis and local validation remain necessary Which laboratory runs the test, and what stability and reference data support its report?
HemaSpot Simplified collection, storage, and shipment Not equivalent to every quantitative fixed-volume method What is the sample capacity, analyte menu, and regulatory status in the shipping region?

These are collection tools that feed a laboratory workflow, not standalone home chemistry analyzers. Regulatory status can differ by country and intended use. For example, the manufacturer describes US Mitra use as research-only, while the HemaXis DB10 product statement identifies it as a CE/IVD and FDA Class 1 medical device. Those labels do not by themselves prove that a particular blood test is clinically validated.

How to collect a mail-in sample correctly

  1. Read the kit’s analyte-specific instructions first. Fasting, timing relative to medication, hydration, and collection time can matter for the requested measurement.
  2. Wash and warm your hands. Warm fingers generally provide better capillary flow. Dry thoroughly so water does not dilute the specimen.
  3. Use the specified puncture site and lancet. Do not reuse a lancet or share it. Dispose of it in an approved sharps container.
  4. Wipe away the first drop if instructed. Some protocols remove surface fluid; others specify using the first drop. Follow the laboratory’s exact procedure rather than improvising.
  5. Let drops form naturally. Gentle pressure around the fingertip is preferable to forceful squeezing. Fill every marked circle or device channel to its stated endpoint.
  6. Keep blood within the printed boundaries. Layering several tiny drops onto one spot, touching the paper to the skin, or creating a partially filled circle can produce an uneven sample.
  7. Dry flat and uncovered for the stated time. Do not use heat, a hair dryer, or a sealed plastic bag while the sample is wet.
  8. Package only when fully dry. Include the required identification and collection time, and use the supplied desiccant or humidity barrier if specified.
  9. Ship on the schedule provided. A sample that is chemically stable for several days is not necessarily stable for weeks in a hot mailbox.

What can make a result unreliable?

  • Underfilled or oversaturated spots: The laboratory may not know the true blood volume, so concentration calculations become uncertain.
  • High or low hematocrit: Cells move through paper differently from plasma, changing spot spread and the fraction recovered by a punch.
  • Contamination: Alcohol, hand cream, food residue, water, or dirty surfaces can dilute or add interfering substances.
  • Excessive squeezing: Interstitial fluid can enter the drop and lower or otherwise alter measured concentrations.
  • Incomplete drying: Moisture accelerates degradation and can cause analytes to migrate within the card.
  • Heat and humidity during transit: Environmental exposure can reduce stability even when the collection itself was correct.
  • Wrong reference interval: A venous-plasma range may not apply to a dried capillary extract.
  • Unvalidated conversion: Applying a generic capillary-to-plasma correction factor can create a false sense of precision.

  1. Identify the exact analyte and clinical purpose. A screening measurement, therapeutic-drug decision, and environmental-exposure panel have different validation requirements.
  2. Confirm specimen validation. Ask whether the laboratory validated capillary blood, dried blood, and the specific device together, including paired venous comparisons where relevant.
  3. Check volume and replicate needs. Fixed-volume formats can help when the assay needs a known input or repeat measurements; a conventional card may be sufficient for qualitative or robust assays.
  4. Review stability and shipping limits. Obtain the accepted drying time, temperature range, maximum transit time, and packaging requirements in writing.
  5. Verify regulatory and geographic status. A CE, FDA, or research-use designation applies to a stated jurisdiction and intended use, not automatically to every test performed with the device.
  6. Understand the report. The laboratory should identify the method, specimen type, units, reference interval, and any limitations that affect clinical interpretation.

How results should be interpreted

Treat an unexpected value as a question about both biology and measurement. First check whether the specimen met acceptance criteria and whether the report uses a capillary-specific interval. Then discuss the result with a clinician who can consider symptoms, medications, timing, and prior results.

A confirmatory venous draw may be appropriate when a result is near a treatment threshold, conflicts with the clinical picture, or comes from a method without an established capillary-to-venous relationship. Do not change medication or make an urgent diagnosis from a mail-in microsample report alone.

Can you mail a blood spot to a laboratory?

Yes, provided the laboratory accepts that specimen type and gives a validated collection and shipping protocol. Mailing is practical because a dried specimen is lighter and generally less demanding than liquid blood, but “dried” does not mean indefinitely stable. The laboratory must specify packaging, humidity control, transit time, and acceptable collection volume for the requested assay.

Key takeaways for home testing

  • One drop can be enough for a quantitative result when the entire method is validated for that analyte and specimen type.
  • The device collects blood; the laboratory performs the measurement.
  • Published performance is method-specific: examples include creatinine measured from 2.74 µL spots, mean bias below 3% for five analytes in one matched comparison, and PFAS recovery criteria met for 45 of 49 compounds with one device under defined conditions.
  • Hematocrit, drying, extraction, shipping, and reference intervals can materially affect interpretation.
  • Ask the laboratory for analyte-specific validation and use a venous confirmation when the result will drive a consequential medical decision.
