For occasional use with mild dry eye and an otherwise healthy eye surface, preserved artificial tears may be enough. If you need lubricating drops frequently and over time—more than four times a day is one example—preservative-free tears are generally recommended. The right choice also depends on your eye health, contact lenses, package handling, cost, and the specific product label.

How to choose between preserved and preservative-free tears

For artificial tears, use frequency and duration are the most useful starting points. The American Academy of Ophthalmology (AAO) says preserved tear substitutes may be sufficient for mild dry eye when the ocular surface is otherwise healthy. For frequent, chronic use, it generally recommends nonpreserved tears; more than four applications per day is an example, not a study statistic or a universal cutoff.

  • Occasional use: Preserved tears may be suitable if symptoms are mild and your eye surface is healthy.
  • Frequent, ongoing use: Consider preservative-free tears, particularly if you use drops more than four times daily.
  • Moderate or very dry eyes: Mayo Clinic also describes preservative-free drops as generally recommended in these circumstances.
  • Persistent or worsening symptoms: Seek professional advice rather than repeatedly switching products.

These are general recommendations for lubricating artificial tears, not an individualized diagnosis. They do not automatically apply to prescription medicines, redness-relief drops, antibiotics, or every other eye preparation. Product ingredients and directions vary.

What the preservative does—and what the concern is

Preservatives help discourage microbial growth in multi-dose eye-drop containers after opening. Single-use vials that are discarded after one use do not need a preservative for that packaging purpose. Preservative-free options also include some multi-dose bottles with specialized dispensing systems.

TFOS DEWS II identifies benzalkonium chloride (BAK) as the most frequently used preservative in eye-drop preparations and describes evidence associating chronic exposure to topical preservatives with ocular-surface toxicity and adverse changes. The report reviews laboratory and clinical concerns; that evidence does not mean every person using every preserved lubricant will be harmed. Preservative-free drops avoid an added preservative, but still need careful handling and must be used according to their instructions.

Compare the practical trade-offs

Contact lenses Preservative status alone does not establish that a drop is safe to use over lenses. Preservative-free status alone does not establish compatibility with lenses either.

AAO says most artificial tears may have comparable efficacy, but notes important inconsistencies in the underlying studies. The evidence does not establish one brand as best for everyone; compare the product’s formulation, directions, and usability instead.

How to check whether drops can be used with contact lenses

If you plan to put drops in while your lenses are in, choose a product clearly labeled as safe or designed for contact-lens wear. Other drops may instruct you to remove lenses and wait before putting them back in. Follow the directions for the specific product; being preservative-free is not a substitute for checking its lens instructions.

When to get medical advice

Contact an eye doctor promptly for pronounced redness, pain, light sensitivity, blurred vision, or discharge; these symptoms may indicate a serious eye problem. Persistent or worsening dry-eye symptoms also warrant professional assessment instead of ongoing product changes.
