RTX 4090 16-pin cables melt when a high-current connection develops excessive resistance—most often from incomplete insertion, cable tension or a tight bend, contamination, or a terminal losing contact. The revised 12V-2×6 connector reduces the risk of drawing power before full insertion, but it cannot make a damaged or mechanically stressed connection safe.
Why a 12VHPWR connector melts
The connector carries substantial GPU power through a compact housing. A properly engaged terminal has low resistance. If the plug is not fully seated, is pulled sideways by a sharp bend, contains foreign-object debris, or develops a weak terminal contact, the effective resistance rises. Heat then concentrates at one or more pins instead of being spread evenly across the connector.
As the plastic softens, contact pressure can fall further. That increases resistance and accelerates the failure. A pin that loses contact can also force the remaining pins to carry more current, creating a local hot spot even when the rest of the plug appears normal.
The recurring failure combination
GamersNexus identified two repeatable factors in first-generation 12VHPWR failures: partial engagement combined with cable tension, and foreign-object debris. A connector can look nearly inserted while still leaving a terminal inadequately engaged, so visual alignment alone is not proof of a sound electrical connection.

What NVIDIA reported about RTX 4090 failures
In a support update dated 18 November 2022, NVIDIA said it was aware of about 50 cases globally. Its findings indicated that connectors were commonly not fully plugged into the graphics card. NVIDIA advised connecting the power dongle to the GPU first, pressing it in firmly and evenly, and installing the card only after the connector was seated. It also offered expedited RMA support for reported cases.
That statement described the cases known to NVIDIA at that time; it was not a worldwide failure-rate study. It also does not mean every melted connector was caused by the same single mistake.

Installation clearance and bend control
NVIDIA’s RTX 40-series guidance specifies at least 1.4 inches (36 mm) of clearance above the graphics card for cable bending and airflow. The measurement applies to the space immediately above the connector and should be treated as a minimum, not an invitation to fold the cable tightly at the housing.
- Connect the 16-pin dongle or cable to the GPU before installing the graphics card in the motherboard.
- Push the plug straight in until the latch is engaged and there is no visible gap between the connector housings.
- Route the cable with a gradual bend. Do not pull it sideways or create a sharp turn directly at the plug.
- After the card is installed, check that the side panel, drive cage, or another cable is not pressing on the connector.
12VHPWR versus 12V-2×6
12V-2×6 is a revised header and connector implementation incorporated into PCIe CEM 5.1 in July–August 2023. The changes are designed to reduce power draw when the plug is not properly inserted, rather than to tolerate a bad connection indefinitely.

The revised design lowers one important risk; it is not a guarantee that every 16-pin installation is safe under every mechanical or electrical condition.
Has NVIDIA fixed the problem?
There is no evidence of a single worldwide 2026 failure-rate denominator that can establish the problem as either eliminated or universal. The hardware revision addresses incomplete-insertion behavior, while installation quality, connector condition, cable design, and the GPU receptacle still matter.

Tom’s Hardware reported a melted official RTX 4090 cable after two years of use even though the reviewer described the connection as secure. The report also noted that some RTX 40- and 50-series add-in boards continued to ship with older 12VHPWR receptacles. Later incident reports describe 12V-2×6 failures in which one failed pin overloaded the remaining pins. These are individual reports, not a measured population-wide rate.
How common are failures?
A GamersNexus survey with approximately 23,000 responses reported that 3.3% of respondents experienced an old PCIe-connector failure and 4.0% reported a 12VHPWR failure. Those figures are survey results, not market failure rates: respondents self-selected, exposure time differed, and the sample does not represent every GPU or power supply in use. No authoritative global 2026 statistic is available.

Can you replace the adapter with a native cable?
Yes, when the power supply supports an approved native PCIe 5/ATX 3.x cable for that exact PSU model and the GPU accepts the corresponding connector. A native cable can simplify routing compared with an adapter, but “16-pin” or “12V-2×6” in a product name does not make a cable universal.
Compatibility rules
- Use the cable supplied with the PSU, or one explicitly listed by its manufacturer as compatible with the exact PSU model and modular socket.
- Match the GPU-side connector and header revision. Board revisions and production batches can differ.
- Never reuse a modular cable from another PSU merely because the plug fits; pin assignments can differ between brands and models.
- Confirm the vendor’s stated power rating, warranty, and RMA process. A cable advertised as a generic replacement is not automatically approved.
For a replacement search, the precise phrase “12V-2×6 PCIe GPU power cable” is more useful than a generic “16-pin cable,” but the exact PSU-model compatibility check remains mandatory.
Inspection checklist before powering on
- Identify whether the GPU receptacle is first-generation 12VHPWR or 12V-2×6; do not infer the revision solely from the GPU family name.
- Inspect the plug for a visible gap, cocking, uneven insertion, damaged latch, or a terminal that appears recessed.
- Look for dust, foreign-object debris, discoloration, softened plastic, or a burnt smell.
- Verify at least 36 mm of bend clearance above the card and remove any side pressure from the case panel or adjacent hardware.
- Check both ends of a modular cable: the GPU connector must be fully seated, and the PSU-side plugs must be in the sockets specified for that cable.
What to do if the connector is hot, discolored, or melted
- Shut the system down and disconnect AC power. Do not keep rebooting to see whether the connector still works.
- Do not continue using a cable with softened plastic, burn marks, a burnt odor, or visible terminal damage.
- Photograph the GPU receptacle, cable plug, and PSU-side connection before removing evidence of the failure.
- Contact both the GPU and PSU manufacturers for their RMA or service procedure. Replacing only the cable can leave a heat-damaged GPU receptacle in service.
- Do not force a replacement plug into a deformed receptacle or attempt to repair terminals yourself.
A connector-temperature monitoring adapter can provide an additional inspection signal, but it is not a repair and cannot guarantee that a connector will not fail.
