A newborn can be carried in a wrap only when the wrap’s instructions allow it for the baby’s size and the baby stays in a safe, supported position. Keep the face visible, nose and mouth clear, chin away from the chest, and hips and knees bent with the thighs supported. Check often, and manage clothing for the room and the warmth of your body and the carrier.
How to position a newborn safely in a wrap
Follow the specific wrap’s instructions for newborn use, fit, and adjustment. A correctly tied wrap is not enough by itself: the baby’s face and breathing position must remain easy to see and check.
- Face visible: Keep the baby’s face uncovered and in view. Fabric must not cover the nose or mouth, and the face should not be pressed into your body.
- Airway open: Keep the neck straight and the chin away from the chest. Avoid a curled, chin-to-chest posture, which can make breathing harder.
- Body supported: Support the baby in the position approved by the carrier instructions. Check frequently that the baby has not slumped or shifted.
- Feed outside the wrap: NHS guidance says to take the baby out of the sling to feed. After nursing, reposition the baby with the head facing up and clear of both the sling and caregiver before carrying again.
The NHS summarizes its sling checks as T.I.C.K.S.: Tight, In view, Close enough to kiss, Keep the chin off the chest, and Support the back. These are useful checks, but they do not replace the carrier’s instructions.
What a newborn’s legs and hips should look like
A hip-supportive carried position lets the hips and knees stay bent, with the thighs spread and supported. The International Hip Dysplasia Institute (IHDI) calls this the M-position, also known as the jockey, straddle, or spread-squat position. Support the thigh toward the knee, particularly during prolonged carrying and for infants under six months.

Check the carrier’s fit against the baby’s size so the legs are supported without forcing them straight together. The IHDI recommends carrying with the lower extremities in an M-position during the first six months as educational guidance for healthy hip development. It also says there is no evidence that failing to use a hip-healthy carried position causes hip dysplasia in otherwise healthy infant hips.
Can a wrap cause or prevent hip dysplasia?
No carrier can be said to cause or prevent hip dysplasia on the evidence described by IHDI: its carrier consensus says, “Based on the best available evidence, IHDI maintains that no baby carrier can cause or prevent hip dysplasia.” The M-position is a positioning recommendation, not proof that a particular wrap prevents the condition or treats a diagnosed case.

IHDI separately notes that approximately one in ten newborn infants has mild hip instability and one in 100 may require treatment; the page does not state a publication year. Those figures should not be read as global prevalence estimates. IHDI also distinguishes carrying in a wrap from tightly swaddling an infant’s legs straight for many hours.
How to keep a baby from overheating in a wrap
There is no universal carrier-specific layer count or temperature formula in the guidance. Account for the baby’s clothes, the carrier, and the warmth of your body, then check the baby rather than relying on a fixed number of layers.

- Feel the baby’s chest or the back of the neck. If the skin is hot or sweaty, remove a layer.
- Watch for sweating, flushed skin, or a hot chest. The American Academy of Pediatrics (AAP) advises dressing infants for the environment and generally using no more than one layer beyond what a comfortable adult would wear; this is general clothing guidance, not a wrap-specific rule.
- Remove hats and extra clothing when going indoors or into a warm vehicle.
When extra medical guidance matters
Premature infants and babies with respiratory problems may need individualized advice before being carried. The AAP cautions that premature infants or babies with respiratory problems should not be placed in backpacks or other upright positioning devices without clinician guidance, because positioning may make breathing harder. Ask the baby’s clinician about an appropriate carrying position and product for your child.

- Confirm the product is suitable: Check the current instructions for newborn use and the baby’s size. Do not assume every wrap or carrier fits every newborn.
- Prepare and inspect it: Follow the manufacturer’s tying and adjustment instructions, and check materials, seams, and fasteners for damage.
- Check the position: Make sure the face is visible, nose and mouth are clear, chin is off the chest, back is supported, and thighs are supported with hips and knees bent and spread.
- Recheck frequently: Watch for slumping or a changed position, and reassess breathing and temperature during the carry.
In the United States, the Consumer Product Safety Commission (CPSC) describes a mandatory federal standard for infant sling carriers covering loading, structural integrity, occupant retention, warning labels, and instructions. CPSC reported 159 sling-carrier incidents from January 2003 through September 2016: 17 were fatal and 142 nonfatal. Of the nonfatal reports, 67 involved an infant injury during product use, and 10 injury reports involved hospitalization. These historical incident reports are not current rates or a measure of an individual baby’s risk.
