In a game, motion blur is more likely to bother you if movement looks smeared or camera turns feel uncomfortable; film grain is more likely to bother you if you dislike a textured image, especially when the scene is still. They are often separate settings, so you can adjust each on its own and compare the same scene.
What motion blur and film grain mean in a game
Motion blur is a simulated blur associated with camera movement or fast-moving objects. Its appearance and strength depend on the game’s implementation. Insomniac describes motion blur in Marvel’s Spider-Man 2 accessibility options as applying to camera motion and fast-moving objects.
Film grain is a visual texture effect in a game menu. The label does not guarantee that the game precisely simulates physical photographic film. In games such as God of War Ragnarök and Marvel’s Spider-Man 2, grain and blur can be adjusted separately. PlayStation’s accessibility documentation also includes both among options relevant to reducing motion.
Which one should you turn down first?

To compare fairly, use the game’s graphics or accessibility menu and change one control at a time. Revisit the same scene, including a still view and a camera turn, before deciding whether to leave an effect on, reduce it, or disable it. Defaults and menu paths vary by game, so there is no single setting to recommend for every title.

Why the answer depends on the game and the player
Motion blur changes how movement appears; grain changes the image’s apparent texture. A player who values crisp movement may notice blur most, while someone who wants clean-looking surfaces may notice grain most. The amount, implementation, and available controls differ by game. Official settings documentation establishes that these effects can be adjusted separately in some games, but it does not establish that one is always worse or that one visual style is objectively better.
Accessibility guidance treats motion blur and film grain as adjustable visual options, not as effects that affect everyone in the same way. Microsoft’s Xbox Accessibility Guideline 117 addresses customizable camera movement and visual movement settings. If an effect is uncomfortable or makes details harder for you to follow, reducing it is a reasonable personal setting choice; that does not establish a universal medical effect.

If you mean photography, they are different trade-offs
In photography, motion blur happens when a subject or camera moves while the shutter is open. FUJIFILM explains that “anything that moves while the shutter is open will blur” in its guide to shutter speeds and motion blur. Blur can be intentional for a sense of movement or accidental when you want a sharp image.
Film grain is the granular appearance of photographic film. Higher-ISO film generally shows more grain. Digital noise can look similar, but it is not the same thing as film grain; see Canon’s film-grain definition and Nikon’s explanation of ISO sensitivity.

Unlike two separate game sliders, shutter speed, aperture, and ISO interact when taking a photograph. A longer exposure may let you use a lower ISO, but it increases the chance of motion blur. More light, a wider aperture, or flash may help you use a faster shutter or lower ISO, but each has its own trade-offs. A tripod steadies the camera, but it cannot freeze a moving subject.
