Japan has reported the first RiHEART heart-cell-sheet transplant after the treatment became eligible for public health insurance. The procedure took place at Osaka Police Hospital on September 29, 2026, after coverage began September 1. It is a shift from clinical-trial use to treatment under conditional approval—not the first-ever transplant of an iPS-cell-derived heart sheet, and not a therapy for heart disease generally.

What happened in Japan?

Cuorips announced on October 1, 2026, that a patient at Osaka Police Hospital received RiHEART on September 29. The company described it as the first transplant after the product became eligible for Japan’s public health insurance system on September 1. Cuorips’ announcement marks a post-coverage milestone, not the beginning of human use.

Jiji Press, in a report carried by Nippon.com on September 30, identified the recipient as a woman in her 50s with severe heart failure and reported that her condition was stable after surgery. That is an early postoperative description, not evidence about long-term recovery or lasting benefit. Nippon.com’s report covers the patient and procedure.

What is RiHEART?

RiHEART is a sheet of cardiomyocytes—heart muscle cells—made from donor-derived induced pluripotent stem cells (iPS cells). Because the cells are allogeneic, they come from a donor rather than being made from the recipient’s own cells. Cuorips says the sheet is placed on the heart’s surface.

The company describes the intended mechanism as a paracrine effect: the transplanted cells are expected to release cytokines and other factors that may help the heart. This is the company’s proposed explanation for the treatment’s intended benefit; it should not be read as proof that the treatment improves outcomes. Who is the treatment intended for?

Cuorips describes RiHEART as intended for people with severe heart failure caused by ischemic cardiomyopathy that remains refractory to standard pharmacological and invasive therapies. Ischemic cardiomyopathy is heart muscle disease associated with inadequate blood supply, often following coronary artery disease or a heart attack. The indication is specific: this is not a general treatment for anyone with heart failure, nor does insurance eligibility mean every patient with heart disease qualifies.

Whether a particular patient is eligible depends on clinical assessment within the approved indication and the relevant treatment setting. The sources describing this milestone do not establish a broader patient-selection checklist or comparative advantage over other heart-failure treatments.

Is RiHEART approved and covered by insurance?

Japan’s Ministry of Health, Labour and Welfare granted RiHEART conditional, time-limited marketing approval on March 6, 2026. The Japan Agency for Medical Research and Development (AMED) identifies Cuorips as the product’s developer and notes that it builds on research by Professor Yoshiki Sawa’s group at the University of Osaka. Conditional approval is not the same as a final determination that efficacy and safety have been fully established. Under Japan’s pathway, approval may be granted when safety has been confirmed and efficacy is reasonably predicted, with further efficacy confirmation and safety evaluation required after marketing. The company reported that insurance eligibility began September 1, 2026; that date and the September 29 procedure are separate milestones.

Why this was not the first-ever RiHEART transplant

People received iPS-derived heart-cell sheets in clinical research before insurance coverage. AMED and the University of Osaka reported that the first participant in an investigator-initiated study for ischemic cardiomyopathy was transplanted in January 2020. That earlier study was planned for 10 cases. The 2026 procedure is therefore best described as the first reported transplantation after insurance coverage, rather than the first human transplant of an iPS-derived heart-cell sheet. What evidence is still being collected?

A separate registered Osaka study is evaluating allogeneic iPS-derived cardiomyocyte sheets for dilated cardiomyopathy, with a target enrollment of four. Its registered outcomes include safety measures and cardiac clinical events through 52 weeks, alongside measures such as left-ventricular function, heart remodeling, heart-failure severity, exercise tolerance, rejection events, and whether participants are judged improved. These are planned endpoints, not reported results. The Japan Registry of Clinical Trials entry lists the study design and outcomes.

The trial record and the conditional-approval framework explain why one operation cannot settle questions about efficacy or durability. The reported stable condition after surgery is useful early information, but it does not demonstrate sustained improvement, prove the proposed mechanism, or establish how RiHEART compares with other care.

What the milestone means for patients

RiHEART’s first reported post-coverage transplant shows that the treatment has moved beyond investigator-initiated trial use into use following conditional approval and insurance eligibility in Japan. It does not mean the treatment is broadly available to all heart-failure patients, that every eligible patient will benefit, or that long-term effectiveness has been established. For patients, the relevant questions remain whether their condition fits the approved indication, whether a specialist considers the procedure appropriate, and what evidence is available for their situation.
