Hellraiser: Revival is not a direct sequel to the films. It is a new, single-player survival-horror story built from Clive Barker’s novella The Hellbound Heart and the first two Hellraiser movies. Its developers preserve Barker’s collision of desire, pain, pleasure and spiritual temptation, then make those ideas interactive through first-person exploration, puzzles, stealth and combat. The game also adds the Scarlet Church and the Unworthy, human factions that show what happens when people imitate or exploit Cenobite practices. That approach produced some of the strongest content warnings attached to the project: Video Games Chronicle reported that Indonesia’s rating system gave it an RC (Refused Classification) decision, while the team’s stated ambition was to be “banned, unrated, or restricted” somewhere.
Is Hellraiser: Revival a direct sequel to the films?
No. Revival tells an original story in the Hellraiser universe rather than continuing the continuity of Hellraiser or Hellbound: Hellraiser II. Xbox Wire’s 2025 announcement describes the first two films as key inspirations, while the developers also studied Barker’s original novella, The Hellbound Heart.
The result is a source-first adaptation rather than a scene-by-scene recreation. Aidan uses the Genesis Configuration to try to rescue his girlfriend, Sunny, from the Labyrinth. The puzzle box, the Labyrinth and the Cenobites remain central, but the characters, conflicts and playable structure belong to a new story.
What did Barker contribute?
Xbox Wire says Barker was involved from the beginning, helping shape the visual style, story motifs, lore and character personalities. Inverse reported that he took part heavily in ideation and later supplied approval and corrections concerning the Labyrinth and Cenobite powers. Mad Head Games told Daily Dead that he periodically reviewed gameplay and scripts.

Pocket Tactics reported that the team deliberately limited its source study to The Hellbound Heart and the first two films. Once Barker believed the artists understood Hellraiser’s “soul,” he gave them room to make their own game—described by the team as “he just let us rip.”
How does the game turn Hellraiser’s themes into play?
The films can place a viewer outside the action; a game has to make the player repeatedly choose whether to investigate, hide, fight or solve a puzzle. Revival uses Aidan’s agency to turn the pain-and-pleasure philosophy into encounters rather than only cinematic imagery.
A more capable protagonist
Inverse describes Aidan as more violent and capable than the largely vulnerable protagonists typical of the films. That ability supports an action-survival structure, but it does not remove the series’ emphasis on dread. The game alternates stealth, outright horror, combat, exploration, puzzles and drama so that violence is one response among several, not the entire identity of the experience.

The Genesis Configuration is both a puzzle object and the means by which Aidan tries to reach Sunny. Its role keeps the franchise’s fascination with forbidden knowledge and impossible spaces inside the player’s objectives. The Labyrinth is therefore not just a backdrop for Cenobite appearances; it is a place the player must navigate and understand.

How does Revival expand Cenobite lore?
The game adds human institutions that interpret, copy and misuse Cenobite ideas. Those groups create conflicts that a purely monster-focused story could not provide.
The Scarlet Church
The Scarlet Church is a new religious movement organized around the Cenobites. Its doctrine presents pain as a gift and forced “enlightenment” as recruitment. Members seek sensation, power and intimacy, but the story separates those human motives from the Cenobites’ more spiritual identity as explorers of experience.
This distinction matters to the adaptation. The church is not simply another name for the Cenobites; it is an example of ordinary people turning an alien philosophy into an institution and a source of control.

Familiar figures in a larger system
Pinhead, Butterball, Chatterer and Fallen appear alongside new factions, artifacts and environments. The developers describe the Cenobites as hunters and manipulators whose interest reaches forms of suffering beyond straightforward physical violence. That framing follows Barker’s more ambiguous conception: Antony De Fault told Inverse that the team returned to the “esoteric original vision” in which Cenobites are “morally gray explorers of sensation.”
Why was the game at risk of bans or restrictions?
Revival makes taboo material interactive and presents it through an intimate first-person perspective. The reported content includes full-frontal nudity, extreme gore, references to self-mutilation, sexual material and disturbing situations involving the player character.
Narrative director Antony De Fault told Video Games Chronicle in 2026: “It was an unofficial goal of the team to be banned, unrated, or restricted at least somewhere in the world.” VGC reported that Indonesia’s Game Rating System assigned an RC (Refused Classification) decision, effectively blocking sale there. The Indonesia RC decision shows that this was not merely a marketing claim about provocation: the game’s combination of explicit content and player participation created a real classification barrier in at least one market.
