I built a handover board for a friend who had become the person everyone relied on to remember what was happening. The board gave routine information a shared place to live, so keeping track did not depend on one person carrying every detail in their head. It was a practical coordination aid—not a replacement for care records or conversations with professionals.
Why I made a handover board
When one person is the default keeper of appointments, visits, and updates, other people may not know what is planned or what has changed. A shared board makes some of that everyday information visible to the people who need it. The point is not to make care feel like a project-management exercise; it is to make the next handoff easier and reduce the number of routine details one person has to repeat.
This is a familiar coordination problem in home care. A 2024 qualitative study reports that people receiving care and their relatives can take on substantial organizational coordination and information exchange when collaboration between professionals is perceived as limited. That finding describes the study’s participants and setting; it is not a measure of how often this happens in every household. The study also describes a therapist’s example of a household weekly calendar or board used to track visits from different therapists (2024 qualitative home-care study; participant account of a weekly board).
What a household handover board can do
A board can help household members see the practical information needed for the next day or next visit: what is scheduled, who is expected, and whether something needs attention. Its value comes from being easy for the group to find and update, not from having a complicated layout.

Other informal tools serve a similar purpose. The home-care study describes households using weekly calendars or boards, along with handover books and therapy sheets. These are examples of ways people organize information, not evidence that one format works best for every family.
How to make shared information useful
Put it where the people coordinating care will see it
Choose a location or shared digital space that the relevant people can reliably access. A physical board is useful only if people pass it or check it; a digital version is useful only if everyone knows where it is and can open it.

Keep the information practical and current
Use the board for routine coordination details the group needs to hand off. Make updates promptly when plans change, and agree on who will maintain it. A board that looks authoritative but is stale can create confusion rather than prevent it.
That is not just a household concern. A qualitative action-research study of palliative-care handovers found that out-of-hours forms were sometimes out of date, complicating decisions when other records were not immediately available (qualitative study of palliative-care handover forms). The setting differs from a family board, but the practical lesson is direct: visible information is useful only when people can trust that it is current.

Keep sensitive details off an exposed surface
A shared board may be visible to visitors or other people in the home. Put only information there that is appropriate for that audience. Keep private or clinically detailed material in the appropriate secure record or communication channel.
What the board should not replace
A household board is an informal coordination aid. It does not replace professional documentation, a care plan, or direct communication with clinicians and care services. If a detail could affect a care decision, make sure it reaches the relevant professional through the channel they use—not only through a note on the household board.

Choosing a format
There is no single right format. A paper calendar, a simple board, a notebook, or a shared digital document can all make handoffs easier if the people involved can access it and keep it up to date. Choose the least complicated option that fits the group’s habits. If a physical board would help, a small dry-erase board is one possible format; the studies support household boards and calendars generally, not a particular material or product.
