Validate Medicare Advantage quality data by first locking the contract year, reporting section, measure set, and final CMS technical specification, then tracing every reported value back to source records through documented extraction, transformation, reconciliation, testing, review, and approval. Keep the evidence package so another reviewer can reproduce the result. The exact fields, deadlines, file formats, and submission mechanism depend on the contract year and reporting section.

Do not treat all Medicare Advantage data as one validation exercise. Part C and Part D Reporting Requirements data are subject to annual retrospective independent validation by an external contractor. QRS and HEDIS-related measures require source, capture, format, quality-control, rate-impact, and primary-source checks. Risk-adjustment diagnosis support is a separate RADV audit, not ordinary quality-reporting validation.

What Medicare Advantage data validation is designed to prove

Validation is the documented process that shows a reported number is complete, correctly defined, accurately calculated, properly formatted, and supported by records. CMS requires each Medicare Advantage organization and Part D sponsor to maintain an effective procedure to develop, compile, evaluate, and report required information. That obligation is broader than running a final spreadsheet check: the organization must be able to explain how each value was produced and demonstrate that its controls worked.

Start with the current CMS Part C and Part D Reporting Requirements materials. The CMS reporting page was last modified August 5, 2026 and listed calendar-year 2026 requirements with technical specifications dated December 1, 2025. CMS warns that reporting details and specifications can change from year to year, so a prior-year workbook, definition, deadline, or file layout is not an authority for the current submission.

The outcome of a defensible validation

  • A named contract, reporting year, data period, section, measure set, and specification version.
  • A field-level lineage record showing the source system, accountable owner, extraction date, transformations, and CMS definition used.
  • Reconciliations tying reported totals, denominators, numerators, and categories to source records.
  • Tests for required fields, formats, allowed values, date ranges, duplicates, omissions, and internal consistency.
  • An exception log showing investigation, correction, rerun results, and approval.
  • A retained evidence package that lets an independent reviewer reproduce the submitted file.

Choose the correct validation pathway first

The data’s purpose determines the controls, evidence, and reviewer you need. Use this distinction before assigning work or building tests.

Data type What to validate Who performs or reviews it Key CMS reference
Part C/Part D Reporting Requirements Contract-year reporting section, required elements, definitions, calculations, formats, completeness, and source support. The organization performs its internal preparation and controls; an external contractor performs the required independent retrospective validation. CMS Part C and Part D Data Validation
QRS/HEDIS quality measures Measure specification, source type, data capture, file format, quality controls, effect on rates, audited calculations, and primary-source evidence where needed. Measure owners and quality teams prepare and test the data; audit procedures determine audited results and public-reporting status. CMS 2027 QRS Measure Technical Specifications
Risk-adjustment diagnoses Whether diagnosis codes are supported in enrollee medical records under the RADV audit scope. RADV is a separate CMS audit program; it is not a substitute for reporting-data validation. CMS Medicare Advantage Risk Adjustment Data Validation Program

If a data set serves more than one purpose, run the applicable controls separately and label the evidence by purpose. A file can be accurate for a quality measure while still lacking the medical-record support required in a RADV review.

Step 1: Freeze scope and the governing specification

Do not begin with a data extract. Begin with a scope record that prevents the team from validating the wrong year or measure.

  1. Identify the contract. Record the Medicare Advantage or Part D contract number, organization, reporting entity, and any delegated source owners.
  2. Identify the reporting year and data period. Distinguish the contract year from the calendar year in which validation occurs. Record start and end dates for every measure.
  3. Name the reporting section and measure set. Capture the section title, measure identifiers, indicators, numerator and denominator definitions, exclusions, and stratifications that apply.
  4. Retrieve the final CMS specification. Save the document title, publication or revision date, and a controlled copy. CMS’s current reporting page and data-validation page are the starting points, but the final instruction for your section controls.
  5. Record submission mechanics. Note the required file layout, allowed values, naming conventions, transmission method, validation edits, and submission deadline stated for that contract year.
  6. Assign accountability. Name the preparer, source-system owner, measure owner, quality reviewer, compliance approver, and contact for each delegated source.

Keep this scope record with the final file. If CMS issues an update near submission, record what changed, which tests were rerun, and who approved the impact assessment. The exact deadline and required elements cannot be inferred from a general checklist; confirm them in the final CMS materials for your contract and reporting section.

Step 2: Build field-level data lineage

Lineage is the chain from a reported field to the original event, record, or calculation. Build it before testing so a failed value can be traced without reconstructing the process from memory.

Minimum lineage record

  • Reported field or measure: the CMS field name, measure, indicator, numerator, denominator, or category.
  • Originating system: claims, encounter, enrollment, appeals, pharmacy, care-management, survey, provider, or another approved source.
  • Accountable owner: the person responsible for source completeness and definitions.
  • Extraction details: query or report name, extraction date, data cutoff, filters, population criteria, and record count.
  • Transformation logic: joins, deduplication, date handling, exclusions, imputation, recoding, aggregation, and manual adjustments.
  • CMS mapping: the exact definition, code set, format, and version used to map source values.
  • Output location: the table, worksheet, file, or system field in the submission package.
  • Control evidence: query files, code, parameter snapshots, mapping tables, change history, and reviewer sign-off.

For supplemental quality data, document how information is collected, transferred, controlled, and incorporated into the measure. A source that is valid in concept can still produce an unreliable rate if capture rules changed, files were truncated, or the same member was counted twice.

Step 3: Test completeness, format, and population boundaries

Run automated checks before asking whether a rate is plausible. A plausible rate with missing records is still wrong.

Completeness checks

  • Compare the number of submitted records with the source-system extract and the expected eligible population.
  • Confirm every required field is populated or carries an allowed CMS value for not applicable, unknown, or unavailable conditions.
  • Check that all required reporting sections, measures, indicators, and contract segments are present.
  • Look for unexplained gaps by month, county, product, provider, line of business, or other required stratification.
  • Confirm that records excluded by business rules are listed with a reason and can be reproduced.

Format and value checks

  • Validate field length, data type, decimal precision, date format, sign, and case requirements.
  • Compare categorical values with the specification’s allowed-value list; reject spelling variants and obsolete codes.
  • Check that dates fall within the defined measurement period and that event dates precede outcome or submission dates when the measure requires that sequence.
  • Detect duplicate member, claim, encounter, appeal, or event keys using the uniqueness rule for that file.
  • Confirm that nulls, zeroes, suppressed values, and not-applicable codes are not being used interchangeably.

Save both the test logic and its output. A reviewer should see the population tested, the rule applied, the number of failures, and the disposition of each failure—not merely a statement that the file passed.

Step 4: Reconcile calculations to source records

Reconciliation demonstrates that the submitted totals are the result of the intended population and formula. Perform it at more than one level.

Record-level reconciliation

Select records from the submitted file and trace them to the source extract, including the member or event identifier, source dates, eligibility conditions, and values used in the calculation. For any sampled record that cannot be traced, determine whether the problem is an extraction filter, a transformation, a late-arriving record, a manual change, or an invalid source.

Aggregate reconciliation

Tie the submission’s totals to source counts by measure, period, and required category. Recalculate denominators and numerators independently from the lineage extract. Explain every difference, including legitimate exclusions, rounding, suppression, and timing cutoffs.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reasonableness and trend tests

Compare the current result with prior periods only as an investigation aid, not as a replacement for the current definition. Flag abrupt changes, extreme rates, empty categories, unexpected reversals, and unusually high or low provider or subgroup results. A trend change can be legitimate after a specification revision, population change, or improved capture; document the cause and retain the supporting analysis.

Never invent a universal pass threshold. CMS requirements differ by measure and year. Use the tolerances, edits, and definitions in the applicable technical specification, and escalate unexplained differences rather than silently adjusting the result.

Step 5: Investigate exceptions and control corrections

An exception log is part of the validation evidence, not an administrative afterthought. For each failed test or unexplained variance, record:

  • the rule, field, measure, or reconciliation that failed;
  • the affected population and count;
  • the suspected cause and the owner assigned to investigate;
  • the evidence reviewed, such as source records, query output, interface logs, or mapping tables;
  • the corrective action, including code, configuration, source, or specification changes;
  • the rerun date and result;
  • the residual limitation, if the issue could not be fully corrected; and
  • the independent reviewer’s disposition and approval.

Preserve the pre-correction result as well as the final result. Overwriting a failed file removes the history needed to explain why the submitted number changed. If a manual adjustment is unavoidable, require a second-person review and retain the calculation, rationale, source evidence, and approval.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Step 6: Assemble reproducible evidence

Another reviewer should be able to recreate the submission without relying on an individual employee’s memory or an unstable live dashboard. Retain a controlled package containing:

  • the final CMS specification and scope record;
  • raw or frozen source extracts used for the submission;
  • queries, scripts, formulas, mapping tables, and parameter files;
  • intermediate transformation outputs and record counts;
  • completeness, format, duplicate, and consistency-test results;
  • reconciliation workpapers and sampled primary-source evidence;
  • the exception log, correction history, and rerun outputs;
  • the final submitted file and a checksum or other version identifier where your controls use one;
  • preparer, reviewer, and approver sign-offs; and
  • an index explaining how each artifact supports a reported field or measure.

For QRS and HEDIS-related data, CMS materials specifically call attention to source review, data capture, file formats, quality-control processes, effects on reported rates, and primary-source verification where appropriate. The 2027 QRS technical specifications also describe HEDIS audit results at the measure and indicator level and whether measures may be publicly reported. Retain the source and audit evidence needed to support those determinations.

Step 7: Complete independent review and freeze the submission

The person who prepared a calculation should not be the only person deciding that it is ready. Assign a reviewer who understands the specification but was independent of the preparation for the controls being reviewed.

  1. Compare the final file column by column with the current specification.
  2. Confirm that all open exceptions have an approved disposition and that corrections were rerun.
  3. Reperform key reconciliations and inspect a sample of source records.
  4. Verify that the submitted copy is identical to the approved copy and that no post-approval edits occurred.
  5. Sign and date the approval, naming the specification version and data cutoff.
  6. Freeze the evidence package and record its storage location and retention owner.
  7. Transmit through the contractually required mechanism and retain the submission confirmation.

Coordinate this review with the external validator early enough to provide extracts, definitions, workpapers, and sampled records without changing the frozen submission. Internal review prepares the organization; it does not replace an independent validation that CMS requires.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Part C and Part D Reporting Requirements: plan for retrospective validation

The CMS Part C and Part D Data Validation program describes an annual retrospective validation performed in the year following the contract year. The sponsoring organization must select an external contractor to conduct the work independently under CMS specifications. The organization’s own staff do not perform that independent validation.

Before the contract year closes, confirm the validator’s information requests, sampling expectations, file definitions, and delivery dates. Maintain a clean, frozen copy of the data used for the original submission and the evidence that supports it. If the validator identifies a discrepancy, provide the lineage and exception history rather than creating a new result that cannot be tied to the submitted file.

Because CMS lists validation packages through 2026 and notes that specifications may vary by year, do not assume that a prior validation package, sample design, or deadline carries forward. Check the current CMS page and the final contract-year instructions.

QRS and HEDIS-related quality data: validate the source as well as the rate

Quality-measure validation is not limited to recomputing a numerator and denominator. Review whether the source captures the required event, whether the transfer preserves it, and whether the measure logic uses it correctly.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Source and capture review

For each source, document what is captured, when it becomes available, which fields are mandatory, and how late, corrected, or deleted records are handled. Confirm that supplemental files use the required format and that interfaces did not truncate fields, shift dates, or change code values. Where a measure depends on clinical or administrative evidence, retain the primary-source record or an approved reference to it for sampled cases.

Rate-impact analysis

Quantify how source gaps, exclusions, mapping changes, and corrections affect the reported rate. Explain whether a change reflects a true population shift, a specification revision, improved data capture, or an error. Keep the before-and-after calculations and approval for any correction that changes a published or submitted result.

Audit result and public-reporting status

Use the applicable QRS technical specification to determine how audited rates or calculations are reported at measure and indicator level and whether a measure may be publicly reported. Do not infer public-reporting status from an internal dashboard or from a previous year’s treatment.

Do not confuse reporting validation with RADV

Medicare Advantage Risk Adjustment Data Validation examines whether risk-adjustment diagnoses are supported by enrollee medical records. Unsupported diagnoses may lead to overpayment recovery. That audit has a different purpose and evidence standard from validating Part C or Part D reporting data and from QRS/HEDIS source and rate checks.

A shared data warehouse does not make the controls interchangeable. Label workpapers by program, apply the relevant definitions, and obtain the evidence each program requires. A quality-reporting record cannot substitute for medical-record support in RADV, and a RADV medical record does not prove that a quality-measure denominator was complete.

Star Ratings integrity makes completeness a governance issue

Data-quality failures can affect more than a single submission. CMS’s 2027 Medicare Advantage and Part D Star Ratings fact sheet, published October 8, 2026, describes completeness checks using Part C Reporting Requirements data for IRE appeals information and scaled reductions when CMS identified data-integrity issues.

The practical implication is to escalate missing, inconsistent, or manipulated-looking data through compliance and senior quality governance. Do not treat a completeness failure as a cosmetic file-edit problem when the data may feed ratings, public reporting, oversight, or corrective action.

Common failure modes and the corrective response

Failure mode Why it happens Corrective response
Using last year’s specification The workbook or code was copied forward without confirming revisions. Replace it with the final contract-year specification, document the version, and rerun definition, format, and calculation tests.
Totals match but records are not traceable A manual spreadsheet or undocumented aggregation produced the number. Rebuild lineage from source extract through transformation and retain the logic and intermediate counts.
Missing values are silently converted to zero Null handling was not defined or the file export applied a default. Separate zero, null, unknown, and not-applicable states and test each against the CMS allowed values.
Duplicate or omitted events Joins multiplied records or a date filter excluded late-arriving data. Define the record key, run duplicate and coverage tests, inspect boundary dates, and document exclusions.
Source interface changed A vendor, system, code set, or file layout changed during the measurement period. Compare pre- and post-change capture, perform a rate-impact analysis, and retain change-control evidence.
Internal review is mistaken for independent validation The organization assumes its own sign-off satisfies the external requirement. Plan the required external retrospective validation and provide the frozen submission evidence to the contractor.
RADV evidence is used for quality reporting Teams combine programs because they share member or diagnosis data. Separate program scope, definitions, samples, and evidence requirements.

Final pre-submission checklist

  • Contract, reporting year, data period, section, measure set, and owner are documented.
  • The final CMS specification version and date are stored with the workpapers.
  • Every submitted field has a source, transformation, definition, and accountable owner.
  • Required fields, formats, allowed values, dates, duplicates, and omissions were tested.
  • Numerators, denominators, totals, and categories reconcile to source data.
  • Outliers and material changes have documented explanations.
  • Exceptions show correction, rerun, residual limitation, and approval.
  • Primary-source evidence is retained where the measure or audit requires it.
  • The final file matches the approved file and the submission confirmation is retained.
  • The external validator has been engaged when Part C or Part D retrospective validation applies.
  • CMS updates were checked close to submission and any change impact was recorded.

Frequently Asked Questions

Can our internal quality team perform the required Part C or Part D independent validation?

No. CMS describes the annual retrospective validation as independent work performed by an external contractor selected by the sponsoring organization. Internal staff should prepare, test, and preserve the data, but their review does not replace that independent validation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Is there one CMS pass rate or tolerance that every Medicare Advantage quality measure must meet?

No universal threshold is established for all measures. Apply the edits, definitions, tolerances, and evidence requirements in the final specification for the applicable contract year and reporting section.

How far back should validation evidence be retained?

The applicable CMS contract, reporting specification, audit instructions, and your organization’s records policy determine retention. At minimum, preserve the frozen submission, source extracts, transformation logic, tests, exceptions, approvals, and submission confirmation for the period required by those authorities.

What should we do when CMS changes a specification shortly before submission?

Record the revised document version and effective date, perform an impact assessment, rerun every affected extraction and test, update the exception and approval records, and submit only the file produced under the final applicable instructions.
