Symptoms can offer clues, but they cannot reliably tell you whether you have COVID-19 or flu; colds overlap too. A test is needed to confirm COVID-19 or flu. If you are at higher risk of severe illness, contact a healthcare provider promptly rather than waiting for symptoms to make the diagnosis clear.
Can symptoms tell COVID-19, flu, and a cold apart?
No. COVID-19 and flu share many symptoms, including fever or chills, cough, shortness of breath, fatigue, sore throat, a runny or stuffy nose, aches, headache, vomiting, and diarrhea. The CDC says symptoms alone cannot distinguish flu from COVID-19: many of their signs and symptoms are the same.
A cold often causes milder symptoms, especially a runny or stuffy nose, while flu more often starts abruptly and feels more intense. Those are tendencies, not diagnostic rules. COVID-19 can range from mild to severe, and cold and flu symptoms overlap. Even a symptom pattern that seems typical of one illness cannot confirm it.
How the usual symptom patterns compare
| Pattern | Common cold | Flu | COVID-19 |
|---|---|---|---|
| Onset | Usually gradual. | Often abrupt. | Symptoms overlap with flu; timing alone cannot confirm COVID-19. |
| Typical intensity | Usually milder than flu. | Often more intense than a cold. | Can range from mild to severe. |
| Nose symptoms | Runny or stuffy nose is common. | Can occur, but is less prominent in the CDC comparison. | Runny or stuffy nose can occur. |
| Fever, chills, aches, and fatigue | Fever and aches are less typical; symptoms are often mild. | Fever or chills, body aches, and fatigue are more common. | These symptoms can also occur, so they do not separate it reliably from flu. |
| Change in taste or smell | Not a reliable way to distinguish a cold in this comparison. | Can occur. | More frequent than with flu, but not a dependable stand-alone test. |
The CDC’s cold-versus-flu comparison describes these general patterns. Use them to understand what may be more typical, not to score symptoms or self-diagnose.

When and how to test
Testing is useful when knowing the cause could affect treatment or how you reduce the chance of spreading illness. Some FDA-approved rapid tests for home use detect both influenza A and B and SARS-CoV-2. A combination test can help check for both flu and COVID-19, but a rapid result has limitations.
- COVID-19: A viral test can identify current SARS-CoV-2 infection. NAATs, including PCR tests, are more likely to detect the virus than antigen tests. A negative antigen result does not rule out COVID-19; follow the FDA’s current instructions for whether and when to test again. See the CDC’s COVID-19 testing guidance.
- Flu: Rapid flu antigen tests can provide results quickly, but may be less accurate than other flu tests. A negative rapid result does not necessarily mean you do not have flu. The CDC describes testing methods and rapid-test limitations.
- Both viruses: Molecular testing may be recommended for symptomatic people suspected of having flu, COVID-19, or both when results can support timely treatment decisions. The CDC’s respiratory-virus testing guidance explains the role of testing.
Home-test availability and labeling can change. Check the current instructions for the specific test you use, including which viruses it detects and what to do after a negative result. A negative rapid test should not be treated as proof that you are uninfected when symptoms or exposure still raise concern.

If you are at higher risk, contact a clinician early
If you may be at increased risk of severe illness and suspect COVID-19 or flu, contact a healthcare provider promptly. Antiviral treatment works best when started soon after symptoms begin, and the treatment windows are short. CDC guidance gives 5–7 days after symptom onset for starting COVID-19 treatment and 2 days for starting flu treatment; these are treatment-start windows, not a reason to wait until the last day. A clinician can assess your situation and whether testing or treatment is appropriate. See the CDC’s common-cold and respiratory illness treatment guidance.
When symptoms need urgent medical attention
Seek immediate medical care for difficulty breathing, persistent chest or abdominal pain or pressure, confusion, inability to wake or stay awake, or seizures. Other concerning signs include not urinating, severe muscle pain, severe weakness or unsteadiness, and a fever or cough that improves and then returns or worsens. The CDC notes that warning-sign lists are not exhaustive: seek care for any severe or worrying symptom. See its flu emergency warning signs.

What to do with an ordinary cold-like illness
Many respiratory infections begin with symptoms that are hard to identify by appearance alone. If symptoms are mild and you are not at higher risk, consider testing when the result could change what you do, and follow current test instructions. Most people with a common cold do not need specific treatment; antibiotics do not treat viral colds. If symptoms worsen or concern you, contact a healthcare provider.
