Set approval and escalation rules before an AI agent goes live, and enforce them in deterministic code at the orchestrator or side-effecting tool boundary—not by asking the agent to decide when it should be supervised. Allow low-risk, reversible actions within guardrails; require human review for consequential, sensitive, destructive, ambiguous, or hard-to-reverse actions. Define who reviews each request, what context they see, how long they have to respond, and what happens if they do not.

A workable policy routes the right actions to a person without making every routine action a rubber stamp. The steps below turn that principle into a risk policy, an approval workflow, and an auditable operating process.

Why do AI agents need approval and escalation rules?

An AI agent can do more than produce text: when connected to tools, it may change records, send messages, modify systems, or initiate other side effects. A mistake in a suggestion can be corrected before anyone acts on it; a mistake executed through a tool may be costly, expose sensitive information, disrupt service, or be difficult to undo.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Approval rules decide which actions may run automatically and which must pause for a person. Escalation rules cover what happens when the assigned reviewer cannot decide, is unavailable, or misses the response deadline. Together, they define a controlled boundary between an agent’s autonomy and human judgment.

Guidance from the Canadian Centre for Cyber Security puts responsibility for deciding when approval is required on system designers or operations, rather than delegating that policy choice to the agent. Microsoft Learn’s responsible AI guidance likewise recommends keeping a human involved in consequential actions and defining escalation paths for cases the agent should not resolve alone. The operational implication is important: the model may provide information to support a decision, but it should not be the authority that lowers its own approval requirements.

Which actions should require human approval?

Classify actions according to their potential impact and the context in which they occur. Require approval when an action could create material harm, expose sensitive data, affect compliance, cause a broad or irreversible change, or when the agent cannot establish what it is being asked to do with enough confidence.

Do not make the policy a simple list of tool names. The same tool can perform a harmless lookup or a consequential update. Assess the specific operation, target, data, permissions, environment, and likely effects.

Build an action inventory

List the operations the agent can request, including actions performed by tools called by other agents. For each operation, record the following fields before deciding its approval tier:

  • Target resource and operation: what the agent will read, create, change, delete, send, or trigger.
  • Data sensitivity: whether personal, confidential, regulated, or otherwise restricted information is involved.
  • Blast radius: how many records, users, systems, or external parties could be affected if the action is wrong.
  • Reversibility: whether the change can be undone completely and reliably, and whether undoing it would itself create harm.
  • Financial or compliance impact: whether the action can commit funds, change an obligation, or affect a regulated process.
  • Requester identity and environment: who initiated the work and whether it concerns a test, staging, or production system.

These attributes make the policy explainable and reviewable. They also help distinguish a narrowly scoped, reversible operation from a broad production change even when both use the same integration.

Use risk tiers that change what the system does

A four-tier scheme is a practical starting point, not a universal standard. Define each tier with examples from your own systems, then connect it to an explicit runtime outcome.

Tier Typical characteristics Default handling
Low Limited impact, reversible, low-sensitivity operation in an approved scope. May run automatically within input, output, and tool-argument guardrails.
Medium Meaningful but bounded impact; some uncertainty, sensitivity, or recovery effort. Allow only when the relevant conditions are explicitly covered by policy; otherwise route for review.
High Consequential, privacy-sensitive, compliance-sensitive, broad, or difficult-to-reverse action. Pause for human approval before execution.
Critical Potentially severe impact, destructive scope, sensitive security operation, or major uncertainty. Require stronger review or escalation as defined by the organization; do not execute while approval is pending.

Tier names alone do not protect anything. The policy needs rules that map specific action attributes to a tier and a tier to a control. For example, it can require approval whenever an operation affects production records, involves restricted data, or exceeds an established scope. Set the thresholds with the owners of the affected systems and applicable legal or compliance requirements; the sources do not establish universal numeric cutoffs.

Escalate ambiguity instead of guessing

Route an action upward if the request, target, arguments, evidence, or scope is incomplete or contradictory. Also re-evaluate risk at runtime when signals such as unusual frequency, time of day, source location, privilege changes, or anomalous context make a normally routine action less trustworthy. A request should not stay low-risk merely because its tool is usually low-risk.

Where should the approval check run?

Place the check in deterministic orchestrator code or immediately at the boundary of every side-effecting tool, before execution. A model-generated message such as “I will ask for approval” is not itself a control: the agent could omit it, phrase it incorrectly, or proceed through a different tool path.

Separate automatic guardrails from human decisions. Code can validate tool arguments, enforce allowed targets, check input and output constraints, and reject malformed requests. Reserve human review for choices requiring judgment about intent, context, or consequences. AWS’s Agentic AI Lens warns against both extremes: routing every action to review encourages rubber-stamping, while routing none leaves autonomy unbounded.

Cover nested tools and agent handoffs

In a manager or multi-agent workflow, do not assume a guardrail attached to one agent protects tools called by another agent. An action may be delegated, transformed, or issued through a nested tool path. Apply independent policy checks wherever an action can cause a side effect, use least-privilege credentials, and keep policy enforcement outside the agent’s ability to change.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For sensitive cybersecurity operations, the review should verify the target, scope, identity, exact arguments, and authorized engagement window before execution. If the reviewer is unavailable or the timeout expires, fail closed rather than letting the operation continue.

What should a reviewer see before approving an agent action?

Show enough information to decide what will happen, why it is proposed, and what could be affected. A button labeled “Approve” without the underlying action and evidence invites accidental or uninformed approval.

Present the request in a review screen or event containing:

  • the agent identity and the user or process that initiated the work;
  • the exact tool name and arguments that will be executed, not a paraphrase;
  • the data sources or evidence the agent used, with references that let the reviewer inspect them;
  • the intended effect and the affected records, systems, or external parties;
  • the assigned risk tier and the policy reasons that produced it;
  • whether the change can be reversed and any material consequence of carrying it out;
  • the deadline for review and the result if that deadline is missed; and
  • links to relevant prior decisions, where available.

Keep the original user request with the approval record. Make the available decisions explicit: approve, reject, request changes, or escalate. If a reviewer requests changes, the revised action must be evaluated again at the control boundary; approval of one set of arguments must not silently authorize a different one.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reviewers should be able to distinguish evidence from the agent’s interpretation of that evidence. If a source is missing, stale, or conflicts with another source, surface that limitation rather than presenting a confident summary as a verified fact.

How do you design an escalation path that will not stall?

Treat approval as a small state machine with named transitions, owners, and timeout outcomes. A minimal path is pending → approved or pending → rejected; when a decision needs another owner, use pending → escalated → approved or pending → escalated → rejected.

  1. Assign a primary reviewer. Route the request to someone authorized and competent to assess that action, rather than to a generic inbox with no clear owner.
  2. Name a backup route. If the primary reviewer is unavailable, send the request to a named backup or an on-call queue with equivalent authority.
  3. Set a response window. Define how long the request may remain pending based on operational urgency and risk. The policy should state the deadline rather than leaving the agent to infer one.
  4. Choose an explicit timeout behavior. For high-risk actions, pause or reject by default when no decision arrives. Do not let a timeout become implicit approval.
  5. Notify the requester. Tell the user or initiating process when an action has been blocked, escalated, rejected, or timed out, and explain the next available step.
  6. Record the transition. Preserve who handled the request, when it changed state, and the reason for the decision.

The response window and backup design depend on your operations; the available guidance does not prescribe a universal deadline. The important invariant is that an unavailable reviewer cannot cause an unsafe action to execute by default.

How do approval workflows work in agent frameworks?

Frameworks can provide mechanics for pausing and resuming work, but they do not decide your organization’s risk policy. Verify that the implementation covers every relevant tool, preserves the pending state, identifies the exact action awaiting approval, and handles rejection and timeout without executing it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Pause and resume rather than re-create the action

OpenAI’s SDK documentation describes an approval predicate on tools: when approval is required, a run pauses with an interruption and can resume from the same serialized state after a decision. This pattern helps preserve the request context through the review. Treat the serialized state as sensitive if it contains user data, tool arguments, or evidence, and ensure that the resumed action remains the one the person approved.

Handle approval events and repeated tool calls

Microsoft’s Agent Framework documentation describes emitting an approval request event and expecting the caller to supply an approval response. A run may produce more than one tool call, so the application needs to handle repeated approval rounds instead of assuming that one approval clears the entire task.

These behaviors are implementation patterns, not substitutes for policy. Product APIs and model support can change, so check the current framework documentation and confirm the behavior in the version you deploy. In either pattern, keep the enforcement decision deterministic and outside the model’s control.

Compare implementations on operational behavior

When choosing or reviewing an implementation, evaluate the following capabilities rather than relying on the presence of an “approval” feature in a product description.

Capability Question to verify
Trigger determinism Is approval required by a fixed policy rule, or can a model’s judgment be the only trigger?
Tool-boundary coverage Does the control cover nested agents and every route to a side-effecting operation?
Pause and resume durability Can work resume with the same state and exact proposed action after the decision?
Reviewer context Can the review request expose arguments, evidence, effects, risk reasons, and affected resources?
Timeout and backup handling Can the application route to a backup, enforce a deadline, and fail closed when necessary?
Identity and least privilege Can it preserve agent and user identity while limiting the credentials available to the agent?
Audit and policy versioning Can decisions be tied to a policy version and a complete record of execution?
Monitoring Can operators detect anomalous activity, repeated retries, and changing approval patterns?
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Support on Ko-Fi

What should you log and monitor?

Keep an auditable record that connects the proposed action, the policy that evaluated it, the human decision, and what actually happened. A decision without its arguments or execution result may not be enough to investigate a failure or explain why a system acted.

Log the approval request, policy version, agent and user identities, exact arguments, evidence references, risk classification, reviewer identity, timestamps, decision rationale, escalation events, timeout outcome, and execution result. Preserve the original request as well as the reviewer’s reason. Apply retention periods that meet your legal, regulatory, and organizational requirements.

Monitor more than approval volume. Useful operational signals include approval and rejection rates, time to decision, repeated retries, near misses, privilege changes, anomalous destinations, and drift from approved objectives. A sudden shift can indicate an overly broad policy, a changed workload, or an agent behaving differently than expected; investigate the cause before changing thresholds.

Use periodic policy review and testing to feed reviewer corrections and operational findings back into the rules. Test that high-risk requests pause, that a timeout does not execute them, that a backup path works, and that a changed tool argument is reviewed as a new action. Keep policy ownership with system designers or operations, not with the agent whose actions the rules constrain.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How can you put the rules into operation?

Start with a bounded set of actions and make the enforcement path testable before expanding the agent’s privileges. A policy is only useful if its classifications, review experience, escalation ownership, and logs work together.

  1. Inventory tool capabilities. Identify all operations that can change data or systems, including nested and delegated tools.
  2. Classify the actions. Apply impact, sensitivity, blast radius, reversibility, financial or compliance effect, requester, and environment to each operation.
  3. Write tier outcomes. Specify which actions may run automatically and which must pause, escalate, or reject. Route incomplete or ambiguous cases to a human rather than guessing.
  4. Implement the deterministic check. Enforce policy before side effects at the orchestrator or tool boundary, independently of model-generated prompts.
  5. Build the review record. Provide exact arguments, evidence, affected resources, risk rationale, reviewer actions, and an explicit deadline.
  6. Configure the escalation state machine. Assign a primary reviewer and backup, define timeout behavior, and notify the requester when progress stops.
  7. Test failure paths. Exercise unavailable reviewers, expired deadlines, rejected actions, changed arguments, nested tool calls, and repeated approval events.
  8. Monitor and revise. Review decisions, delays, retries, anomalies, and near misses; update policy through an accountable review process.

Do not expand autonomous access until the controls are verified for the specific tools and environment involved. A test environment can confirm workflow behavior, but it does not establish that a policy is adequate for production or for a different risk profile.

Frequently Asked Questions

Can the person who requested an agent action approve it too?

A policy can prohibit self-approval for actions where independent judgment matters, such as sensitive or consequential changes. Decide this by action tier and organizational roles, and record the approver’s identity so the rule can be audited.

Should approval requests expire if the underlying data changes?

For time-sensitive or state-dependent actions, make the review request expire or require a fresh check when its evidence or target state changes. Otherwise a person could approve an action based on context that is no longer true.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

How should teams handle emergency actions when reviewers are unavailable?

Define an emergency route in advance, with a narrow scope, authorized roles, and a record of the reason for using it. Do not create an informal bypass that allows an agent or requester to disable the approval policy without a trace.

How often should an AI-agent approval policy be reviewed?

Review it after a material change to tools, permissions, workflows, or the agent’s objectives, and periodically using operational signals such as near misses and changing rejection patterns. Applicable retention and compliance obligations may also shape the review process.
