Use a long, unique password, enable the strongest multifactor authentication (MFA) option your provider supports, and sign in only through the provider’s official website or app. Because portal features differ by provider, check its official instructions for setup and account recovery. Secure any health records you download, too: HIPAA protections may not extend to every service where you store a copy.
Secure your account in this order
- Open the official portal. Type the address from your provider’s official website or use its official app. If an email or text prompted you to sign in, verify the message through a provider channel you already trust before entering credentials. The Office of the National Coordinator for Health Information Technology (ONC) advises verifying a source before sharing personal or medical information; see its consumer guidance.
- Set a unique password. Use a password that you have not used for email or any other account. A password manager with a generator can help create and use a different password for each account.
- Turn on MFA if the portal offers it. Look in the portal’s account or security settings, then follow the provider’s official enrollment and recovery instructions. Prefer a passkey or security key if the portal and your device support one. If not, enable the strongest available option, such as an authenticator code or an approval prompt.
- Check recovery details. Make sure the email address and phone number on file are current. If the provider issues recovery codes, store and use them as the provider directs, separately from your password.
- Protect the device and session. Use a screen lock or passcode on your device. Sign out when finished on a shared device, and avoid saving your password on a computer other people use.
- Secure downloaded records. Store copies somewhere protected by a password or encryption, and review the privacy terms of any service you use to keep or share them.
Choose a strong password without outdated rules
NIST Special Publication 800-63B, Revision 4, sets a minimum of 15 characters for passwords used as a single authentication factor. It permits a minimum of eight characters when the password is used only as part of MFA. These are requirements for identity-system verifiers, not a promise that every patient portal follows them. Use the longest unique password the portal accepts.
NIST does not require arbitrary mixes of uppercase letters, numbers, and symbols, and says verifiers should not require routine password changes. If you have evidence your password was exposed or compromised, change it promptly through the official portal. A password manager can make unique credentials easier to create and use; NIST says verifiers should allow password managers and autofill.

MFA adds a separate login step, but methods do not offer identical protection against phishing. NIST distinguishes manually entered one-time codes—which are not phishing-resistant—from cryptographic authentication, which can be phishing-resistant.

Only choose among methods the portal actually offers. Do not buy a security key before confirming compatibility with both the portal and your device. NIST recommends phishing-resistant authentication in specified AAL2 contexts, but that does not mean every patient portal provides it.
Respond to a suspected password exposure
- Go to the portal through the provider’s official website or app and change the password.
- Review account activity for unfamiliar sign-ins if the portal provides that feature.
- Contact the provider using a phone number or website you verify independently—not contact details supplied only in a suspicious message.
- Follow the provider’s directions to restore or update MFA and recovery details.
Know where HIPAA protections end
HIPAA generally requires covered providers and health plans to safeguard health information; its Security Rule addresses electronic protected health information. Safeguards can include access controls such as passwords or PINs, encryption, audit trails, and workstation security, as ONC explains in its security guidance.

Those protections do not necessarily follow a record after you download or share it with an organization outside HIPAA coverage. ONC’s consumer guidance and Blue Button FAQ explain that privacy and security safeguards depend on the organization providing access. ONC’s consumer guidance also cautions: “Never post anything online that you don’t want made public.” Protect copies you control and check the privacy terms of any destination service.
