Do not immediately rerun a failed migration or issue a rollback command. First pause further schema deployments and determine exactly what ran, what the database actually changed, and what the migration tool recorded. A failed migration may leave part of its work in place, and the safe recovery depends on the database, migration tool, migration configuration, and production writes since the failure.
Stabilize the deployment and preserve evidence
Stop new schema deployments to the affected database while you assess the incident. Preserve the deployment logs and record the release, migration identifier, exact error, database engine and version, migration-tool version, and time window. Keep the migration file and its history unchanged for now: editing or rerunning them before checking the live state can make the incident harder to diagnose.
Coordinate application changes with the database response. If the failed release is still running, determine whether that application version can safely operate against the schema as it stands; do not assume either that it can or that reverting application code will undo database changes.
Find out what actually happened
Check whether the migration was atomic
Whether a failed migration leaves changes behind depends on the database’s DDL transaction support and the migration’s configuration. In Django, operations run inside one transaction by default on SQLite and PostgreSQL; backends without DDL transactions, including MySQL and Oracle in Django’s documentation, run operations without a transaction. Django migrations can also be configured as non-atomic. Check the deployed backend and migration code rather than inferring behavior from the framework name alone. Django’s migration documentation

Rails similarly wraps a migration in a transaction when the database supports transactional schema changes. Its Active Record Migrations guide cautions that, without that support, statements that succeeded before a failure may remain applied. Inspect database state and migration history separately
Use your database’s inspection and monitoring procedures to establish which statements took effect, which schema objects or data changed, and whether any writes occurred during the affected period. Separately check the migration tool’s history for a success, failure, or partial-progress record. A failed deployment log alone does not prove the database is unchanged, and a migration-history entry alone does not establish the live schema.

Include data effects in the assessment: a migration may transform or delete values as well as change schema. Identify whether any affected data can be reconstructed and whether valid application writes have continued since the change.
Choose a recovery method that fits the damage
These approaches are conditional alternatives, not interchangeable rollback buttons. Base the choice on reversibility, partial application, application/schema compatibility, valid writes since the change, recovery time and downtime, and whether migration history can be made consistent with the database.

Review rollback output before execution
If the migration tool can generate rollback SQL, preview it and compare it with the live database before running it. Liquibase recommends a corresponding SQL preview before rollback. Confirm the intended tag or target, dependencies, constraints, and data effects. Liquibase also warns that rollback can lose data as data changes over time and that inconsistent handling across environments can cause database drift. Do not treat a schema rollback as data recovery. If rows or values were dropped or overwritten, assess a tested backup or point-in-time recovery plan. Before restoring, determine how to preserve or reconcile valid writes made after the recovery point; otherwise, recovering the earlier database state may erase newer legitimate work.

Account for migration-tool behavior
Django
Use the deployed database backend and the migration’s atomic setting to assess whether operations could have partially applied. Django’s documented defaults differ between backends, so a default observed in one environment is not a guarantee for another. Django migrations
Rails
Check whether the database supports DDL transactions and whether the migration disables them for operations that cannot run transactionally. If it does not support transactional schema changes, inspect for earlier statements that may have succeeded before choosing cleanup or a corrective migration. Rails Active Record Migrations

Liquibase
Rollback behavior depends on the changeset and target. Review its SQL preview and any custom rollback logic against the actual database; confirm version and edition before relying on a particular command or feature. Liquibase 6.0 rollback reference
Flyway
Flyway’s migration documentation notes that when a database does not cleanly support transactional DDL, a failed migration may not be cleanly rolled back; manual cleanup and history repair may be needed. After correcting the database state, a Flyway repair operation may resolve a failed history entry. It is not a substitute for checking and fixing the schema itself. Verify the behavior for the exact database and Flyway version, and maintain a tested backup and restore strategy. Flyway migrations documentation

Reconcile history, validate, and resume deployments
- Apply only the reviewed recovery action. Use the normal controlled deployment or incident procedure, with the target state and expected data effects understood.
- Reconcile the migration record. If manual cleanup was required, ensure the tool’s migration history agrees with the resulting database state. Do not resume later migrations while history and schema disagree.
- Validate the result. Compare the live schema and migration history with the intended state, verify affected data, and check that the application version can operate with the recovered schema.
- Resume in a controlled manner. Retry or deploy subsequent migrations only after the state is consistent and the application and database have passed the relevant checks. Preserve the evidence and recovery actions in the incident record.
