Reduce false or unsafe AI outputs with a repeatable lifecycle: define acceptable behavior and harms, test realistic and adversarial cases, measure factuality and safety separately, fix the specific failure, rerun the original and regression tests, and continue monitoring after release. A benchmark score alone is not enough when the system retrieves documents, remembers context, calls tools, or can trigger real-world actions.
Start with a written definition of “good” behavior
Testing becomes useful only when the team can decide, consistently, whether an output passes. Write the intended behavior before collecting scores. Describe the deployment context, users, data, connected tools, likely misuse, and consequences of an error. A customer-support assistant, a medical-triage helper and an internal coding agent need different thresholds and different failure controls.
Define factuality as a measurable property
“Accurate” should refer to an observable condition, such as:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- the answer matches an authoritative reference answer;
- each material claim is supported by a specified retrieval passage or tool result;
- the response conforms to a required JSON or database schema;
- the model asks for clarification when the question is ambiguous; or
- the model abstains when the available evidence is insufficient.
Set separate thresholds for critical and non-critical tasks. In a high-impact workflow, an unsupported but fluent answer may be worse than an explicit “I do not have enough information.” Record the allowed error rate, the required abstention behavior and the cases that always require human review.

Define unsafe behavior and escalation
Create a harm taxonomy tied to the application. Typical categories include self-harm, violence, illegal activity, privacy exposure, cyber abuse, discrimination, prompt injection, data exfiltration, unauthorized tool use and domain-specific harms. For every category, document allowed, disallowed and borderline examples, plus the action to take when a case is uncertain. A safe refusal, a limited answer, a redirect to help, a block or a human escalation are different outcomes and should not be collapsed into one “safe” label.
Use the NIST lifecycle instead of a one-time test
NIST’s AI Risk Management Framework 1.0, released on January 26, 2023, organizes risk work into four functions: Govern, Map, Measure and Manage. Its Playbook says the actions are meant to be adapted to the organization and use case, not treated as a rigid checklist. Apply the functions as a loop:
- Govern: assign owners, approval authority, incident handling and documentation requirements.
- Map: describe users, operating conditions, assets, harms, dependencies and knowledge limits.
- Measure: run documented tests with defined metrics, evaluators and uncertainty.
- Manage: prioritize findings, apply mitigations, accept or transfer residual risk, and monitor the live system.
Re-enter the loop whenever the model, system prompt, retrieval index, tool, policy, user population or deployment environment changes.
Build test sets that resemble real use and real misuse
Start with representative workflow data
Sample prompts from the actual workflow, not only from a benchmark. Stratify by task, user type, language, input length, data source, time period and outcome severity. Hold back a versioned regression set that is never used to tune prompts or policies. Keep personally identifying or confidential data out of shared test artifacts; substitute realistic synthetic or de-identified examples where possible.
Add difficult and rare cases deliberately
Include out-of-domain questions, ambiguous wording, missing fields, contradictory instructions, stale documents, malformed tool responses, long context, multilingual requests, coded language and unusual combinations of otherwise ordinary inputs. Rare cases can dominate risk even when they contribute little to an average score. Label each slice so a passing aggregate cannot hide a failure concentrated in one language, customer group or workflow.

Create adversarial misuse sets
Adversarial prompts should attempt to elicit harmful output or bypass controls. Use paraphrases, role-play, indirect requests, conflicting system and user instructions, prompt injection in retrieved content, requests to reveal hidden data, tool-call manipulation and attempts to make the model continue after a refusal. Red-team the complete application, including retrieval, memory, guardrails, tools, credentials and downstream actions; testing the base model alone misses system-level failures.
Make evaluations reproducible and statistically honest
For every run, record the model identifier and version, system prompt, retrieval corpus and index version, tool definitions, tool results, temperature or other sampling settings, context limits, safety policies, evaluator instructions, test-set version and execution environment. Store the exact input and output, including intermediate tool calls when they matter.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Run multiple stochastic trials when sampling is non-deterministic. Report the distribution, confidence interval or other uncertainty measure rather than a single favorable completion. If a system fails one time in many attempts, that frequency still matters when the action is consequential. Repeat tests after infrastructure changes as well as model changes; a new parser, connector or memory layer can alter risk without changing the model weights.
Separate the questions your metrics answer
| Evaluation question | Useful measures | Important qualification |
|---|---|---|
| Is the answer factually supported? | Exact or semantic correctness, entailment against an authoritative answer, citation support and unsupported-claim rate | Define what counts as a material claim and how partial answers are scored |
| Does the response follow the required format? | Schema-valid rate, required-field completeness and parser success | A valid schema can still contain false or unsafe content |
| Does the system refuse or redirect harmful requests? | Unsafe-compliance rate, correct-refusal rate and over-refusal rate | Score borderline cases separately from clearly disallowed requests |
| Can the system cause a harmful side effect? | Unauthorized-action rate, argument-validation failures and end-to-end incident rate | Text-only testing cannot establish safety for an agent with tools |
| Is behavior stable? | Variance across repeated runs and worst-slice performance | Report the number of repetitions and sampling controls |
OpenAI’s guidance for third-party evaluations emphasizes that a useful report must state what claim the setup was designed to test and provide evidence that the result is valid. That means explaining why the test set represents the intended use, how labels were produced, and which limitations could change the conclusion.
Reduce false or hallucinated outputs
Improve grounding before changing the model
Check whether the needed information is present in retrieval results, whether sources are current, and whether ranking returns the relevant passage. Inspect the prompt instructions that govern evidence use. A model cannot reliably cite a fact that the retriever never supplied. Measure retrieval coverage and freshness separately from answer quality so a grounding failure is not misdiagnosed as a generation failure.

Bind claims to evidence
Require the model to cite the retrieved passage or tool result used for each material claim. Validate that the citation points to an available source and that the quoted span actually supports the claim. Do not accept a plausible URL or a citation generated from memory. If evidence is missing, route to an abstention or clarification path rather than allowing the model to fill the gap.
Constrain the output where the task permits
Use a strict schema for machine-consumed responses, enumerated values for classifications and deterministic post-processing for calculations. Validate every field before it reaches a user or downstream service. Schema validation prevents malformed data, but it does not prove that the values are true; retain factuality checks as a separate gate.
Use tools with verification
For arithmetic, lookups or database operations, prefer a trusted tool and verify its result before presenting it. Check units, date ranges, permissions and error states. Treat tool output as untrusted input when it can contain instructions or user-controlled text. A tool result should be bound to the answer that cites it, and stale or incomplete results should trigger an explicit limitation.
Set an abstention and human-review policy
Define confidence or evidence conditions that cause the system to abstain. Tune the policy against both accuracy and abstention cost: a stricter threshold can reduce unsupported claims while increasing unanswered questions. Send high-impact decisions, low-evidence cases and unresolved evaluator disagreements to a trained reviewer. Log the reason for abstention so teams can distinguish missing data from model uncertainty.
Reduce unsafe outputs with layered controls
Screen inputs and classify policy risk
Apply an input policy check before generation where it is appropriate, but do not rely on a single classifier. Test evasive wording, multilingual and coded requests, prompt injection, long contexts and instructions hidden inside documents. Keep the policy version with the test record so a later change can be attributed.

Limit what tools can do
- Use least-privilege credentials scoped to the smallest required data and operation.
- Expose narrow, typed tool schemas rather than a general shell or unrestricted API.
- Validate arguments, destinations, quantities and authorization on the server side.
- Sandbox code, network access and file operations when they are not essential.
- Require explicit human approval for irreversible, financial, safety-critical or privacy-sensitive actions.
- Apply rate limits, transaction limits and emergency revocation paths.
Test whether a malicious prompt can turn a read-only capability into a write, cross a tenant boundary, exfiltrate secrets or bypass confirmation. A refusal in the chat transcript is not a pass if the agent has already executed the dangerous action.
Validate outputs and provide safe redirects
Run output checks for prohibited content, personal data, secrets, policy violations and unexpected instructions before delivery or execution. A safe response should explain the permitted alternative when that helps the user, without revealing hidden policy text or sensitive internal details. Check that a redirect does not accidentally provide the harmful information in another form.
Red-team the system, then distinguish it from ordinary evaluation
Ordinary evaluations measure intended behavior on known tasks. Red teaming deliberately probes misuse, abuse, unexpected inputs and policy or security failures. Use internal experts, affected-domain specialists or qualified third parties when the team lacks the necessary perspective. OpenAI’s red-teaming guidance names Promptfoo as an open-source framework for evaluating prompts, agents and AI applications; a tool can organize attacks and results, but it does not replace a threat model or expert review.
Plan attacks around the harm taxonomy and the actual attack surface. Include direct requests, indirect prompt injection through retrieved pages, memory poisoning, tool-result manipulation, data-leakage attempts, repeated conversations and combinations of benign instructions that produce a harmful final action. Record whether the failure is textual only, exposes sensitive data, changes system state or creates an external side effect.
Use this failure-triage table
| Finding | Diagnose | Mitigation and retest |
|---|---|---|
| Fluent but unsupported factual claim | Check retrieval coverage, source freshness, prompt instructions and evaluator agreement | Improve grounding and source checks; require abstention when evidence is missing; rerun factuality and regression sets |
| Correct answer with a fabricated citation | Inspect citation generation and source binding | Bind citations to retrieved passages or tool results; validate links and quoted spans |
| Unsafe answer to a direct request | Identify the harm category and policy gap | Add targeted refusal and redirect examples, policy checks and adversarial variants; rerun the red-team set |
| Unsafe tool action | Review permissions, argument validation and confirmation flow | Use least privilege, schemas, sandboxing and human approval; test end-to-end side effects |
| Failure only in long or multilingual context | Measure context length, language and instruction conflicts | Add stratified tests and context-aware defenses; monitor those slices separately |
| Large variance across runs | Repeat with controlled sampling and inspect unstable components | Increase repetitions, set risk-based thresholds and report the distribution rather than one output |
Apply a targeted fix and prove that it worked
Change one control or a clearly documented bundle of controls at a time when practical. Examples include improving retrieval filters, adding citation validation, tightening a tool schema, changing a refusal example, inserting a human-approval gate or lowering an action limit. Preserve the failing prompt exactly, add paraphrases and related cases, and rerun the original test set, the held-out regression set and the adversarial variants.

Compare more than the target metric. A mitigation that reduces unsafe compliance but causes widespread over-refusal may be unsuitable; a grounding change that improves accuracy but increases latency or exposes more sensitive passages needs a broader decision. Record the before-and-after result, uncertainty, affected slices, new limitations and the owner who approved residual risk.
Report evidence so another team can judge it
A credible evaluation report states:
- the claim or risk being tested and the deployment context;
- model, application, policy and system versions;
- how the test set was sampled, labeled and protected from leakage;
- metrics, thresholds, repetitions and uncertainty;
- evaluator instructions, disagreement handling and relevant expertise;
- tools, retrieval sources, environment and side effects available during tests;
- representative failures, including severity and exploitability;
- known limitations and slices that were not covered;
- the exact mitigation change; and
- post-mitigation results on original, regression and adversarial cases.
Keep an incident corpus and regression corpus under version control. Preserve enough context to reproduce a failure without retaining unnecessary personal data. Independent reviewers or affected-domain experts are especially valuable when the risk involves health, finance, employment, minors, privacy or public safety.
Set release gates and monitor production
Evaluate before deployment and at regular intervals in operation. A release gate can require zero failures in specified catastrophic categories, a maximum unsafe-action rate, a minimum evidence-supported answer rate, and successful completion of all tool-permission tests. Choose thresholds from the harm model rather than from a convenient benchmark average.
In production, monitor refusal and escalation rates, unsupported-claim reports, citation failures, policy-classifier alerts, tool denials, authorization errors, latency and changes in traffic mix. Sample conversations for expert review under an appropriate privacy process. Watch for drift in language, topics, retrieval sources and user behavior. Give operators a rapid rollback or disable switch, and define how incidents are triaged, contained, communicated and added to the regression corpus.
NIST’s core guidance calls for documented measures and limitations, regular safety evaluation, performance measurement under deployment-like conditions, production monitoring, documented generalization limits and safe failure beyond the system’s knowledge limits. Those requirements matter after launch as much as before it.
Choose an evaluation approach that fits the risk
| Approach | Best use | What it can miss |
|---|---|---|
| Fixed benchmark | Tracking a stable capability or regression over time | Deployment-specific harms, novel attacks and tool side effects |
| Representative human evaluation | Judging nuanced quality, usefulness, borderline safety and domain correctness | Inconsistent labels, reviewer fatigue and limited scale unless the rubric is controlled |
| Automated graders | High-volume format, citation and policy checks | Grader bias, correlated model errors and weak judgment of subtle harms |
| Adversarial red teaming | Discovering misuse paths, prompt injection and security failures | Coverage gaps if attackers lack domain knowledge or access to the real environment |
| End-to-end agent testing | Verifying retrieval, memory, tools, permissions and real side effects together | Higher cost and operational complexity; requires safe test accounts and sandboxes |
Compare approaches on factuality and safety coverage, deployment realism, adversarial depth, reproducibility, evaluator independence, uncertainty reporting, tool and agent coverage, privacy handling, operational cost and post-release monitoring. Use a combination when the system is consequential; no single score establishes trustworthiness.
A practical implementation sequence
- Map the system: inventory prompts, models, retrieval, memory, tools, data flows, users and actions.
- Write the policy: define factuality, allowed and disallowed behavior, abstention, escalation and action-approval rules.
- Build the corpus: collect representative tasks, high-risk cases, rare slices and adversarial variants; create a held-out regression set.
- Instrument the run: capture versions, settings, tool traces, evaluator rubric, outputs and uncertainty.
- Measure separately: score correctness, evidence support, format, refusal, privacy, security and side effects.
- Prioritize: rank findings by severity, likelihood, exploitability, affected users and reversibility.
- Mitigate: use grounding, abstention, validation, least privilege, sandboxing, rate limits and human approval where appropriate.
- Retest: rerun the original failures, regression set, adversarial variants and deployment-like end-to-end scenarios.
- Release with gates: document residual risk, owner approval, rollback conditions and monitoring alerts.
- Learn in operation: review incidents, update the taxonomy and add every confirmed failure to the versioned corpus.
The goal is not to claim that a model is universally safe or truthful. The goal is to make the system’s limits visible, prevent foreseeable failures, detect new ones quickly and ensure that uncertainty results in a safe action rather than a confident guess.
