Tinnitus can remain after vestibular schwannoma (acoustic neuroma) surgery, radiosurgery, fractionated radiotherapy, or observation. The practical approach is to reassess hearing— including speech understanding—then match rehabilitation and tinnitus-distress treatment to what remains. Education and counseling come first; hearing technology, cognitive behavioral therapy (CBT), and carefully adjusted sound enrichment can make tinnitus less intrusive, even when they do not remove it.

Keep every MRI and specialist follow-up arranged by your treating team. Tinnitus strategies address hearing access, sleep, concentration, and distress; they do not determine whether the tumor is controlled.

What tinnitus after vestibular schwannoma treatment means

Vestibular schwannoma (VS), also called acoustic neuroma, affects the hearing and balance nerves. Tinnitus is the perception of sound without an external source. It is the second most common presenting symptom of VS after hearing loss, according to a 2025 review. Treatment may control or remove the tumor while leaving altered hearing pathways, hearing loss, or reduced speech clarity. Consequently, tinnitus can persist, change, or become more noticeable in quiet surroundings even when follow-up imaging is reassuring.

There is no reliable, treatment-specific percentage for tinnitus persistence after VS surgery, radiosurgery, or radiotherapy in the available literature. Do not use a generic internet statistic to predict your outcome. Your prognosis and management depend on the treated ear’s residual hearing, speech discrimination, cochlear-nerve function, whether the other ear is affected, the treatment used, and how much tinnitus interferes with daily life.

The European Association of Neuro-Oncology (EANO) guideline describes observation, surgery, fractionated radiotherapy, and radiosurgery as VS treatment options. It emphasizes that controlled-trial evidence is limited and that treatment and follow-up should reflect the clinical presentation, tumor size, and expertise of a specialist center. Tinnitus care runs alongside—not instead of—that plan.

Start with a hearing and symptom reassessment

Arrange an appointment with your treating neurotologist or ENT specialist and an audiologist. A new baseline is more useful than relying on a pre-treatment audiogram because hearing, speech understanding, balance, and coping can change after treatment.

  1. Repeat pure-tone audiometry. This measures the quietest tones you can hear across frequencies and shows whether the treated ear has aidable hearing.
  2. Add speech audiometry. Ask for speech-recognition testing, not just tone thresholds. The 2025 VS review notes that speech discrimination may be worse than pure-tone results would suggest. A hearing aid that makes sounds louder cannot fully solve disproportionately poor speech understanding.
  3. Record communication function. Note whether you can follow conversation in a quiet room, in background noise, on the telephone, and when the speaker is on the treated side.
  4. Describe the tinnitus precisely. Record whether it is in one ear, both ears, or “in the head”; continuous or intermittent; tonal, buzzing, roaring, or pulsatile; and whether movement, exertion, or posture changes it.
  5. Measure the burden. Tell the clinician how tinnitus affects sleep, concentration, mood, work, and communication. A distress score or diary can help track change, but it does not diagnose tumor activity.
  6. Review associated symptoms and medicines. Mention dizziness, imbalance, facial sensation or movement changes, ear fullness, headaches, sound sensitivity, and any medication changes. Your clinician can decide whether additional vestibular, neurological, or medical assessment is needed.

Basic audiology can address tinnitus-related problems for many people. The 2024 VA/DoD guideline also provides referral pathways when hearing rehabilitation, behavioral-health care, or specialist intervention is needed.

Choose hearing rehabilitation according to residual function

The right device is determined by residual hearing and speech discrimination, not by the tinnitus label alone. Candidacy testing and fitting should be done by an audiologist and otologist who understand post-VS hearing loss.

Hearing situation Options to discuss What the option does Important limitations
Useful hearing remains in the treated ear Conventional hearing aid Improves access to speech and environmental sound; reducing the contrast between tinnitus and silence may make tinnitus less prominent. Benefit depends on speech discrimination, loudness tolerance, ear anatomy, and fitting. Amplification does not treat the tumor or guarantee tinnitus relief.
Little or no useful hearing in the treated ear; the other ear hears well CROS hearing aid Captures sound from the poorer side and sends it to the better ear, improving awareness of talkers on the deaf side. It does not restore hearing to the treated ear and may not provide normal localization or speech understanding in noise.
Hearing loss in both ears, with one side substantially poorer BiCROS hearing aid Routes sound from the poorer side to the better ear while also amplifying the better ear. Programming must balance audibility, comfort, and speech clarity; outcomes vary with the better ear’s hearing.
Conductive or mixed components, or preference to avoid an ear-level receiver Bone-conduction device or bone-conduction implant Transfers sound through bone to stimulate the functioning cochlea, depending on the anatomy and hearing of the receiving side. Implantable systems require surgery and specialist candidacy review; non-implant systems still require professional programming.
Severe hearing loss with an intact, functioning cochlear nerve and appropriate anatomy Cochlear-implant evaluation Provides electrical stimulation to the cochlea for selected people who obtain insufficient benefit from hearing aids. It is not suitable for everyone after VS treatment. Testing must establish cochlear-nerve integrity, anatomy, speech benefit potential, and medical suitability.
Bilateral profound loss or some NF2-related injuries Auditory brainstem implant evaluation Stimulates the auditory brainstem when cochlear-nerve or cochlear function is not adequate for a cochlear implant. This is a specialist option with different expectations and risks; it is not a routine tinnitus device.

When a conventional hearing aid makes sense

If the treated ear still provides useful speech information, a conventional hearing aid may improve communication and reduce the perceptual gap between tinnitus and a silent room. Ask the audiologist to verify speech audibility and comfort, not simply to increase volume. A trial period is useful because tinnitus loudness, sound sensitivity, and distortion can affect acceptance.

When to consider CROS or BiCROS

CROS is generally considered when one ear has little or no usable hearing and the opposite ear is sufficiently good to receive the routed signal. BiCROS adds amplification for the better ear when it also has hearing loss. These systems can reduce the “head shadow” effect—missing a talker on the poorer side—but they do not recreate binaural hearing. Discuss performance in restaurants, meetings, and cars, where routing sound to one ear may not solve speech-in-noise problems.

Bone-conduction options

Bone-conduction devices may be considered when routing through air-conduction hearing aids is impractical or when the hearing pathway on the receiving side is suitable. Implantable versions involve a procedure; non-implant options still need programming and verification. Ask how the device will interact with glasses, masks, skin sensitivity, and MRI requirements.

Cochlear-implant assessment

The VS guideline identifies cochlear implants for selected patients with preserved cochlear-nerve function. The VA/DoD guideline suggests implants for people who derive no benefit from hearing aids and meet candidacy criteria. An evaluation commonly considers aided speech testing, imaging, cochlear-nerve integrity, the condition of the cochlea, the other ear, previous radiation or surgery, and your communication goals. Do not assume that profound hearing loss automatically means an implant is possible, or that an implant will eliminate tinnitus.

Auditory brainstem implants

For bilateral profound loss or certain NF2-related injuries, a specialist center may discuss an auditory brainstem implant. The decision is substantially different from choosing a hearing aid: expected sound quality, rehabilitation, surgical factors, and safety must be reviewed by a multidisciplinary team.

Reduce tinnitus distress with education and behavioral care

Management starts with explanation and counseling. Understanding that tinnitus can persist despite tumor control helps prevent a common and distressing misinterpretation: the sound itself is not proof that treatment failed. Counseling should also cover hearing results, realistic device goals, sleep, concentration, and how to respond when tinnitus spikes.

Cognitive behavioral therapy

CBT is the best-supported behavioral treatment in general tinnitus guidelines for reducing tinnitus-related distress. It does not require the tinnitus signal to disappear. A trained clinician helps identify catastrophic interpretations, avoidance patterns, sleep-disrupting habits, and attention loops, then practices more useful responses. Delivery may be individual, group-based, or structured through a validated remote program, depending on local services and clinical suitability.

Evidence specifically in people treated for VS is thinner than the broader tinnitus evidence. The VS-focused review notes that CBT and mindfulness are commonly recommended in general tinnitus clinics, while direct VS-specific trials remain limited. That distinction matters: CBT is a reasonable, evidence-informed way to reduce suffering, but no clinician should promise a tumor-specific cure.

Mindfulness and other counseling approaches

Mindfulness-based approaches may help some people disengage from automatic monitoring of the sound and reduce emotional reactivity. Use them as part of a clinician-guided plan, particularly if anxiety, depression, trauma symptoms, or insomnia are prominent. If distress is severe, ask for behavioral-health assessment rather than trying to manage it alone.

Use sound enrichment safely

The 2024 VA/DoD guideline suggests sound-based enrichment with ongoing directed tinnitus education by an audiologist and repeated visits over time. Sound enrichment can be used alone or alongside CBT. Options include environmental sounds, music, radio, recorded audio, smartphone applications, wearable sound generators, and bedside or tabletop generators.

Setting up a bedside white-noise or sound generator

  1. Choose a device that can play steady noise, nature sounds, or music without abrupt volume changes. A bedside sound generator is an adjunct, not a tumor treatment or hearing-restoration device.
  2. Start below the tinnitus level or use partial masking. The VA/DoD approach is to set noise or music at the lowest level that provides relief, rather than turning it up until the tinnitus is completely covered.
  3. Place it near the bed or listening position, not directly against the ear. Keep the sound comfortable enough for conversation and sleep.
  4. Use it during the situations in which tinnitus is most intrusive, such as falling asleep, reading, or working in a quiet room. You do not need constant loud masking.
  5. Adjust one variable at a time—sound type, level, distance, or duration—and record whether sleep, concentration, and distress improve.
  6. Stop or lower the level if the sound causes pain, sound sensitivity, headache, dizziness, or a sustained increase in tinnitus. Discuss persistent worsening with your audiologist.

Wearable sound-masking devices are another category listed by the guideline. Because programming interacts with hearing loss, speech discrimination, and sound tolerance, have an audiologist select and adjust one rather than buying a generic device without assessment.

Build a practical care plan

A useful plan separates three goals that are often confused: monitoring the tumor, improving communication, and reducing tinnitus distress.

  1. Keep the surveillance plan. Attend MRI and specialist reviews on the schedule set for your treatment, tumor size, symptoms, and center’s protocol. Do not change imaging intervals because a sound improves or worsens.
  2. Obtain the audiology baseline. Keep copies of pure-tone and speech results so future changes can be compared.
  3. Set a communication target. Examples include hearing a partner from the treated side, following a small meeting, or using the telephone more reliably. The target determines whether conventional amplification, CROS/BiCROS, bone conduction, or implant evaluation is most relevant.
  4. Set a distress target. Choose a measurable outcome such as falling asleep without prolonged monitoring, concentrating for a work period, or reducing panic during a tinnitus spike.
  5. Begin education and counseling. Ask for CBT or another appropriately trained tinnitus clinician when tinnitus remains bothersome. Include sleep and mood treatment when needed.
  6. Add sound enrichment if useful. Use low-level, comfortable sound and review the response with an audiologist instead of escalating volume on your own.
  7. Review and adjust. Device settings, counseling goals, and sound choices may need several visits. Bring your symptom notes and examples of difficult listening situations.

Common problems and how to respond

Problem Likely next step
Pure-tone thresholds look acceptable but conversation remains unclear Request speech audiometry and discuss speech discrimination, listening-in-noise testing, and whether amplification is likely to help.
A hearing aid makes tinnitus or sound sensitivity worse Stop increasing volume; return for verification, gradual acclimatization, compression changes, or a different technology. Persistent pain or sudden hearing change needs clinical review.
CROS helps awareness but not restaurants or meetings Ask about directional microphones, remote microphones, communication positioning, and realistic limits of routing sound to one ear.
White noise is irritating or keeps you awake Lower the level, try a different sound spectrum or natural recording, move the speaker farther away, or discontinue it. Sound enrichment is optional.
Tinnitus spikes after a stressful night Use the coping plan from counseling, protect sleep, and avoid repeatedly checking the sound. Seek help if distress becomes unsafe or unmanageable.
Tinnitus changes with new neurological or balance symptoms Contact the treating team urgently; use emergency care for severe vertigo, cranial-nerve weakness, stroke-like symptoms, or suicidal thoughts.

Questions to take to your appointments

  • What are my current pure-tone thresholds and speech-discrimination scores in each ear?
  • Is the treated ear providing useful speech information, and what does that imply for a conventional aid, CROS, or BiCROS?
  • Is my cochlear nerve intact and am I a candidate for cochlear-implant evaluation?
  • Would bone-conduction technology be appropriate for my anatomy and hearing pattern?
  • What communication situations should we test before choosing a device?
  • Can you refer me to a clinician trained in CBT for tinnitus?
  • What sound level and type are safe for my hearing and sound tolerance?
  • Which symptoms should trigger an earlier visit or emergency assessment?
  • When is my next MRI, and whom should I contact if tinnitus changes before then?

What current evidence supports—and what it does not

The strongest practical support is for individualized audiology, hearing rehabilitation, education and counseling, CBT for tinnitus-related distress, and optional low-level sound enrichment. The VA/DoD guideline specifically recommends ongoing audiologist-directed sound education and suggests cochlear implants for people who receive no hearing-aid benefit and meet candidacy criteria.

Evidence boundaries are important. Direct trials of CBT or mindfulness in VS-specific populations are limited, and the available literature does not establish one dependable tinnitus-persistence rate by treatment type. Device outcomes also vary with speech discrimination, cochlear-nerve integrity, the opposite ear, anatomy, fitting quality, rehabilitation, and personal goals. A specialist assessment is therefore more informative than a single percentage or a product claim.
