A scanner alert is a lead, not proof of exploitability: context matters, and a human should review security-sensitive changes.
How to assess an AI code-scanner finding
Automated analysis can spot risky patterns, but it may not establish whether an attacker can reach the code, control the relevant value, or exploit the operation in its business context. OWASP notes that static application security testing (SAST) can have difficulty proving that a finding is a true vulnerability: OWASP Source Code Analysis Tools. Manual review complements scanning, particularly when the decision depends on application logic or context-specific security rules.
- Locate the reported code. Identify the precise source line, the value being flagged, and the operation that consumes it.
- Trace the data flow. Determine whether the value can come from an attacker or other untrusted source, whether it crosses a security boundary, and whether the path is reachable.
- Check the sensitive operation. Look for a database query, shell or other interpreter call, browser rendering or DOM operation, or filesystem access that could treat the value as instructions or escape its intended scope.
- Choose the fix for that destination. A generic “sanitize input” step is not a universal defense. The right control depends on whether the destination is SQL, a browser context, a shell, or a filesystem path.
Treat the scanner’s severity as an initial prioritization signal rather than evidence that the finding is exploitable. If the path is not reachable or the value is not attacker-controlled, document the reasoning and have a reviewer confirm it rather than suppressing an alert based on its wording alone.
Fix common vulnerability patterns

Why fixes must match the data destination
SQL: keep values out of query structure
When code combines query text and untrusted values through string concatenation, the database may interpret data as part of the SQL instructions. Use the parameterization facility provided by the application’s database driver or framework so query structure and values remain separate. Also restrict the database account to the operations and data the application actually needs; least privilege limits the potential impact if a flaw remains.

Browser output: handle the context where content appears
Examine where the value is rendered and how browser-side code manipulates it. HTML text, attributes, scripts, and DOM operations do not all share one safe handling rule. Use output encoding appropriate to the context and review DOM manipulation paths; filtering input once does not make it safe for every later use.
Shells and interpreters: do not turn data into instructions
Trace untrusted values into every relevant interpreter call, not just functions whose names contain “SQL” or “shell.” Prefer an API that accepts data separately from executable instructions. When a shell is necessary, use the language or platform’s safe argument-handling facilities and verify their behavior in the applicable documentation.

Filesystem paths: keep access inside the intended location
Inspect how untrusted components are combined into a path and whether resolution could escape the directory the application is meant to serve. Constrain the resolved location to the intended base and confirm that the platform’s path and filesystem APIs enforce the expected boundary.
Validate the change before merging
- Add or update tests. Exercise ordinary expected values as well as adversarial boundary cases relevant to the flaw. The exact test cases depend on the application and the affected API; there is no single universal suite.
- Rerun the relevant scanner. Check whether the reported path is addressed, but do not treat a clean scan as proof that unrelated business-logic vulnerabilities are absent.
- Inspect the diff. Confirm the change protects the actual sink without creating a new unsafe path or weakening another control.
- Request human security review. Keep a qualified reviewer involved for changes that affect trust boundaries, permissions, authentication, or other security-sensitive behavior.
Review risks introduced by AI-assisted changes
A proposed fix can address one alert while adding new risk elsewhere. Review changes to dependencies, secrets exposure, and configuration as part of the same code review.

- Secrets: check that credentials and other secrets are not exposed in prompts or coding-tool context.
- Persistent rules and configuration: inspect edits to agent instructions, build scripts, and deployment settings; these can affect future code generation or production behavior.
