If an achievement or trophy did not appear, first verify the game’s exact criteria, then refresh the account connection: sign out of Xbox, PlayStation Network, Steam, or the relevant PC launcher, reboot the device, sign back in, synchronize the list when that option exists, and repeat the requirement. A missing pop-up can be a delayed notification rather than a failed unlock.
If the unlock still does not register after a clean retry, the likely causes are a game bug, a platform-list synchronization problem, a cross-save mismatch, or a condition the game did not count. Publisher and platform support are the next step; support teams may be unable to grant an achievement manually.
Why an achievement or trophy can be missing
Unlock systems have two separate parts: the game detects that its conditions were met, and the platform records and displays the result. Either part can fail or be delayed.

- The criteria were narrower than expected. Some objectives require a particular order, difficulty, game mode, character, location, or uninterrupted sequence. A similar-looking action may not satisfy the internal condition.
- The notification was delayed. Network delivery can postpone the pop-up even after the game has recorded the unlock.
- The platform list has not synchronized. The achievement may be stored locally or on a service but absent from the visible profile until a refresh.
- Connectivity or account state changed. A temporary service outage, an incomplete sign-in, or a switch between profiles can interrupt reporting.
- Cross-platform progression did not transfer. Cross-save and cross-platform games may keep progression separate from trophy or achievement flags.
- A game defect blocked the trigger. Updates can introduce or fix achievement bugs, and some titles have objectives that fail to fire under particular circumstances.
- Achievements were disabled. In games that support console commands, using a command can disable achievements for that session or save. Bethesda says rebooting the game typically re-enables them in supported titles.
There is no dependable industry-wide statistic for how often these failures occur. Treat the affected game’s current support guidance and update history as more authoritative than assumptions based on another title.

Do these checks before repeating the objective
Read the exact in-game description
Open the achievement or trophy list in the game and compare every word with what you did. Check whether the condition specifies a difficulty, mode, mission order, number of items, time limit, no-death run, online status, or a particular profile. Hidden sequence requirements are especially easy to miss.
Confirm the correct account and platform
Make sure the game is running under the Xbox, PlayStation Network, Steam, or other platform account that owns the list. Guest profiles and secondary console users can complete the action without adding it to your account. In a cross-platform title, confirm which platform is responsible for the achievement or trophy rather than assuming that cross-save also transfers unlocks.
Allow time for a delayed notification
Stay connected to the service and wait briefly. A late pop-up does not prove that the requirement failed. Check the platform list from its normal achievement or trophy screen after the delay.

The standard recovery sequence
Use this order on any platform unless the game’s support page gives a more specific procedure.
- Close the game completely. Do not leave it suspended in a quick-resume or rest-mode state.
- Sign out of the relevant account. Sign out fully from Xbox, PlayStation Network, Steam, or the launcher carrying the game.
- Reboot the console or PC. A full restart clears a stuck service session more reliably than returning to the dashboard.
- Sign back in and launch the game. Use the same profile that performed the original action.
- Synchronize the list. On platforms that provide a manual sync command, run it before retrying. On others, opening the list while online allows the service to refresh.
- Repeat every requirement. Bethesda’s guidance says that if you are positive everything was completed and refreshing did not resolve the issue, you must do the steps again and re-attempt the achievement.
- Check the list again. Look for the unlock on the platform profile, not only for an on-screen toast.
Keep a save from before the retry when possible. It gives you a way to repeat the condition without losing unrelated progress.
Xbox: sign out, reboot, and retry
Procedure
- Quit the game from the Xbox guide so it is no longer running.
- Sign out of the Xbox profile that should receive the achievement.
- Restart the console.
- Sign back in to the same profile and launch the game while connected to Xbox services.
- Review the achievement criteria, then perform the entire requirement again.
- Open the Xbox achievement list and allow time for the result to appear.
Bethesda specifically recommends verifying every requirement, accounting for network delay, signing out, rebooting, signing back in, and retrying. It also notes that console commands can disable achievements in supported games; rebooting the game typically re-enables them. If a command was used, repeat the objective only after a clean restart and in a state where achievements are enabled.

When the Xbox retry still fails
Do not keep repeating a long objective indefinitely. Record the game version, the exact achievement text, the save or mission where it failed, whether commands or mods were active, and the time of the attempt. Check the publisher’s current support page for a known bug or required workaround, then contact the publisher and Xbox support if the issue persists.
PlayStation: sync with PlayStation Network
Procedure
- Exit the game and sign out of PlayStation Network.
- Restart the PlayStation console.
- Sign back in to the same PSN account.
- From the trophy area, select Sync with PlayStation Network.
- If servers are busy, wait and try the synchronization again later.
- Launch the game and repeat the complete trophy requirement.
- Return to the trophy list and check the result after synchronization.
Dovetail’s support guidance recommends checking the criteria, signing out, rebooting, using “Sync with PlayStation Network,” waiting during server congestion, and repeating the criteria. The sync operation updates the account list; it does not repair a requirement that the game never detected, so the retry remains necessary.
Diagnostic tests
If the trophy is still missing, Dovetail suggests two diagnostic alternatives: attempt the requirement offline, or try it with a new player profile. These tests can reveal whether the problem is tied to the online session or to the original profile. They are diagnostic only; a trophy earned on another profile will not normally move to your main account.

Steam, PC, and cross-platform games
Steam or launcher account refresh
- Exit the game and the Steam or other PC launcher completely, including its system-tray process.
- Sign out of the account connected to the game.
- Restart the PC.
- Sign back in, confirm the correct account and online status, and launch the game.
- Repeat the exact achievement conditions, then inspect the platform list.
Bethesda’s cross-platform guidance applies the same full sign-out, reboot, sign-in, and retry sequence to Xbox, Steam, and PlayStation. If the result remains absent, contact the platform support team as well as the game publisher.
Cross-save is not automatically cross-achievement
A cloud save can transfer character or mission progress while the achievement service keeps separate flags for each platform. Rockstar identifies network or synchronization delays as possible causes of a trophy not appearing in a cross-platform context. Verify where the objective was completed, which account owns the platform list, and whether the game documents separate achievement handling before attempting it again.

- Game title, edition, platform, and current game version.
- The exact achievement or trophy name and its complete description.
- Your platform account name or ID, following the support channel’s privacy instructions.
- When and where you completed the requirement, including mode, difficulty, mission, and sequence.
- Whether the device was online, whether the notification appeared, and whether the list later updated.
- Any commands, mods, offline play, profile changes, cross-save transfers, or recent patches involved.
- What happened after signing out, rebooting, signing back in, synchronizing, and repeating the objective.
Contact both sides when appropriate: the publisher owns the game’s trigger logic, while Microsoft, Sony, Valve, or another platform operator owns the account service and list synchronization. Bethesda explicitly warns that its customer support does not have the ability to unlock achievements manually, so a support ticket may produce a fix or confirmation rather than a granted trophy.
Preventing another missed unlock
- Read the complete description before starting a multi-hour attempt.
- Stay signed in to the intended account and avoid switching profiles mid-session.
- Keep the console or PC connected when the game requires online reporting.
- Avoid console commands or other features that disable achievements until the objective is complete.
- Keep a backup save before a one-time sequence or missable mission.
- After a major game update, check the publisher’s current support notes because behavior can change.
When to stop troubleshooting
After one careful refresh-and-retry cycle, further attempts are unlikely to help if the same reproducible condition fails. Preserve your evidence, check for a current game-specific bug report, and escalate to the publisher and platform support channels. Platform behavior and game bugs can change after updates, so avoid treating an old workaround as permanent.
