Choose a cognitive training app for a modest, clear purpose—such as enjoyable, structured practice—and assess its evidence, ease of use on the older adult’s own device, cost, privacy, and likelihood of continued use. Some structured training can improve skills it directly practices, but that does not show that consumer apps prevent dementia or deliver the results of clinical trials.

What cognitive training can—and cannot—tell you

Cognitive training is structured practice aimed at skills such as memory, reasoning, or processing speed. It is not simply any puzzle or game promoted as “brain training.” The National Academies’ review found the clearest evidence for improvement on practiced or closely related tasks; transfer to other abilities and everyday independence is less certain. Why the ACTIVE study does not validate every app

The ACTIVE study enrolled 2,802 older adults in a particular structured intervention. The National Academies reported moderate-strength evidence of improvement in the trained domain at two years, but low-strength evidence at five and ten years; transfer to other domains was rare. The report cautioned: “At present, there is no evidence to support the notion that the beneficial long-term cognitive effects suggested by the ACTIVE trial apply to computer-based ‘brain training’ applications.”

The National Institute on Aging likewise says there is not enough evidence that commercially available computer-based apps have the same impact on cognitive abilities as ACTIVE. Most computer-based app trials identified by the National Academies review were excluded from its systematic review because follow-up was too short to assess long-term outcomes. NIA guidance

How to interpret the 2026 dementia finding

In a 2026 release about a study following adults aged 65 and older for 20 years, NIH reported a 25% lower rate of dementia diagnosis, measured using Medicare claims data, among people assigned to a specific rapid object-detection speed-training regimen. The other two training types were not associated with that finding. This result concerns one regimen and an association in a particular study; it does not establish that commercial apps generally prevent dementia or that a particular app reproduces the regimen. Decide what the app is for

Start by distinguishing enjoyable practice from a clinical need. A general consumer app may provide structured exercises, but it is not a diagnostic tool. If memory or thinking changes are new, worsening, or concerning, discuss them with a healthcare professional rather than relying on an app score.

For a specific goal, look for what the exercises actually train and what outcomes have been studied. Evidence for performance on a practiced task is not the same as evidence for improvement in unrelated abilities, daily function, or disease risk.

Compare apps on the factors that matter

What to check Questions to ask
Evidence Does the provider identify the exact program and publish peer-reviewed studies involving a relevant older-adult population? Were outcomes measured on trained tasks, other skills, everyday function, or disease diagnosis?
Goal and fit Do the exercises match the person’s aim—such as structured practice—or is the app being mistaken for rehabilitation or memory evaluation?
Usability and access On the person’s device, are text, contrast, audio, captions, instructions, language, tap targets, and dexterity demands manageable? Does it work with their connectivity needs, and can someone help set it up?
Engagement Are the exercises clear and enjoyable enough that the person wants to return? Do not assume a universal daily duration is necessary.
Cost and access What does it cost after a trial, how often does it renew, and how do cancellation and account deletion work? Check whether a local library or health plan provides access before subscribing.
Privacy What account, performance, or health-related information is collected and shared? What does the current privacy policy say about deleting the account?
Claims Does the provider promise to prevent Alzheimer’s disease, reverse aging, or diagnose impairment? Treat such claims skeptically.

These are practical comparison questions, not a validated scoring system. Vendor summaries can point to studies or features, but they are not independent proof. The National Academies notes that commercial apps generally involve a purchase or subscription cost.

Test accessibility on the person’s own device

Before paying, try the app on the phone or tablet the older adult will actually use. Check whether they can read the screen, hear or follow instructions, make selections comfortably, and understand how to move between exercises. App-store accessibility declarations can be incomplete or change over time, so a hands-on check is more useful than assuming a product will fit.

For example, the BrainHQ App Store listing captured in the cited source says the developer had not yet indicated which accessibility features the app supports. That is a prompt to test it directly, not proof that the app is inaccessible. Apple App Store listing

Use product descriptions as starting points, not rankings

BrainHQ

Posit Science describes BrainHQ as offering exercises for memory, attention, processing speed, social cognition, decision-making, and navigation, with adaptive levels, progress tracking, and short exercise levels. The company also says some libraries and some Medicare Advantage plans offer access; availability must be confirmed with the relevant library or plan. These are vendor descriptions, not independent evidence that BrainHQ is the best app or that its results match ACTIVE. The National Academies cautions that ACTIVE’s complex, group-based program cannot establish equivalent long-term benefits for commercial apps. BrainHQ National Academies review

CogniFit

CogniFit describes training and assessment tools and an offering for healthy aging. This identifies what a reader can inspect, not evidence that it is superior to other consumer apps. CogniFit healthy-aging page

Keep consumer training separate from clinical screening

The FDA’s classification entry concerns a computerized cognitive assessment aid used by clinicians as a screening aid for adults 55 and older, with referral for further testing where warranted.
