Build agent governance as an accountable operating system, not a one-time model-approval checklist. Register every agent and dependency, name a business owner and technical owner, classify the actions it can take, enforce a distinct identity with least-privilege authorization, require informed human approval for high-impact actions, preserve tamper-evident evidence, and continuously test and monitor the system.
Agentic systems can plan, call tools, delegate to other agents, alter records, send messages, execute code, and trigger effects outside the model itself. The framework below combines the NIST AI Risk Management Framework (AI RMF) lifecycle, ISO/IEC 38507 governance responsibilities, ISO/IEC 42001 management-system practices, OWASP agent-security guidance, and the EU AI Act where its legal scope applies.
Why agentic AI needs a different governance model
A conventional model review often asks whether a model is accurate, fair, secure, and suitable for a proposed use. An agent adds an operational chain: it interprets an objective, chooses a plan, retrieves information, calls tools, uses credentials, writes to systems, delegates work, and may continue until a stopping condition is reached. Accountability therefore follows the complete action path, not only the model that generated a response.
Recommended Free Tools
For each material action, an organization should be able to answer five questions:
#1 Best Overall

- Handy Design & Portable:Cordless air duster electric nozzle is Light weight and small size, design and comfortable to hold and space saving, convenient to carry around. You can put it in your room or inside the car without taking up space.
- Economical &Reusable: Built durable materials with a long-lasting battery, our CAGIWIRU air duster it's a sustainable alternative to disposable air cans.
- Which business process and outcome does the action support?
- Which named owner accepted the residual risk?
- Which identity and policy decision authorized the specific tool call?
- What information did a human approver see, and what limits applied?
- What evidence allows reconstruction, containment, and remediation afterward?
The NIST AI RMF 1.0, published in 2023, supplies a voluntary lifecycle structure with four functions: Govern, Map, Measure, and Manage. Its Playbook provides suggested actions and references for those outcomes. The framework is not a certification scheme; NIST’s FAQ explicitly describes the AI RMF as voluntary. Agentic controls must be added to that lifecycle rather than treated as a separate compliance exercise.
Set accountability before deployment
Use a separation-of-duties model. The team that delivers an agent should not be able to silently approve its own risk, issue its own unrestricted credentials, and declare its own controls effective.
Recommended ownership model
| Role | Accountability | Decisions and evidence |
|---|---|---|
| Board or executive sponsor | Sets risk appetite and authorizes organizational use of agentic AI. | Approves prohibited uses, material exceptions, funding, and receives incident and control-effectiveness reports. |
| AI governance committee | Runs the cross-functional governance process. | Owns policy, risk taxonomy, intake criteria, escalation thresholds, and decisions that cross business, security, privacy, legal, and technology boundaries. |
| Business or process owner | Owns the business outcome and user impact. | Documents purpose, data use, affected people, residual risk, service levels, and acceptable failure modes. |
| Agent or product owner | Owns the implementation. | Maintains instructions, model and tool configuration, evaluations, release records, and change control. |
| Security and identity owner | Controls machine identity and privilege. | Issues credentials, enforces least privilege, operates authorization policy, manages secrets and isolation, and leads security response. |
| Privacy and legal/compliance | Determines obligations that apply to the use case. | Assesses privacy, sector, consumer-protection, employment, contractual, and AI-law requirements. |
| Platform or SRE owner | Runs the production environment safely. | Owns availability, observability, logging, rollback, kill-switch operation, dependency hygiene, and recovery objectives. |
| Human approver or operator | Approves defined high-impact actions and can pause or stop the agent. | Reviews the actual proposed action, target, data, limits, and consequence before approval. |
| Independent assurance | Challenges the control design and risk acceptance. | Tests controls, reviews evidence, validates remediation, and reports weaknesses independently. |
ISO/IEC 38507:2022, Edition 1 (April 2022), is aimed at governing bodies and executive managers. That emphasis is important: technical teams can implement controls, but leadership remains accountable for acceptable purposes, resources, oversight, and risk decisions. NIST likewise describes risk management as involving actors across the AI lifecycle.
Inventory every agent and its operating boundary
Create a register that is continuously maintained, not a spreadsheet completed once at project intake. An entry should identify the deployed version and the full chain of components that can influence an action.
Minimum inventory fields
- Agent name, unique identifier, version, environment, deployment date, and planned retirement date.
- Business owner, agent or product owner, security and identity owner, platform owner, and escalation contacts.
- Model family and provider, model version, system instructions, policy prompts, retrieval configuration, and evaluation set.
- Every tool, API, database, file store, code-execution function, browser capability, MCP server, and inter-agent endpoint.
- Data classes the agent can read, transform, retain, or transmit, including personal, confidential, regulated, and customer data.
- Delegated identities, credential scopes, token lifetime, environment restrictions, and the central authorization policy used for each action.
- Geographic scope, users, affected populations, downstream decisions, external recipients, and regulatory classifications under consideration.
- Whether it can read, write, transact, communicate externally, execute code, change configuration, create credentials, or delegate work.
- Dependencies, vendors, open-source packages, prompt and memory stores, logging destinations, and rollback artifacts.
Define the boundary in plain language
Write a short statement that says what the agent may do, for whom, with which data, in which environments, and what it must never do. For example: “This agent may draft internal support responses from the approved knowledge base. It may not send a message, change a customer record, access payment data, create a credential, or delegate to another agent without a separate approved workflow.” The boundary becomes the basis for tests, authorization rules, monitoring alerts, and human-approval prompts.

Classify risk by impact and autonomy
Do not classify an agent solely by the model name or whether a use case is called “assistive.” A drafting agent with send access can create an external effect; a narrow workflow agent can still expose sensitive records. Score the use case and its actions against impact, data sensitivity, privilege, autonomy, reversibility, scale, user vulnerability, regulatory exposure, and external effect.
Example risk tiers and required controls
| Tier | Typical capability | Baseline requirements |
|---|---|---|
| Informational | Answers questions or summarizes approved material without changing records or contacting outsiders. | Named owner, approved data sources, prompt-injection testing, output monitoring, user-visible uncertainty, and ordinary access controls. |
| Internal workflow | Reads internal systems and prepares drafts, tickets, or recommendations for an employee. | Distinct agent identity, read-only scoped credentials, source and output logging, data-loss controls, representative evaluation, and operator review before commitment. |
| Customer-impacting | Communicates with customers, changes service records, or makes recommendations that affect access, price, eligibility, or support outcomes. | Purpose-bound authorization, approved data minimization, per-action policy checks, human escalation for defined outcomes, quality and fairness testing, complaint handling, and rollback. |
| Regulated or high-impact | Supports employment, credit, insurance, health, public-service, safety, or other legally sensitive decisions. | Legal and privacy assessment, documented role and classification, stronger validation, human decision authority, explainable evidence, heightened monitoring, retention controls, and documented incident and appeal paths. |
| Irreversible or critical | Moves money, changes production infrastructure, executes code in a sensitive environment, publishes externally, changes privileges, or triggers physical or safety-relevant effects. | Default deny, isolated runtime, short-lived credentials, dual control or explicit human approval, transaction and spend limits, tested kill switch, rollback or compensating action, immutable evidence, and emergency response. |
NIST’s Playbook is intended to help organizations tailor actions to risk and lifecycle context. Use the tiers as an internal decision tool, then map each tier to concrete approval, testing, monitoring, and intervention requirements. Do not assume that every agent is a “high-risk AI system” under the EU AI Act; classification depends on the facts, the role of the organization, the system’s intended purpose, and the Act’s applicable provisions.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Give every agent a real identity and narrowly scoped authority
Identity is the foundation of accountability. A shared human account or a long-lived administrator token makes it impossible to distinguish an agent’s actions, enforce purpose boundaries, or revoke one deployment without disrupting unrelated work.
NIST’s National Cybersecurity Center of Excellence agentic-AI identity project warns that weak identity, authorization, and governance can lead to data leaks, compliance failures, prompt injection, and unpredictable autonomous behavior. OWASP’s agent guidance similarly treats identity and privilege abuse as core threats.
Authorization design
- Issue a distinct machine identity. Bind it to the registered agent, version, environment, and owner. Do not reuse a person’s account.
- Use short-lived credentials. Obtain tokens just before an approved operation, set an expiration, and revoke them when the run ends or a risk signal appears.
- Separate read and write permissions. A tool that retrieves a record should not automatically be able to edit or delete it. Separate production from test and development environments.
- Bind permission to purpose. Include the approved workflow, data class, tenant, geographic scope, transaction ceiling, and time window in the authorization decision.
- Re-authorize each privileged action. Route tool calls through a centralized policy decision point. Never treat an earlier user login or an agent’s natural-language explanation as continuing authorization.
- Constrain delegation. A child agent must receive an explicit, narrower scope and a traceable parent identity. It must not inherit unrestricted parent privileges.
- Log the decision. Record agent identity, user or initiating service, requested action, parameters, target, data classification, policy result, credential used, approver, and outcome.
Permission review checklist
- Can the agent access only the records required for the approved purpose?
- Can retrieved content cause a tool call to bypass policy?
- Can the agent create, modify, or export credentials?
- Can it call an unapproved domain, MCP server, plugin, or delegated agent?
- Are denied requests and repeated retries visible to security operations?
- Can a single policy change expand privileges without independent review?
Make human oversight specific and meaningful
“Human in the loop” is not a control unless the person has enough information, authority, time, and independence to stop the action. Approval should expose the actual tool call and consequence, not only a polished summary generated by the same agent.

Define approval triggers
Require approval before actions such as sending an external communication, releasing sensitive data, moving money, changing a production system, escalating privileges, creating a credential, making a regulated or customer-impacting decision, or taking an irreversible action. Set numeric limits for transaction value, record count, data volume, and geographic scope. Use dual control where one person should not both prepare and approve the action.
Design the approval surface
The approval screen should show the initiating user, agent identity and version, exact tool and parameters, target system or recipient, records or data categories involved, policy checks, predicted consequence, reversibility, and the reason for escalation. Let the approver edit or reject the action, request more evidence, and see relevant prior attempts. Expired or unanswered requests should fail closed rather than execute automatically.
Provide emergency authority
Name operators who can pause a run, disable a tool, revoke credentials, isolate the environment, or place the agent in read-only mode. Test the kill path under realistic conditions. Approval does not prevent manipulation by itself; it must be combined with authorization, logging, monitoring, and a trustworthy display of the real action.
Contain the runtime and treat inputs as hostile
Agentic behavior is shaped by more than the base model. Prompts, retrieved documents, memory, tool descriptions, MCP servers, dependencies, user messages, and delegated-agent outputs can all carry instructions or data that alter behavior. Treat each as untrusted until its provenance, integrity, and permitted use are established.
Runtime controls
- Run code and file operations in a sandbox with a minimal filesystem and no unnecessary host access.
- Restrict network egress to an allowlist of destinations and methods; block arbitrary domains by default.
- Allow only registered tools and validate arguments against strict schemas before execution.
- Enforce rate, concurrency, recursion, token, time, and spend limits. Stop loops and repeated retries.
- Separate environments and data stores; prevent test credentials from reaching production.
- Apply content and data-loss controls to outbound messages, exports, and tool parameters.
- Use circuit breakers for abnormal error rates, privilege changes, data access, or spend.
- Maintain a tested rollback or compensating-action path for every write operation.
OWASP’s agent-specific threat categories include goal hijacking, tool misuse, supply-chain vulnerabilities, unexpected code execution, memory and context poisoning, insecure inter-agent communication, MCP authentication failures, and cascading failures. These are security threats and mitigation guidance, not legal requirements, but they are useful prompts for threat modeling and engineering tests.

Evaluate before release and monitor continuously
Release approval should be based on evidence from representative and adversarial scenarios, not a demonstration that succeeds on a few friendly prompts.
Pre-release evaluation set
- Normal tasks across supported users, languages, data classes, and failure conditions.
- Direct and indirect prompt injection through user text, retrieved documents, tool output, memory, and web content.
- Attempts to access records outside the approved tenant, purpose, geography, or role.
- Tool misuse, malformed parameters, unsafe code execution, secret exposure, and data exfiltration.
- Unauthorized delegation, privilege escalation, recursive loops, and conflicting instructions.
- Human-approval usability: whether the reviewer can identify the real action and reject it in time.
- Failure recovery, rollback, timeout, duplicate requests, partial completion, and service outages.
- Changes in model, prompt, policy, dependency, tool, MCP server, credential scope, or data source.
Document test cases, expected outcomes, observed results, severity thresholds, unresolved risks, and the person who accepted each exception. Re-run relevant tests after material changes.
Production telemetry
Monitor tool calls, denied requests, privilege changes, abnormal data access, model and prompt versions, delegation depth, latency, token and spend consumption, error and retry rates, approval outcomes, user complaints, incidents, and near misses. Alert on patterns rather than a single metric: for example, a sudden increase in denied exports followed by repeated alternate-tool attempts.
Preserve tamper-evident records sufficient to reconstruct the run: input and instruction versions, retrieved sources or their hashes, tool requests and responses, policy decisions, approvals, credentials or identity references, outputs, timestamps, and system configuration. Apply retention and access controls appropriate to the data in those logs.
Govern the supply chain and every change
The attack surface includes model providers, prompts, retrieval indexes, memory stores, tools, MCP servers, packages, infrastructure, vendors, and delegated agents. Maintain provenance and an owner for each component.

Changes that require review
- Model or provider changes, including fallback models.
- System instructions, policy prompts, routing logic, memory format, or retrieval-source changes.
- New tools, APIs, MCP servers, plugins, browser capabilities, or inter-agent endpoints.
- Dependency, container, runtime, or infrastructure updates.
- Credential-scope, policy, network-egress, rate-limit, or transaction-limit changes.
- New data sources, retention periods, geographic locations, users, or downstream decisions.
For each release, retain the versioned artifacts, security and privacy review, evaluation results, approval record, deployment time, rollback package, and named owner. OWASP’s MCP guidance highlights token exposure and insufficient authentication or authorization in multi-agent ecosystems; do not connect an MCP server merely because its tool description appears useful.
Prepare for incidents and learn from near misses
Write an agent-specific response plan before production. It should cover:
- Detection and triage: define severity levels for unauthorized access, harmful external effects, privacy exposure, service disruption, and policy bypass.
- Containment: pause the run, disable the affected tool or endpoint, isolate the environment, revoke credentials, and block egress as appropriate.
- Evidence preservation: protect logs, prompts, retrieved content, tool responses, policy versions, approvals, and deployment artifacts from alteration.
- Notification: identify internal owners, affected users, customers, vendors, regulators, and law-enforcement contacts that may need assessment under applicable obligations.
- Remediation: correct the policy, credential, prompt, dependency, data source, or runtime weakness; restore from a known-good version or perform a compensating action.
- Return to service: require evidence that the cause is understood, controls are retested, owners approve residual risk, and monitoring is heightened.
- Learning: update the risk score, evaluation scenarios, controls, training, and inventory. Track near misses, not only confirmed harm.
NIST describes AI risk management as continuous and responsive to changing knowledge and context. An incident process that closes a ticket without changing tests, policy, or ownership is not a learning loop.

A practical implementation sequence
Organizations usually get better results by establishing a minimum control system, then increasing rigor according to risk.
- Set the mandate. Name the executive sponsor, approve risk appetite and prohibited uses, and charter the governance committee.
- Build the register. Inventory production and pilot agents, their capabilities, identities, tools, data, dependencies, owners, and retirement dates.
- Classify use cases and actions. Score impact, sensitivity, privilege, autonomy, reversibility, scale, vulnerability, regulatory exposure, and external effect.
- Close identity gaps. Replace shared accounts, issue distinct short-lived credentials, separate read and write access, and centralize policy decisions.
- Define approval and stop conditions. Publish action thresholds, approver evidence, timeout behavior, dual-control rules, kill-switch authority, and rollback paths.
- Contain the runtime. Apply sandboxing, egress allowlists, tool schemas, rate and spend limits, delegation limits, circuit breakers, and environment separation.
- Test adversarially. Run prompt-injection, authorization, data-exfiltration, tool-misuse, unsafe-delegation, and recovery scenarios before release.
- Instrument production. Collect policy decisions, tool calls, approvals, denials, anomalies, spend, complaints, and tamper-evident evidence.
- Integrate change control. Require review and rollback artifacts for model, prompt, tool, MCP, dependency, data, policy, and credential changes.
- Exercise response. Simulate credential revocation, tool disablement, evidence preservation, notification, remediation, and return to service.
- Review effectiveness. Report control results and exceptions to leadership, reassess risk after incidents or material changes, and retire agents that no longer have a justified purpose.
What good governance looks like in practice
Before an agent can update a production record, a registered identity requests a specific operation through the policy decision point. The policy verifies the approved workflow, target tenant, data class, environment, transaction limit, credential lifetime, and current risk signals. If the operation crosses a defined threshold, the approval interface displays the exact record, fields, parameters, and consequence to an authorized human. The system records the policy result and approval, executes only the permitted call, and keeps a rollback or compensating action available.
If retrieved content tells the agent to ignore its instructions, call an unapproved endpoint, or disclose secrets, the content is treated as untrusted data. The tool allowlist, schema validation, egress restriction, and authorization policy prevent the instruction from becoming an action. The event is logged and may trigger an alert. This is the difference between a governance statement and an enforceable control.
