A production agent should not change its behavior every time a user submits feedback. Instead, treat continual learning as a controlled software-and-data lifecycle: capture representative traces, turn verified failures into versioned evaluation and training assets, test a candidate against the incumbent, and release it through progressively safer stages with a tested rollback path.
This design uses two connected loops. The outer loop learns from production evidence; the inner release loop validates and promotes changes. Prompts, tool policies, retrieval indexes, memory schemas, graders, orchestration code, and model weights all belong under the same version and approval discipline.
The two-loop architecture
Separate learning from deployment so that production feedback can propose a change without changing the live agent immediately.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
Outer learning loop: production evidence to candidate assets
- Production: run the incumbent agent under controlled tool permissions and record the context needed to understand the result.
- Feedback and telemetry: collect user ratings, reviewer judgments, corrections, tool errors, latency, cost, safety events, and service-level indicators.
- Behavior analysis: sample traces, group failures, inspect retrieved evidence and tool decisions, and identify recurring patterns.
- Classification: label each case by failure mode and severity, such as factual error, retrieval miss, wrong tool, policy violation, unsafe action, formatting defect, latency regression, or cost regression.
- Asset updates: create a proposed prompt, policy, retrieval, memory, grader, dataset, or model change. Every proposal receives a version, owner, rationale, and rollback target.
Microsoft describes this sequence as production, feedback and telemetry, behavior analysis, classification, and asset updates. The important boundary is that the outer loop produces candidates; it does not write directly to the serving configuration.

Inner release loop: candidate to approved deployment
- Check in code, prompts, tool policies, retrieval configuration, memory rules, graders, datasets, and model references.
- Run unit, integration, contract, safety, regression, latency, and cost tests in continuous integration.
- Evaluate the candidate on fixed holdouts and replayed production cases.
- Compare it with the current incumbent across overall and segment-level metrics.
- Obtain the required technical, safety, and domain approvals.
- Deploy to shadow traffic or a canary, observe it against release gates, and increase traffic gradually.
- Record the final decision, evidence, approver, and rollback pointer in an immutable changelog.
Google’s MLOps guidance maps to this loop through source-controlled development, CI tests and builds, pipeline deployment, automated triggers, continuous delivery, monitoring, metadata, and rollback. For agents, the same stages must cover non-weight assets as well as model artifacts.
Version every behavior-changing asset
An agent’s behavior is the product of more than its model. A reproducible release identifies all of the following:
- Model name, provider or checkpoint, inference parameters, and safety settings.
- System and task prompts, routing rules, templates, and output schemas.
- Tool definitions, argument validation, authorization policy, sandbox limits, and API versions.
- Retrieval sources, chunking and ranking configuration, index build identifier, freshness policy, and citation rules.
- Memory schema, write and read policies, expiration rules, summarization prompts, and user-consent settings.
- Graders, rubrics, evaluator model versions, calibration examples, and score aggregation code.
- Training, replay, safety, and protected holdout datasets, including hashes and lineage.
- Orchestration code, feature flags, infrastructure configuration, and dependency lockfiles.
Use a release manifest that points to immutable versions rather than mutable names. A minimal manifest can look like this:
release_id: agent-2026-10-20.0042
model: model-artifact@sha256:...
prompt_bundle: prompts@3.7.1
tool_policy: tools@2.4.0
retrieval_index: index@2026-10-19T22:10Z
memory_policy: memory@1.9.2
eval_suite: evals@8.1.0
graders: graders@5.3.0
rollback_to: agent-2026-10-18.0039
Store the manifest with evaluation results and deployment events. If a failure appears later, operators should be able to reconstruct the exact behavior without guessing which prompt or index was active.
Capture traces that can be safely replayed
For each run, retain enough context to reproduce the decision while enforcing privacy, access, and retention controls. A useful event record includes:
- Task input, conversation state, user or account scope, and consent status.
- Model, prompt, policy, orchestration, and memory versions.
- Every tool call, validated argument, response, authorization decision, timeout, and retry.
- Retrieved passages, source identifiers, ranking scores where available, and the context supplied to the model.
- Intermediate and final outputs, structured-schema validation results, latency, token usage, infrastructure cost, and failure codes.
- User feedback, reviewer labels, intervention events, and links to incident records.
Privacy and retention controls
Redact or mask secrets, authentication material, payment data, and unnecessary personal information before analyst or subject-matter review. Restrict raw transcripts to authorized roles, set a retention period by data class, and log access. Keep an immutable or append-only audit record for traces used in safety investigations, training, evaluation, or internal deployment. That record should preserve the original event hash and the transformations applied to any derived dataset.
Build learning cases instead of training on raw logs
Do not stream every trace into a fine-tuning job or preference optimizer. Curate each candidate case by:

- Deduplicating near-identical conversations and repeated alerts.
- Removing secrets, personal data, and irrelevant context.
- Assigning a failure mode, severity, affected segment, and confidence.
- Writing the expected behavior or an acceptable answer range.
- Recording the evidence a reviewer used to make that judgment.
- Separating the case into an appropriate training, replay, or evaluation set.
Keep protected safety and regression holdouts inaccessible to the adaptation job. Otherwise, a rising score can reflect memorization rather than a safer or more useful agent.
Choose the smallest adaptation that fixes the problem
Start with the mechanism that addresses the failure while minimizing irreversible change. The trade-offs below are operational rather than tied to a particular vendor.
| Mechanism | Best fit | Speed to benefit | Main risks | Data and compute needs | Rollback and reproducibility |
|---|---|---|---|---|---|
| Prompt or policy update | Instruction, routing, refusal, formatting, or tool-use defect | Fastest; often one release cycle | Prompt interactions, hidden regressions, instruction conflicts | Small curated cases; low compute | Easy when prompts and policies are immutable and tested |
| Retrieval or memory update | Changing facts, account state, or domain knowledge | Fast after source and index validation | Stale, poisoned, or unauthorized content; wrong retrieval | Versioned sources and index rebuilds | Good if source snapshots and index identifiers are retained |
| Fine-tuning or distillation | Consistent behavior across many examples, lower token use, or lower latency | Slower; requires a training and validation cycle | Catastrophic forgetting, data leakage, overfitting, and reproducibility gaps | Curated dataset, protected holdouts, repeatable training, and compute | Moderate; keep checkpoints, code, seeds, data hashes, and lineage |
| Reinforcement or preference optimization | Behavior with a reliable grader or reward signal | Slowest and most sensitive to reward design | Reward hacking, specification gaming, and unsafe optimization | Stable reward model or human preference data plus adversarial tests | Most complex; require reward, policy, and checkpoint lineage |
A MAP study found that 70% of surveyed production systems primarily use prompting of off-the-shelf models and 74% rely primarily on human evaluation; reliability was the top development challenge. Those figures support a conservative starting point: fix controllable prompt, retrieval, memory, and policy defects before changing weights, and introduce optimization only when the expected benefit justifies its added evaluation burden.
When retrieval is the right fix
Use retrieval for facts that change faster than a model can be retrained, private account state, and domain documents that must remain traceable to a source. Version source snapshots, enforce freshness and authorization rules, scan for prompt injection and poisoned content, and test retrieval misses as well as correct hits. A new index is a release asset, not an invisible background update.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →When weight updates are justified
Consider fine-tuning or distillation when the same behavior must be applied consistently across a broad case distribution, or when reducing prompt length and inference cost is a measurable requirement. Before promotion, demonstrate that the change generalizes beyond training examples, preserves protected capabilities, and does not increase unsafe actions or policy violations.
Design an evaluation suite that can block a bad release
Build the suite from representative tasks, high-severity incidents, known edge cases, and protected safety scenarios. Keep a fixed core so scores remain comparable over time, and add a replay set from newly verified production cases.
Required evaluation layers
- Unit tests: validate parsers, memory operations, policy predicates, routing logic, and cost calculations.
- Tool and API contracts: check argument schemas, authorization, retries, timeouts, idempotency, and handling of malformed responses.
- Task quality: measure success, factuality or groundedness, completeness, and format compliance against human-reviewed references.
- Safety and policy: test refusal, privacy, prompt-injection resistance, unsafe-action prevention, and escalation behavior.
- Replay evaluation: rerun sanitized production cases with the same tools, retrieval snapshots, and relevant state.
- Performance budgets: measure latency, token use, infrastructure cost, intervention rate, and timeout frequency.
- Segment checks: compare performance by task type, language, customer tier, risk class, tool path, and other groups where failures could be hidden by an average.
Set gates against the incumbent
Every candidate must meet or exceed the incumbent on critical quality and safety measures. Define blocking thresholds before looking at candidate scores. A practical gate record contains the metric, baseline, candidate result, minimum acceptable change, confidence method, segment requirement, and owner. A candidate that improves average helpfulness but increases unsafe actions in a protected segment must not pass.

OpenAI’s evaluation cookbook illustrates converting human or evaluator-model feedback into evaluations, aggregating scores, and updating a baseline. Its example target score and retry limit are not universal settings; choose thresholds from the consequences of failure in your domain and document why they are appropriate.

Make graders trustworthy
Calibrate automated graders against independently labeled examples, measure disagreement, and periodically review false positives and false negatives. Keep grader versions and rubrics alongside the suite. For high-impact decisions, require human review rather than allowing a single model-based judge to approve its own behavioral change.
Run a release procedure with explicit approval and rollback
- Open a change record. State the observed problem, affected users, proposed mechanism, expected benefit, risks, owner, and rollback target.
- Freeze inputs. Pin model, prompt, tools, retrieval index, memory policy, dataset hashes, grader versions, and infrastructure dependencies in the release manifest.
- Generate candidate assets. Apply the smallest change that addresses the labeled failure. Preserve the incumbent unchanged.
- Run automated checks. Execute unit, integration, schema, tool-contract, safety, regression, replay, latency, and cost tests.
- Review evidence. Compare overall and segment-level results with the incumbent, inspect severe failures, and have a domain or safety owner sign off where required.
- Shadow the candidate. Send copied requests without allowing candidate outputs to affect users or external systems. Compare decisions, tool calls, retrieval, latency, and cost.
- Canary safely. Route a small, controlled share of real traffic, disable irreversible tools or require confirmation when appropriate, and define an observation window.
- Promote gradually. Increase traffic only while quality, safety, latency, cost, drift, and intervention metrics remain within gates.
- Record the outcome. Attach scores, alerts, approvals, deployment times, final manifest, and rollback pointer to the changelog.
Rollback conditions
Pause promotion or revert automatically for a safety violation, severe quality regression, schema failure, unexpected tool action, material drift, or service-level breach. A rollback is only credible if the previous manifest, model or checkpoint, prompts, index, memory policy, and infrastructure configuration remain available and can be redeployed without rebuilding them from mutable sources.
Use behavior contracts and a safety case
A behavior contract defines what the agent must and must not do independently of how it is implemented. Include:
- Supported tasks, out-of-scope requests, and required escalation paths.
- Facts or sources that must ground an answer and how uncertainty is expressed.
- Tool permissions, confirmation requirements, rate limits, and prohibited actions.
- Privacy, retention, memory-write, and deletion obligations.
- Output schemas, accessibility or language requirements, and service targets.
- Known failure modes, severity definitions, and pause or rollback triggers.
Microsoft recommends keeping behavior contracts separate from implementation because models, prompts, tools, knowledge, memory, policies, orchestration, and user access can all change. Store each contract beside the evaluation suite and require it to pass at every release.
Recommended Free Tools
For a sensitive workflow, prepare a release safety case containing intended behavior, known failure modes, evaluation evidence, containment controls, monitoring coverage, independent or dissenting review, approval ownership, and explicit pause and rollback conditions. OpenAI has argued that structured safety documentation should be required before continuing frontier reinforcement-learning runs; applying that principle to agent releases makes the evidence reviewable even when the underlying change is a prompt or retrieval update.
Monitor the live system as a changing distribution
Release scores are snapshots. Production reliability must be measured continuously because users, tools, documents, and attack patterns change.

Core dashboards
- Task success and user correction rate.
- Factuality or groundedness, citation or source-use quality, and retrieval hit or miss rates.
- Policy-compliance rate, unsafe-action rate, refusal quality, and human intervention rate.
- Tool error, timeout, retry, authorization-denial, and schema-failure rates.
- End-to-end latency, token consumption, infrastructure cost, and budget utilization.
- Input and retrieval distribution drift, new failure clusters, and changes in segment mix.
Alert severity and runbooks
Map each alert to an action, owner, and time limit. A high-severity safety alert should stop promotion or reduce traffic and page the accountable owner. A reliability or drift alert can open a replay and curation job while preserving the incumbent. A low-severity style defect can enter the next labeling batch without interrupting service. Every runbook should specify how to capture evidence, contain the agent, decide whether to roll back, notify affected parties, and close the incident.
Maintain a changelog linking each update to its evidence, decision, approver, deployment scope, observed impact, and rollback target. This turns monitoring from a dashboard exercise into an auditable learning process.
Deployment strategies and when to use them
| Strategy | How it works | Best use | Required checks | Main limitation |
|---|---|---|---|---|
| Offline-only | Evaluate on fixed and replay datasets without serving the candidate | Early screening and high-risk changes before any traffic | Quality, safety, segment, latency, and cost tests | Cannot reveal live distribution, integration, or user-behavior effects |
| Shadow evaluation | Copy live requests to the candidate while the incumbent serves the user | Tool, retrieval, latency, and output-difference analysis | Isolation of side effects, privacy controls, and comparison logging | Candidate feedback cannot safely test actions that change the world |
| Canary | Expose a small controlled traffic share with close observation | Changes that passed offline and shadow gates | Safety, quality, latency, cost, intervention, and rollback thresholds | Small samples can miss rare failures |
| Staged rollout | Increase traffic or scope in defined steps | Broad releases where risk varies by segment | Per-stage approval and automatic pause criteria | Requires operational discipline and enough traffic per stage |
| Full promotion | Serve the candidate to all eligible traffic | Only after staged evidence and owner approval | Live monitoring, alerting, lineage, and tested rollback | Blast radius is highest if an unseen defect remains |
Common failure modes and fixes
“Learning” means updating weights from every conversation
Why it fails: unreviewed data can contain secrets, attacks, incorrect preferences, and correlated errors; online updates also make behavior difficult to reproduce.
Fix: buffer traces, filter and label them, protect holdouts, and release changes through the two-loop process.
Average score improves while a critical group gets worse
Why it fails: aggregate metrics hide segment regressions and rare but severe failures.
Fix: define protected segments and severity-weighted gates before evaluation, then block candidates that fail any critical segment.
A retrieval update silently changes answers
Why it fails: mutable documents, indexes, ranking settings, or permissions were not pinned.

Fix: snapshot and version sources, index builds, ranking configuration, freshness rules, and authorization tests; evaluate both stale and poisoned-content scenarios.
An automated grader rewards the wrong behavior
Why it fails: the grader is exploitable, poorly calibrated, or optimized as if its score were the complete specification.
Fix: use adversarial cases, independent human labels, grader versioning, disagreement analysis, and human approval for consequential changes.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsRollback restores the model but not the surrounding behavior
Why it fails: prompts, tools, memory policies, indexes, or infrastructure were changed independently.
Fix: deploy an atomic release manifest and retain every referenced artifact, not merely the model checkpoint.
A practical operating checklist
- Is the proposed change tied to a labeled production failure or a clearly defined improvement?
- Have secrets and personal data been removed or access-controlled?
- Are training candidates separated from safety and regression holdouts?
- Are model, prompt, tools, retrieval, memory, graders, code, and data versions pinned?
- Does the candidate pass unit, integration, contract, safety, replay, latency, cost, and segment tests?
- Does it meet every critical incumbent gate rather than only improving an average?
- Has a domain or safety owner approved high-impact behavior?
- Has the candidate been shadowed or canaried with side effects contained?
- Are live alerts connected to owners, runbooks, pause rules, and rollback automation?
- Can an operator redeploy the previous manifest and reconstruct the evidence for the decision?
Continual learning is therefore controlled adaptation, not unrestricted online training. Production traces become useful learning assets only after privacy filtering, labeling, deduplication, and holdout separation. The safest pipeline changes the least necessary, proves the change against the incumbent, exposes it gradually, and keeps enough immutable evidence to reverse and explain every decision.
