Pharmaceutical outsourcing has evolved from occasional purchases of laboratory capacity into a strategic, global operating model. Sponsors now combine contract research organizations (CROs), contract development and manufacturing organizations (CDMOs), functional-service providers, specialist laboratories and technology partners across discovery, trials, regulatory work, manufacturing, packaging, data and supply chains.
The change matters because outsourcing can reduce fixed costs, accelerate development and provide capabilities a single company cannot maintain internally. It also creates obligations: the sponsor remains accountable for quality, data integrity, patient protection and product supply even when a contractor performs the work. The strongest model is therefore not “outsource everything,” but a governed network with explicit responsibilities, measurable performance and resilience built into contracts.
From selective contracting to a networked model
Earlier pharmaceutical companies generally kept discovery, development and manufacturing inside the organization. They hired external consultants, testing laboratories or manufacturers when they needed temporary capacity or a specialist that was unavailable internally. This was tactical outsourcing: the company retained the core operating system and bought help around the edges.

Several forces changed that pattern. Sponsors centralized research and development and moved repeatable or specialist work to CROs, converting some fixed internal costs into variable external spending. Global trials became harder to run without local investigators, regulatory knowledge and access to large patient populations. Biotechnology created demand for capabilities—such as complex biologics development and specialized manufacturing—that many traditional pharmaceutical companies did not own. Pressure to reach the market faster made a global provider network more attractive than building every function from scratch.

Outsourcing now commonly covers discovery support, preclinical studies, clinical operations, biostatistics, data management, pharmacovigilance, regulatory affairs, real-world evidence, laboratory and diagnostic services, manufacturing, packaging and parts of distribution. The Scottish Government identifies CRO and CDMO activity across discovery, trials, manufacturing and packaging as a major sector trend.
This is better understood as a networked operating model. A sponsor may retain portfolio strategy, medical decision-making and final quality oversight while assigning different work packages to providers with distinct technical, geographic or therapeutic strengths. The challenge shifts from performing every task to integrating the network without losing control.
The major stages of the outsourcing evolution
1. Internal discovery with occasional external help
Companies first outsourced overflow testing, consulting, specialist assays and short-term manufacturing capacity. The sponsor usually controlled the methods, systems and project management, and the contractor delivered a narrowly defined output.

CROs became operating partners for clinical execution, site and patient recruitment, biostatistics, data management and safety activities. Their geographic footprints let sponsors run studies in multiple countries without establishing a permanent organization in each market. The economic rationale was partly financial—turning fixed infrastructure into variable spend—and partly operational: faster enrollment, access to scarce expertise and the ability to scale a program up or down.
3. CDMOs move beyond spare capacity
Contract manufacturers increasingly added process development, analytical development, scale-up, technology transfer and regulatory support. That made a CDMO a development partner rather than simply a plant that filled an existing process. Biotech companies, in particular, often relied on these partners because they needed sophisticated capabilities without owning a full manufacturing network.
4. Regulatory and data specialization
Regulatory writing, submissions, safety reporting, analytics and data operations moved into specialist teams. IQVIA describes work being coordinated through combinations of offshore and local hubs, with automation changing routine regulatory tasks. This model can provide coverage across time zones and jurisdictions, but it requires clear ownership of source data, decisions and submission content.

5. Functional and hybrid service models
Functional-service-provider (FSP) arrangements let a sponsor buy one capability—such as monitoring, data management or biostatistics—while retaining program management. Hybrid arrangements combine FSP specialists with a full-service CRO or CDMO. PPD’s 2024 FSP Trends Report says adoption of FSP and hybrid FSP/full-service models is accelerating, reflecting the need for flexibility, continuity and access to scarce talent.
6. Digital and resilience-focused networks
Current selection decisions increasingly include artificial intelligence, real-world evidence, interoperable data, nearshoring and supplier diversification. A provider that is fast but opaque, dependent on one site or unable to expose reliable data may create more risk than it removes.
What CROs and CDMOs actually do
The labels overlap in practice, so evaluate the contracted work rather than the acronym. A CRO is usually associated with research and clinical services; a CDMO with development and manufacturing. Both may provide regulatory, analytical, quality or supply-chain support.
| Provider role | Typical activities | When a sponsor uses it |
|---|---|---|
| CRO | Protocol and study planning, site selection, patient recruitment, monitoring, clinical operations, biostatistics, data management, medical writing, safety and pharmacovigilance | When the sponsor needs trial execution capacity, country coverage or specialist clinical expertise |
| CDMO | Process and formulation development, analytical methods, scale-up, technology transfer, clinical or commercial manufacturing, packaging and related quality documentation | When the sponsor needs development or production infrastructure without building its own facility |
| Laboratory or diagnostic partner | Central laboratory testing, bioanalysis, biomarker work and diagnostic services | When validated testing, specialized instruments or broad sample logistics are required |
| Regulatory and data specialist | Submissions, regulatory intelligence, safety analytics, data standards, statistics and real-world data/evidence | When the sponsor needs jurisdictional knowledge, additional capacity or advanced analytics |
A single program can use all four categories. The sponsor must define interfaces—for example, who owns a query between a central laboratory and the clinical database, who approves a manufacturing change, and who reconciles safety data before a submission.
Why companies outsource—and what they give up
Economic flexibility
External teams can convert some permanent salaries, facilities and equipment into project-based or volume-based costs. That is valuable for small biotechs and for large companies whose pipelines fluctuate. It does not guarantee a lower total cost: governance, technology transfer, travel, audits, change orders and rework can erase an apparently low bid.

Specialized capability
Modern medicines often require rare scientific, technical or regulatory expertise. Outsourcing gives a sponsor access to that expertise without maintaining every discipline between projects.
Trade-offs
- Control becomes indirect: the sponsor manages through specifications, oversight and escalation rather than daily execution.
- Knowledge can fragment across vendors, especially when data, samples or processes move between systems.
- Provider capacity may be shared with other clients, creating scheduling or priority conflicts.
- Concentration at one site or supplier can turn a local disruption into a global shortage.
- Switching providers late in development is expensive because methods, records and qualified processes must transfer.
Choosing an outsourcing model
| Model | Best fit | Main advantage | Main watch-out |
|---|---|---|---|
| Single-function outsourcing | A defined need such as monitoring, statistics or packaging | Precise control over scope and specialist selection | The sponsor must integrate more interfaces |
| FSP | Ongoing access to one or more functional teams under sponsor direction | Flexible capacity and continuity without handing over the whole program | Roles can blur unless line management, priorities and escalation are explicit |
| Hybrid FSP/full-service | A sponsor wants a lead provider plus targeted specialists | Combines coordination with access to scarce expertise | Data and decision handoffs need formal governance |
| Full-service CRO or CDMO | Programs requiring broad execution across countries or development stages | Fewer commercial interfaces and a single operational backbone | Concentration risk and less flexibility if performance declines |
There is no universally superior model. The choice should follow the sponsor’s internal strengths, program complexity, geography, phase, need for control and tolerance for vendor-management work.
How to evaluate a CRO or CDMO
1. Define the scope and interfaces
Write a work breakdown before requesting proposals. Identify deliverables, assumptions, dependencies, decision rights, acceptance criteria and what remains inside the sponsor. Ask bidders to mark every handoff and excluded activity. Ambiguity at this stage becomes a change order later.

2. Test technical and therapeutic fit
Review named personnel, relevant therapeutic experience, platform or modality expertise, validated methods, inspection history and the provider’s ability to support the next development stage. A long customer list is less useful than evidence that the proposed team has delivered comparable work.
3. Check geographic reach and real capacity
Confirm where work will actually occur, not merely where the provider has offices. For clinical services, examine investigator and patient access, language coverage and country start-up experience. For manufacturing, verify the specific facility, suite, equipment, batch history and available production windows. PPD reports that partner selection often depends on global capability and support for expansion into countries outside a provider’s existing footprint.
4. Examine quality and data governance
Request the quality manual, deviation and CAPA procedures, training records, validation approach, computerized-system controls, audit reports and data-retention policy. Confirm how electronic records, metadata, audit trails, samples and source documents remain attributable, legible, contemporaneous, original and accurate. EMA guidance states that outsourced activities belong within the sponsor’s Pharmaceutical Quality System and that the sponsor must be able to access relevant contracts and perform qualified audits.

5. Assess resilience
Map critical raw materials, single-site processes, key personnel and subcontractors. Ask about backup laboratories, alternate manufacturing sites, nearshoring options, disaster recovery, cyber response, inventory buffers and business-continuity exercises. FDA materials summarizing HHS work on shortages highlight vulnerabilities in generic sterile injectables and point to domestic capacity, innovative manufacturing, mature quality management and better data use as resilience priorities.

Link fees to transparent milestones and volume assumptions. Define service levels, reporting cadence, forecast changes, pass-through costs, rate cards, travel, change control, termination assistance, ownership of work product and transition support. Include escalation contacts with authority to make decisions, not only account-management titles.
Governance: outsourcing execution does not outsource accountability
The sponsor remains responsible for the product, patient safety, regulatory commitments and the integrity of records, even when a contractor performs the activity. EMA guidance requires clear written arrangements, documented responsibilities, qualified audits and assessment of conflicts of interest. The contract chain should identify every subcontractor and preserve the sponsor’s access to records and inspectors.
“In addition to having their own data governance systems, companies outsourcing activities should verify the adequacy of comparable systems at the contract acceptor.” — European Medicines Agency, GMP/GDP Inspectors Working Group
Translate that principle into operating controls:
- Maintain a responsibility matrix covering planning, execution, review, approval, reporting and escalation.
- Hold joint quality and program reviews with trend data, deviations, overdue actions and inspection findings.
- Use risk-based audits throughout the relationship, including on-site audits where the activity warrants them.
- Control subcontracting through prior approval, qualification and the same data and quality requirements imposed on the primary provider.
- Keep sponsor access to systems, metadata and essential records for the full retention period.
EMA also cautions that paper-based audits cannot replace on-site audits of active-substance suppliers. Desktop reviews can support a risk-based program or serve as an interim measure, but they do not demonstrate actual site conditions.

Supply-chain resilience is part of the business case
Low unit cost is not the same as reliable supply. A sponsor should model the effect of a failed site, delayed raw material, cyber incident, quality deviation or geopolitical disruption on patients and trial timelines. For critical products, qualify alternate sources early enough that a transfer is technically and regulatorily feasible.

Resilience measures include dual sourcing, geographically separated facilities, safety stock for long-lead materials, validated remote access, alternate logistics lanes and periodic recovery tests. The appropriate level depends on product criticality, shelf life, demand volatility and the time required to qualify a replacement. A resilience plan that exists only in a contract appendix is not operational until people have rehearsed it.
Digital, AI and real-world evidence in the next phase
Automation is changing routine regulatory and data work, but it does not remove accountability for the underlying evidence. Require providers to disclose where automated tools are used, what data they train on or access, how outputs are validated, and how human review is documented. Contracts should address model changes, explainability, cybersecurity, intellectual property and retention of prompts, outputs and audit trails where relevant.
Real-world data and evidence can complement trial data, support external-control analyses or inform safety monitoring. Their value depends on provenance, representativeness, linkage quality and a defensible analytic plan. A provider’s ability to produce a large volume of data is not proof that the data answer the sponsor’s clinical or regulatory question.
The Scottish Government’s 2026 sector page cites a projection that the healthcare AI market could approach $188 billion by 2030, implying a 37% compound annual growth rate from 2022 to 2030. That is a market forecast reproduced from Statista, not a guarantee of adoption or regulatory acceptance. The same page cites a US biopharma and pharmaceutical-services outsourcing market estimate of $10.3 billion in 2023 and a projected 5.05% CAGR from 2024 through 2030; the figure is scope- and geography-specific rather than a universal total.
Evidence of scale—and how to interpret it
IQVIA reports that it has supported more than 7,000 product registrations in over 150 countries from 2015 to the present. This illustrates the geographic scale available through specialized providers, but it is an IQVIA-reported service figure, not a count of all industry registrations.
IQVIA also reports that externally sourced compounds rose from about one-third a decade earlier to between 51% and 70% of approved drugs sold by large pharmaceutical companies. The range is an industry estimate in an IQVIA insight brief, not an official regulator database statistic. It signals the strategic importance of external innovation while leaving room for differences in definitions, time periods and company samples.
A practical selection process
- Set the strategy. Decide which capabilities must remain internal, which can be bought, and what “success” means for time, quality, cost, enrollment or supply.
- Build the scope. Document activities, interfaces, assumptions, data flows, milestones, countries, facilities and acceptance criteria.
- Shortlist by evidence. Match therapeutic, technical, geographic, quality and capacity requirements before comparing price.
- Run operational diligence. Interview the proposed team, inspect relevant sites or systems, review deviations and confirm subcontractors and backup plans.
- Score the proposal. Weight delivery record, expertise, collaboration, tailored recruitment, global capability, data governance, resilience and total cost. PPD identifies on-time delivery, expertise, collaboration and expansion support as important selection factors in its 2024 FSP report.
- Contract the interfaces. Finalize the responsibility matrix, quality agreement, data-access rights, change control, service levels, escalation, audit rights, continuity and exit assistance.
- Govern continuously. Use a joint dashboard and scheduled quality reviews; do not wait for a missed milestone to discover that ownership was unclear.
- Plan the exit from day one. Require orderly transfer of data, methods, materials, records and knowledge if the relationship ends or the provider fails.
Common failure modes and fixes
| Failure mode | Why it happens | Preventive action |
|---|---|---|
| Lowest bid wins, then costs rise | Scope exclusions and optimistic assumptions hide effort | Compare total cost, assumptions, change-order rates and internal oversight burden |
| Handoffs lose information | Different systems and unclear ownership create reconciliation gaps | Define data standards, interfaces, review points and a single source of truth |
| Provider staff turn over | Critical knowledge resides with individuals | Require succession plans, documented training and sponsor access to work products |
| Audit finds a systemic issue late | Desktop checks substitute for meaningful oversight | Use risk-based qualification, including on-site audits for critical suppliers |
| One disruption stops supply | Single-site or single-supplier concentration was not mapped | Qualify alternatives, hold appropriate inventory and test continuity plans |
| Automation produces an unreliable output | Tool use was not validated or reviewed | Document intended use, validation, human approval, change control and audit trails |
What the mature model looks like
Mature outsourcing is a controlled extension of the sponsor’s organization. The sponsor chooses where external scale creates an advantage, keeps strategic and quality accountability, and designs contracts around data, decisions and recovery—not just deliverables. Providers are evaluated for demonstrated capability, capacity and resilience, then managed through measurable outcomes and regular oversight. That approach captures the speed and specialization that made outsourcing attractive while limiting the quality, supply and knowledge risks created by a fragmented network.
