Machine learning can help hospitals and insurers spot claims or account activity that looks unusual, including possible misuse of a patient’s identity. It does this by ranking patterns for human review—not by proving that a person’s identity was stolen or that a claim is fraudulent. CMS describes analytics used in Medicare and Medicaid fraud prevention, but the official information available does not establish how much hospital-deployed machine learning reduces medical identity theft specifically.

What medical identity theft is—and why it matters to hospitals

The HHS Office of Inspector General (OIG) defines medical identity theft as someone using another person’s identifying information—such as a name, Social Security number, or Medicare number—without authorization to submit claims to Medicare or another health insurer. Misused information can also interfere with a patient’s care, while fraudulent claims waste money paid by patients, insurers, or public programs.

For a hospital, the problem can involve more than a suspicious bill. A compromised identifier may be used in claims activity, while inaccurate or misused records can create confusion about a patient’s care. The OIG’s 2012 evaluation described financial and patient-safety risks, but its figures cover an earlier period and should not be read as a measure of the problem today.

How machine learning can surface suspicious activity

Machine-learning systems can analyze claims and related information to find patterns that merit a closer look. CMS says that linking claims with provider ownership information, pharmacy records, and external data can reveal activity that might otherwise go unnoticed. Its Fraud Prevention System uses analytics and predictive modeling to flag aberrant billing patterns in real time.

Examples CMS gives include an unusual increase in billing, an improbable combination of services, or activity that looks anomalous geographically. If stolen Medicare Beneficiary Identifiers or other member IDs are being used to submit claims, patterns associated with those claims may help investigators identify suspicious use of an identifier. That is a potential detection route, not evidence that a model can determine on its own whether a particular patient’s identity was stolen.

What a flag means in practice

  1. Data is analyzed. A system looks for deviations or relationships in the available claims and other permitted data.
  2. A case is prioritized. An unusual pattern is surfaced for attention; it is an investigative lead, not a finding of fraud.
  3. A person reviews the context. Trained staff can check records and circumstances, resolve false alarms, and decide whether further action is warranted.
  4. Action is proportionate to the evidence. A flag should not by itself block necessary care or be treated as proof against a patient, provider, or claim.

CMS has emphasized explainable methods and meaningful human oversight in its machine-learning challenge. As CMS testimony put it on March 17, 2026, the challenge sought technologies that reduce labor-intensive processes “while keeping humans meaningfully in the loop to ensure effective oversight and interoperability.”

Detection is only one part of protecting identifiers

Analytics may help identify suspicious use after activity begins, but limiting the usefulness of compromised credentials is another possible approach. In its IDea Challenge, CMS highlighted proposals such as replacing a static member identifier with a transaction-based or provider-specific token, checking provider identity in real time using biometrics or digital credentials, and sending beneficiaries real-time alerts or offering mobile apps.

These were ideas highlighted through a challenge, not controls shown to be universally deployed or proven to prevent identity theft. They also address different points in a process: a token can limit exposure of a reusable identifier, an identity check can add verification at a transaction, and an alert can help a beneficiary notice unexpected activity. Any implementation needs to account for privacy, interoperability, access to care, and what happens when a legitimate patient cannot pass a check.

Approach Potential role What the evidence establishes
Static member identifier Identifies a member across transactions, but may be reused if exposed. CMS says stolen beneficiary identifiers and other member IDs are used in fraudulent claims.
Transaction- or provider-specific token Could reduce reliance on a reusable identifier in a particular transaction or provider relationship. Highlighted as a concept in the CMS IDea Challenge; universal deployment or measured results are not established.
Analytics and predictive modeling Can surface unusual claims patterns for investigation. CMS describes these capabilities in fraud analytics; hospital-specific outcomes for medical identity theft are not established.
Beneficiary alert or identity check Could help verify an interaction or make unexpected activity visible sooner. Highlighted as challenge ideas; effectiveness and routine availability are not established.

Hospitals still need the security groundwork

Machine learning depends on the data and systems around it. HHS’s Office for Civil Rights (OCR) recommends that covered organizations understand where electronic protected health information (ePHI) flows, perform risk analysis and risk management, maintain audit controls, regularly review system activity, authenticate users, and encrypt ePHI in transit and at rest when appropriate. OCR also recommends workforce training and using lessons from incidents to improve security. These are security practices, not machine-learning products.

A concrete reminder came in OCR’s April 17, 2025 announcement about Guam Memorial Hospital Authority. OCR said its investigation found the hospital had not conducted an accurate and thorough risk analysis after complaints about potential ePHI disclosures. The corrective-action terms included reviewing audit logs and access reports, improving access management, and assessing breach-notification obligations. OCR Acting Director Anthony Archeval said failure to conduct a HIPAA risk analysis leaves information vulnerable to ransomware and other cyber-threats.

What CMS’s current activity does—and does not—show

CMS’s 2026 announcements illustrate active work on fraud analytics, but participation figures are not proof of effectiveness against medical identity theft. In the first phase of its Crushing Fraud Chili Cook-Off, CMS reviewed more than 259 applications. Ten finalists in the second phase received access to a CMS Limited Data Set of Medicare Hospice, Part B, and durable medical equipment (DME) claims. The CMS IDea Challenge had 78 attendees from technology, government, and healthcare sectors. These figures describe the agency’s challenge activity, not a hospital deployment or a measured reduction in stolen-identity claims.

The historical OIG evaluation offers a dated illustration of the risks, not current prevalence: from September 23, 2009, through December 31, 2011, it recorded 14 reportable breaches of protected health information affecting 13,775 Medicare beneficiaries. The sources available do not provide a hospital-specific false-positive rate, savings estimate, or patient-outcome statistic for machine learning deployed specifically against medical identity theft.

What patients can do about a questionable charge

Check medical bills and statements for services you do not recognize, and protect personal information. If a charge looks wrong, HHS OIG advises contacting the provider first to find out whether it is a mistake. For unresolved Medicare concerns, OIG points people to 1-800-MEDICARE or a local Senior Medicare Patrol; suspected Medicare fraud can also be reported to the OIG hotline.
