The key was physical propagation: malware on an infected computer could copy itself to a newly attached removable drive, which could then carry it to another computer—including, potentially, one isolated from the internet.
How the USB infection chain worked
Mandiant’s November 28, 2022 report describes USB devices as the campaign’s initial infection vector. The infection chain began when someone plugged in a compromised removable device and manually ran a renamed, signed program from the drive’s root directory. Mandiant identified the files as “Removable Drive.exe” or “USB Drive.exe”; they were versions of the legitimate USB Network Gate application from Electronic Team, Inc.
- A user ran the program on the USB drive. The signed application was abused to side-load MISTCLOAK, a malicious launcher.
- MISTCLOAK loaded DARKDEW. This dropper established persistence and could infect removable drives connected to the compromised computer.
- Infected drives carried the malware onward. A person could plug one into another computer, where a user might again run a malicious executable.
- In a later phase, BLUEHAZE launched NCAT. The renamed networking utility created a reverse shell to a hardcoded command-and-control server, providing a route for remote access.
The chain depended on a person running the initial executable; the report does not describe USB insertion alone as automatically infecting every computer.
What MISTCLOAK, DARKDEW, BLUEHAZE, and NCAT did

Could the malware reach an air-gapped network?
Potentially. Mandiant says the malware could replicate to newly attached removable drives, allowing payloads to propagate to additional systems and potentially collect data from air-gapped systems. A USB drive can act as a physical bridge between computers even when those computers have no direct network connection.

That does not mean an air gap is bypassed remotely or that plugging in any drive automatically causes infection. The drive has to move between systems, and the malware must be able to run or otherwise execute in the destination environment. The campaign illustrates why an isolated network still needs controls for removable media.
Who Mandiant attributed the activity to
Mandiant tracks the activity as UNC4191 and assesses that it has a China nexus. Its report describes the operation as consistent with Chinese cyberespionage aimed at gaining and maintaining access to public and private entities for intelligence collection related to China’s strategic political and commercial interests. Mandiant did not name a specific Chinese government unit, and the reviewed reporting does not establish a reliable total victim count.

The activity primarily affected public and private entities in Southeast Asia, with additional activity extending to the United States, Europe, and the Asia-Pacific region. Mandiant reported that the strongest concentration was on systems physically located in the Philippines—even when the affected organization was based elsewhere. The practical lesson is to treat removable media as an infection boundary, not as a harmless accessory. Controls work best when they limit the chance of execution and give defenders visibility into what happens after a drive is connected.

- Brand Yourself - Brand the flash drive with your company's name and provide company's overview, policies, etc. to the newly joined employees or your customers
- Govern which devices can connect. Use device-control policies to restrict removable storage to approved devices or authorized users, particularly on sensitive and isolated systems.
- Restrict execution from USB paths. Where operationally possible, prevent or tightly control running executables from removable drives. Signed software should not be assumed safe solely because it has a valid signature.
- Correlate device and process activity. Endpoint monitoring should help investigators connect a drive insertion with subsequent process creation and file writes. That context can make suspicious activity easier to distinguish from ordinary device use.
- Hunt for suspicious artifacts. Review unexpected executables or shortcut files on removable media and investigate unusual process ancestry associated with a drive.
- Validate the controls. Mandiant created YARA rules for MISTCLOAK, DARKDEW, and BLUEHAZE and published security-validation actions for UNC4191. Security teams can use those campaign-specific artifacts to support detection and control testing rather than relying on a general assumption that antivirus alone will stop this kind of propagation.
For personal use, do not run an unfamiliar program just because it appears on a USB drive or has a familiar-looking name. For organizational use, encrypted or managed storage can help protect data, but encryption by itself does not prevent malware from being copied to a drive or executed on a connected computer.
