AI can make attacks on critical infrastructure more capable by helping people analyze information, produce convincing material, and target weaknesses in information-technology (IT), operational-technology (OT), or AI components. It can also create new risks when its data, software, hardware, availability, or outputs are compromised. That does not mean AI is autonomously taking over power grids or water plants: the official sources reviewed describe plausible capabilities and safeguards, not a verified catalogue of AI-caused infrastructure outages. Effective oversight combines asset inventories, input and output monitoring, predefined safety limits, qualified operator decisions, and a tested way to run or restore systems without AI.
What counts as critical infrastructure?
Critical infrastructure is the interconnected set of services and assets whose disruption can affect national security, economic stability, public health, or safety. The U.S. Cybersecurity and Infrastructure Security Agency (CISA) identifies 16 critical-infrastructure sectors. Energy, water, transportation, communications, and healthcare are familiar examples, but each sector has different architectures, operating constraints, and consequences when systems fail.
Interconnection matters. A service may depend on telecommunications, cloud services, electric power, fuel, physical access systems, vendors, or shared data. An AI deployment can therefore introduce dependencies outside the immediate plant or control room. Mapping those dependencies is part of understanding the real attack surface.
Why operational technology changes the stakes
Operational technology is not simply another collection of office computers. NIST’s initial public draft of SP 800-82 Revision 4, published September 21, 2026, defines OT broadly as programmable systems or devices that interact with the physical environment or manage devices that do. Examples include industrial control systems, building automation, transportation systems, physical access control, and environmental monitoring or measurement systems.

In OT, cybersecurity objectives sit alongside reliability, availability, and functional safety. A delayed software update may be tolerable in an office but dangerous during a process-control operation. A recommendation that is statistically plausible can still be unsafe if it ignores a maintenance condition, sensor failure, unusual weather, or a physical constraint. The relevant question is not only whether an AI model is accurate on average, but whether its behavior is acceptable for this process, at this moment, with a safe way to intervene.
How can AI help someone attack critical infrastructure?
NIST describes a dual-use environment: AI can give defenders new capabilities while also enhancing people who target organizations through IT and OT attacks. In that sense, AI is an amplifier and an additional attack surface, not an inevitable autonomous attacker.
Processing information and finding opportunities
An attacker could use AI to process large volumes of public or stolen information, identify relationships among organizations and suppliers, or prioritize systems for further attention. The general risk is faster analysis and better targeting by a human operator. The available guidance does not establish a universal method, a particular tool, or a measured increase in successful attacks.
Producing persuasive material
Generative systems can help create or adapt text, translations, images, or other material used in social engineering and impersonation. Critical-infrastructure personnel may receive messages that appear to match local terminology or an ongoing project. Verification procedures, separation of duties, and out-of-band confirmation remain necessary because fluent content is not proof of identity or authorization.
Exploiting weaknesses in AI-enabled operations
An attacker may target the AI component itself, the data used to train or operate it, the software and hardware that run it, or the interfaces connecting it to other systems. NIST highlights confidentiality, integrity, and availability concerns, along with risks to training data and output data. Existing guidance does not comprehensively cover several machine-learning attack classes, including evasion, model extraction, membership inference, and availability attacks. Those gaps mean a conventional OT security review may not identify every failure mode introduced by a model.

Influencing decisions without directly taking control
An AI system that recommends a maintenance priority, alarm response, set point, or access decision can affect operations even when it cannot issue a control command. Corrupted inputs, manipulated context, or a model operating outside its design conditions could produce a recommendation that a busy operator accepts. The risk is greatest when review is nominal: the person has too little time, information, authority, or an alternative operating mode to challenge the output.
Can AI hack a power grid or water system?
AI could be used as part of an attempt to compromise systems supporting a grid, treatment plant, pipeline, or other service, and AI-enabled components could themselves be attacked. However, the official material cited here does not verify a specific AI-caused power-grid or water-system outage, nor does it provide a statistic for AI-enabled critical-infrastructure attacks. Do not turn a capability statement into a claim that an incident has already occurred.
Sector differences are substantial. A utility may use protective relays, supervisory control, and geographically distributed assets; a water operator may combine pumps, chemical processes, telemetry, and laboratory measurements. The same model behavior can have different consequences in each setting. Risk assessment must therefore be tied to the actual process, architecture, and safety case rather than to the label “AI.”
Three distinct risk areas to assess
| Risk area | What can go wrong | Questions for an operator |
|---|---|---|
| AI-assisted attacks | A person uses AI to analyze information, create material, or improve targeting against IT or OT. | Which exposed information, identities, suppliers, and remote-access paths could make targeting easier? Which verification controls detect deception? |
| Attacks on AI components | Inputs, training data, models, software, hardware, interfaces, or availability are compromised. | Can the organization detect altered data or models? Are versions, dependencies, and access rights known? What happens when the model is unavailable? |
| AI-enabled operational failure | A recommendation or automated action is wrong, manipulated, delayed, or outside its validated conditions. | Is the action reversible? Who can approve it? What thresholds block it, and how quickly can operators switch to a known-good non-AI mode? |
What human oversight means in practice
Joint guidance titled Principles for the Secure Integration of Artificial Intelligence in Operational Technology, issued in December 2025 by CISA, Australia’s Australian Signals Directorate’s Australian Cyber Security Centre (ASD’s ACSC), and partner agencies, states: “Ultimately, humans are responsible for functional safety.” That principle requires more than placing a person somewhere in an approval chain.
1. Map every AI dependency
- Inventory models, inference services, data pipelines, sensors, gateways, software libraries, accelerators, cloud services, and vendor-managed components.
- Record which OT assets consume an AI output, which systems can send data to the model, and which downstream systems can act on a recommendation.
- Assign an owner for each component and document its approved purpose, operating conditions, update process, and access permissions.
Include components that rely on AI indirectly. A scheduling, inspection, identity, or maintenance system may create operational consequences even when it is not connected to a controller.

2. Log inputs, outputs, and consequential actions
Record the data presented to the model, the model version, its output or recommendation, confidence or uncertainty information when available, the operator’s decision, and any resulting action. Protect logs from unauthorized alteration and retain enough context to reconstruct an event. Monitoring should cover data quality, missing or unusual inputs, drift, latency, repeated failures, and unexpected command paths—not only whether a service is online.

Define a known-good state and measurable thresholds for safe behavior. Examples include allowable ranges, maximum rate of change, confidence floors, latency limits, sensor-consistency checks, and conditions in which a recommendation must be rejected. Thresholds should be derived from the process and its safety requirements, not selected solely from a model’s laboratory score.
Test abnormal conditions such as stale data, conflicting sensors, communications loss, model unavailability, an out-of-distribution event, and an operator who cannot respond within the required time. Document who can change a threshold and how the change is reviewed.
4. Match authority to consequence
For passive or advisory AI, integrate recommendations into existing decision processes so qualified personnel can review them. For systems that can trigger actions, explicitly identify which actions require human approval, what conditions permit automatic execution, and what event stops or reverses the action. A high-consequence, hard-to-reverse operation warrants stronger approval and interlocks than a low-consequence recommendation that can be undone immediately.
5. Provide a real non-AI fallback
Define the conditions that cause the system to default to non-AI operation when performance or safety thresholds are not met. The fallback may be manual control, a conventional rules-based controller, a prevalidated configuration, or another independent mechanism. It must be available during the incident, accessible to authorized operators, and tested under realistic load and communications conditions.

6. Practice restoration and incident response
Include AI-specific scenarios in security exercises and operational incident plans: poisoned or unavailable data, a compromised model, unexplained recommendations, loss of connectivity to an inference service, and an operator challenge to an automated action. Define how to isolate the AI component, preserve evidence, notify safety and engineering teams, continue essential service, validate a known-good configuration, and restore normal operation. The objective is controlled recovery, not merely detection.
Advisory AI versus action-capable AI
The following comparison uses the decision axes reflected in NIST and joint AI-in-OT guidance. It is a risk framework, not a claim that one architecture is safe by default.
| Axis | Advisory deployment | Action-capable deployment |
|---|---|---|
| Authority | Produces a recommendation for an operator or engineer. | Can initiate or alter an operational action, directly or through another system. |
| Consequence and reversibility | Usually indirect; the operator can reject or revise the recommendation. | Potentially immediate and physical; reversibility must be demonstrated for each action. |
| Visibility | Inputs, outputs, model version, and dependencies should be reviewable before use. | The complete command path, interlocks, permissions, and timing must be observable. |
| Human intervention | Review must fit the operator’s workload and decision time. | Approval, override, and emergency-stop authority must be explicit and usable in time. |
| Thresholds | Low confidence, unusual inputs, or missing data should suppress or flag a recommendation. | Threshold breaches should block, constrain, or safely terminate the action. |
| Fallback and restoration | A conventional process can often continue while the recommendation is investigated. | A tested non-AI mode and restoration procedure are essential before deployment. |
How to evaluate an AI deployment before approval
- Define the operating envelope: identify the data quality, environmental conditions, timing, and process states in which the model was validated.
- Classify consequences: rate impacts on safety, service continuity, equipment, privacy, and regulatory obligations if the output is wrong or unavailable.
- Check independence: determine whether the fallback uses the same sensors, software, network, credentials, or model assumptions. A nominal backup that shares the same failure can provide false assurance.
- Confirm human capacity: ensure the responsible operator has the context, training, authority, and time to understand, challenge, and override the system.
- Verify change control: approve model, data, prompt, firmware, and dependency changes; retain versions so an event can be reconstructed.
- Exercise failure modes: test degraded inputs, unavailable services, maliciously altered data, abnormal process conditions, and recovery without production harm.

NIST AI Risk Management Framework
NIST released the AI Risk Management Framework on January 26, 2023, for voluntary use and reports that it is being revised. It is a risk-management framework, not a certification or a guarantee that an AI system is secure.
Critical-infrastructure AI RMF profile
NIST released a concept note for a critical-infrastructure AI RMF profile on April 7, 2026. The planned profile is intended to help operators identify risk-management practices for AI-enabled capabilities. The concept note is not a final profile, so it should not be presented as completed sector guidance.

CISA’s AI Roadmap
CISA’s 2023–2024 AI roadmap announced that the agency would assess whether AI adoption could make infrastructure more vulnerable to critical failures, physical attacks, and cyberattacks, and consider mitigations. This is evidence of priorities stated at that time, not proof that the contemplated work has been completed or that a particular attack occurred.
What is verified about AI-caused infrastructure incidents?
The official sources used for this explainer do not provide a comprehensive list of real-world critical-infrastructure attacks attributable specifically to AI, and they do not quantify how much human oversight reduces incident probability or severity. They support a careful distinction:
- Established: AI has dual-use potential; AI systems and their data, software, hardware, availability, and behavior create security risks; OT failures can have physical consequences; and agencies recommend oversight, thresholds, logging, and fallback paths.
- Not established here: a verified AI-caused outage at a named power, water, transport, or healthcare operator; a universal attack recipe; or a numerical risk reduction from keeping a human in the loop.
This distinction prevents both complacency and sensationalism. Organizations can act on the documented risk-management practices without claiming that every feared scenario has already happened.
A practical oversight checklist
- Inventory AI and every system that depends on its output.
- Document data sources, model versions, interfaces, owners, and permissions.
- Log inputs, outputs, recommendations, approvals, actions, and failures.
- Set process-specific performance, safety, confidence, and timing thresholds.
- Define which actions require operator or engineer approval.
- Give operators usable explanations, time, training, and override authority.
- Trigger a safe fallback when thresholds, data quality, or availability fail.
- Maintain and test an independent non-AI operating mode.
- Include AI compromise and model unavailability in incident-response exercises.
- Preserve evidence and validate a known-good state before restoration.
The bottom line for operators
AI can raise the capability of people attacking critical infrastructure and can add failure modes inside AI-enabled operations. It does not remove the need for conventional IT and OT security, process engineering, or safety management. The strongest practical defense is visible accountability: know where AI is used, observe what it receives and produces, constrain what it can do, require qualified review for consequential decisions, and keep a tested path to safe non-AI operation. Human oversight works only when the human has the information, authority, time, and fallback needed to act.
