An FDA-approved generic is expected to provide the same clinical benefit and risks as its specific reference brand medicine. It must match that medicine’s active ingredient, strength, dosage form, and route of administration, demonstrate bioequivalence, and meet FDA quality standards. A generic pill may look different, and its inactive ingredients may differ, but those differences do not automatically mean it works differently.
Are generic drugs as safe and effective as brand-name drugs?
For an FDA-approved generic compared with its specific reference medicine, FDA requires the same active ingredient, strength, dosage form, and route of administration. Bioequivalence concerns how quickly and how much active ingredient reaches the body. It does not mean the tablet or capsule must be identical in appearance. FDA reviews generic and brand-name drugs against quality standards and monitors approved products for safety signals after approval.
This comparison applies to a generic approved against its reference medicine—not to any two medicines that happen to have similar names. For an actual prescription, confirm the exact product and reference drug rather than assuming that every product described as “generic” is interchangeable.

Why does my generic pill look different?
Manufacturers may use different colors and other appearance details, so a generic tablet or capsule can look unlike its brand-name counterpart. Conventional generics may also differ in inactive ingredients. FDA says such differences generally do not change how the medicine works.
In a minority of patients, inactive ingredients may affect tolerability or absorption. If a pill’s appearance changes unexpectedly, check its label and ask a pharmacist to confirm what it is before taking it if you are unsure. If you suspect a reaction or a change in effect, contact your prescriber or pharmacist.

How an authorized generic differs
An authorized generic is the exact same product as the branded product, apart from the brand name on the label; a tablet or capsule can still differ in color or markings. FDA says authorized generics are marketed under the brand product’s NDA and are not listed among generic approvals in the Orange Book. FDA: Authorized Generics
Can generic drugs cause different side effects than brand-name drugs?
The active ingredient—and therefore the expected clinical benefits and risks—must match the reference drug for an FDA-approved generic. But an individual may react differently to an inactive ingredient or have a tolerability or absorption concern. A problem after a switch is worth discussing with a clinician; it does not by itself show that generics as a class are less effective.

Why do generic drugs cost less?
Generic manufacturers do not have to repeat the costly animal and human studies that established the original medicine’s safety and effectiveness. Competition among manufacturers can also put downward pressure on prices.

When discussing a prescription with a pharmacist or prescriber, compare the exact active ingredient, strength, dosage form, route, and FDA-listed equivalence or availability. Also mention any allergies or sensitivities to inactive ingredients and ask about your out-of-pocket cost. What to do if you have a concern about a generic
- Confirm the medicine. Check the label and ask your pharmacist to verify the active ingredient, strength, and dosage form if the pill looks unfamiliar.
- Describe what changed. Tell your pharmacist or prescriber when the appearance, symptoms, or perceived effect changed, and provide the product details on the container.
- Ask about the specific product. Discuss inactive ingredients, allergies or sensitivities, and whether an approved generic is listed for the exact brand medicine.
- Discuss next steps before changing treatment. Do not stop or switch medicines solely on the basis of general information; work with your clinician or pharmacist.
- Consider reporting a problem. FDA accepts reports about problems with medicines through its consumer reporting guidance.
