Gene therapy is the broader clinical strategy; gene editing is one way to carry it out. A gene therapy may add or supplement a working gene, change how a gene is expressed, or switch off a harmful gene. Gene editing changes DNA at selected sites, for example by disrupting a harmful sequence or repairing a mutation. Because editing can be used as a gene-therapy method, the terms are not mutually exclusive. The U.S. Food and Drug Administration (FDA) considers human CRISPR/Cas9 use to be gene therapy.

How gene therapy and gene editing differ

The key difference is the level of description. Gene therapy describes a treatment strategy: changing gene function or the biological properties of cells to treat disease. Gene editing describes a technique: making a targeted change to DNA. An editing treatment can therefore be both gene editing and gene therapy.

Not every gene therapy edits DNA. Some treatments deliver a working copy of a gene to cells so they can produce a needed protein; others alter gene expression without making a targeted DNA change. Conversely, editing can disable a harmful gene or attempt to correct a mutation. The intended change, the cells reached, and the method of delivery matter more to a patient than the label alone.

Question Gene therapy, broadly Gene editing
What does the term mean? A clinical strategy to alter gene function or cell properties to treat disease. A technique that changes DNA at selected sites.
What change might be made? Add or supplement a working gene, modify gene expression, or switch off a harmful gene. Disrupt a selected DNA sequence or attempt to repair a mutation.
Must DNA be edited? No. A treatment may supply a gene without targeted editing. Yes. The aim is to alter DNA at a chosen site, though unintended changes are a risk.
How do the categories relate? It is the broader treatment category. It can be a method used within gene therapy; the FDA treats human CRISPR/Cas9 use as gene therapy.

“Targeted” does not mean risk-free or guaranteed to change only the intended DNA. Editing can affect an unintended site or cause other changes to genome structure, which is why developers must study the particular editing system and product.

How treatment reaches the cells

A treatment has to get its genetic material or editing machinery to the cells relevant to a disease. The two main delivery approaches are in vivo, directly into the person, and ex vivo, by modifying collected cells outside the body and returning them. Which is feasible depends on the disease, target organ and cell type, and whether those cells can be collected and reinfused.

In vivo: delivery inside the body

In an in vivo treatment, a vector or editing system is administered to the patient and is intended to reach a target tissue or cell population. Viral vectors are commonly engineered to remove their ability to cause infection while retaining their capacity to deliver genetic material. Reaching the right cells is a central practical challenge: a treatment can only produce its intended effect where enough relevant cells receive it and respond as intended.

The specific route of administration, vector, target tissue, and monitoring plan vary by product. A general description such as “gene therapy” does not establish how a particular treatment is given or whether it can reach a patient’s affected tissue.

Ex vivo: modify cells, then return them

In an ex vivo approach, clinicians collect a population of cells—such as blood or bone-marrow cells—then modify them in a laboratory. The cells may be tested or prepared before being returned to the patient by infusion. This approach can make it possible to work directly with a collected cell population, but it requires a suitable source of cells, individualized processing, and a plan for safely returning them.

Ex vivo treatment is not an option for every condition. Whether it is practical depends on whether the relevant cells can be collected, modified in a controlled process, and reinfused in a way that can help the disease.

What conditions may be treated

Gene-based treatments are being developed for inherited disorders and other diseases, but development is not the same as approval or proven benefit. The National Institutes of Health (NIH) identifies cystic fibrosis, retinal degeneration, thalassemia, hemophilia, sickle-cell disease, and some hearing loss among conditions for which gene therapies are being developed. FDA and NIH also describe approved or clinically used products for selected sickle-cell disease, inherited retinal disease, cancers, and other rare-disease indications.

Those examples do not mean every patient with a named condition is eligible or that every form of the condition has an available treatment. Authorization applies to a defined product and indication. The relevant questions are whether a treatment is authorized for the patient’s exact diagnosis and circumstances, what evidence supports its use, and whether the patient meets the product’s eligibility criteria.

Gene therapy is not a general-purpose cure for genetic disease. A disorder may involve a gene or tissue that is difficult to reach, a cell type that cannot be collected for ex vivo treatment, or biological features that make a proposed intervention unsuitable. Even when a treatment exists for one inherited condition, that does not establish that a similar strategy will work for another.

Risks: what can go wrong

Risk depends on the specific product, delivery method, target cells, disease, and patient. There is no universal success rate, failure rate, or risk percentage that meaningfully compares gene therapy as a whole with gene editing as a whole. A percentage from one product or indication cannot safely be generalized to another.

Risks shared across gene therapies

  • Immune or allergic reactions: The body may react to the treatment or its delivery system. Pre-existing or treatment-induced immunity to a vector can also affect whether delivery is possible or how well it works.
  • Administration-related injury: Giving a treatment can damage an organ or tissue. The nature of that risk depends on the product and how it is administered.
  • Cancer risk: Certain gene-therapy approaches carry cancer-related concerns that need to be assessed for the specific product. Risk is not identical across all forms of gene therapy.
  • Uneven effect: A treatment may not reach or change enough of the intended cells, or the level of gene expression may vary. That can limit the intended benefit.

Additional concerns with genome editing

  • Off-target editing: The editing system may change DNA at an unintended site.
  • Unintended structural changes: Editing can cause changes beyond the planned small alteration, including larger changes that affect genome integrity.
  • Incomplete or uneven editing: Not all target cells may be edited, and the amount of editing can vary between cells.

FDA’s 2024 guidance asks developers to address both the underlying risks of gene therapy and the additional risks from editing. Its 2026 draft guidance recommends using next-generation sequencing and bioinformatics to look for unintended editing events and structural changes. These are regulatory expectations for evaluating products, not guarantees that every possible risk can be detected or eliminated.

Limitations beyond immediate safety

A treatment can be scientifically plausible and still face practical or evidentiary limits. The main issues are whether it reaches the intended cells, whether its effect lasts, whether it can be manufactured consistently for an individual patient, and how much long-term evidence is available.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Delivery and immune barriers

In vivo treatments must reach the relevant organ or cell type. A vector may be less suitable if the patient has pre-existing immunity to it, and a response to the treatment may affect later delivery. In ex vivo treatment, the relevant cells must be collectable and capable of being processed and returned. These constraints help determine whether a delivery route is feasible for a particular disease; they cannot be resolved by choosing an editing tool alone.

Durability and repeat treatment

Whether a treatment’s effects persist—and whether another treatment could be given—is product-specific. The general category “gene therapy” does not tell a patient whether the effect will be permanent, whether treatment must be repeated, or how a repeat dose would work. Those answers depend on the product, the cells treated, and the available follow-up evidence. Ask the treating team what is known for the exact therapy and what remains uncertain.

Manufacturing and follow-up

Ex vivo treatments may require individualized collection and laboratory processing. This can make manufacturing and treatment logistics demanding. Both approaches also require monitoring appropriate to the product and the potential risks. Long-term follow-up matters because some effects or complications may not be apparent soon after treatment, and the evidence base for genome editing is smaller than for gene transfer, which has been used clinically longer.

Is gene editing safer than gene therapy?

There is no sound general answer that one category is safer. The categories overlap, and a broad label does not specify the product, delivery system, target cells, or disease. A non-editing gene therapy can have meaningful delivery or immune risks; an editing treatment adds concerns such as off-target changes and unintended structural effects. The balance of potential benefit and harm must be assessed for the exact product and indication.

For a meaningful comparison, look at what genetic change is intended, how the treatment reaches cells, what risks were observed or anticipated for that product, how long and in whom it has been studied, and what monitoring is planned. FDA requires products to be evaluated in clinical studies for safety and effectiveness before marketing in the United States. That process supports a benefit-risk assessment; it does not mean a treatment has no risk.

Somatic treatment versus heritable editing

Current clinical development focuses on somatic treatment: changing cells in the person being treated. The intended effects are in that individual’s treated cells. By contrast, editing sperm, eggs, or embryos could pass changes to descendants and may have unanticipated effects across generations.

NIH states that it does not perform or fund human germline-editing studies. It also notes that almost all countries and many organizations prohibit or strictly regulate this work. Those statements concern human germline editing; they should not be confused with clinical treatment that modifies a patient’s somatic cells.

Approval, clinical studies, and do-it-yourself claims

In the United States, authorization is specific to a product and its indication. FDA says a gene therapy must be tested in clinical studies for safety and effectiveness before it can be marketed for use in humans, so the agency can consider whether the risks are acceptable in light of potential benefits. Human genome-editing products also require an Investigational New Drug (IND) submission with information on design, manufacturing and testing, nonclinical safety, and clinical-trial design.

A treatment being studied is not the same as an FDA-approved treatment. Before considering a clinical study, confirm that it is appropriately overseen and ask the study team about its purpose, eligibility, known and uncertain risks, and follow-up. FDA warns that do-it-yourself gene-therapy kits offered for self-administration are illegal and unsafe. Use only FDA-approved products or appropriately overseen clinical studies; do not attempt genetic treatment on yourself.

Questions to ask about a specific treatment

Because broad labels hide important differences, use these questions with a qualified clinician or study team when evaluating a treatment for a particular diagnosis:

  • Which cells or tissue is the treatment intended to change, and how does it reach them?
  • Does it add a gene, alter gene expression, switch off a gene, or edit DNA? What change is intended?
  • Is it given in vivo, or are cells collected, modified, and infused back?
  • What evidence applies to this exact product, diagnosis, and patient group? Is it approved for this indication in the relevant country, or is it being studied?
  • What immune, administration-related, cancer-related, off-target, or genome-integrity risks apply to this product?
  • How will the treatment’s effects and possible complications be monitored, and for how long?
  • What is known about durability and repeat treatment, and what remains uncertain?
