Use this checklist to make every enterprise AI agent accountable, least-privileged, testable and interruptible. Build the program around NIST AI RMF 1.0’s continuous Govern, Map, Measure and Manage functions, then use OWASP AISVS 1.0 as the security verification layer. The result should be an auditable record of who owns an agent, what it can do, which evidence supports approval, and how the organization can stop it when behavior becomes unsafe.

This approach applies to chat-based assistants, retrieval-augmented systems, workflow agents, coding agents, customer-service bots and autonomous operators. It covers production and pre-production deployments, internally built and supplier-provided components, and agents that call tools or delegate work to other agents.

What an enterprise AI agent governance checklist must prove

A checklist is useful only when each item produces an owner, a decision or evidence. For every agent, security should be able to answer six questions:

What’s actually slowing this PC down?

Pick the symptom - the matching free tool is one click away.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • What business outcome is the agent allowed to pursue, and who is accountable for it?
  • Which model, prompts, memory stores, retrieval sources, tools, connectors and environments does it use?
  • Which human or workload identity is requesting an action, and how is that action authorized?
  • What could go wrong, how was it tested, and what severity blocks release?
  • What events are logged, for how long, and who can investigate them?
  • How can the organization pause, quarantine, revoke or roll back the agent without disabling unrelated services?

NIST AI RMF 1.0, published by the National Institute of Standards and Technology in 2023, treats risk management as continuous across the AI system life cycle. Its four functions are not one-time project gates: Govern establishes policies and accountability, Map defines context and risks, Measure evaluates performance and security, and Manage prioritizes treatment, response and improvement.

OWASP AISVS complements that governance spine with a vendor-neutral, testable catalogue of security requirements for AI-enabled systems. Version 1.0, released in June 2026, contains 191 requirements in 12 chapters and three appendices. Its scope spans development, deployment, agent orchestration, monitoring and retirement, making it suitable for translating policy into verification tasks.

1. Inventory, ownership and change control

Start with a complete register rather than a list of approved products. Discovery should include shadow deployments, prototypes with production data, agents embedded in SaaS products and automation created by business teams.

Required inventory fields

  • Agent name, unique asset identifier, environment and deployment region.
  • Business purpose, intended users, risk tier and service criticality.
  • Model provider, model name and version, hosting arrangement and fallback model.
  • Orchestrator, frameworks, agent-to-agent dependencies and execution runtime.
  • Every tool, API, connector, plug-in, package, retrieval index, memory store and data source.
  • Accountable business owner, technical owner, security reviewer and escalation contact.
  • Approval date, next review date, current autonomy level and operating hours.

Assign one human who is accountable for outcomes and one security reviewer who can challenge the design. Record the risk tier and approval decision in a system that supports version history. An agent without a named owner should not receive production credentials.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Track material changes

Require a change record when any of the following changes: model or model version, system or developer prompt, policy rule, tool, connector, retrieval corpus, memory behavior, permission scope, tenant boundary, runtime, hosting region or autonomy level. Define which changes require a new threat assessment, regression test and approval rather than a routine deployment.

2. Identity and authorization controls

Agent identity is the control plane for every later decision. Give each deployed agent a unique workload identity or service principal. Do not use shared, long-lived credentials embedded in prompts, code, images or environment files.

Apply least privilege by action and boundary

  • Allow only the tools required for the declared task.
  • Constrain data access by tenant, sensitivity, geography, purpose and environment.
  • Separate read, write, delete, administrative and outbound-communication privileges.
  • Use distinct identities for development, testing and production.
  • Prevent an agent from exchanging its identity for a user’s broader privileges unless a separately authorized, auditable delegation exists.

Traditional IAM may not fully address autonomous-agent risks. NIST’s agentic-identity work highlights possible data leaks, compliance failures, prompt injection and unpredictable behavior when an autonomous system acts across tools and contexts. Treat the agent identity, the requesting human and each tool identity as separate subjects that must be linked in the audit trail.

Require approval for high-impact actions

Define an action policy before deployment. Require step-up approval or explicit human confirmation for irreversible, privileged, financial, legal, safety-related or external-communication actions. Examples include deleting records, changing production access, issuing refunds, signing contracts, sending customer messages, publishing code or moving regulated data outside an approved region.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Approval should identify the proposed action, affected object, agent, requesting human, policy that triggered the pause and the person who approved or rejected it. A time-limited approval is safer than a blanket exemption.

Manage credentials throughout their life

  1. Issue credentials through a workload-identity or secrets-management system, never through static prompt text.
  2. Set an expiry and rotate keys automatically where possible.
  3. Test normal revocation and emergency disablement in a non-production environment, then document the observed time to effect.
  4. Revoke credentials when an agent, tool, supplier or owner is retired.
  5. Maintain a break-glass procedure with dual authorization and post-event review.

3. Threat modeling and security testing

Agent reviews must model more than conventional API vulnerabilities. The security case should explain how an attacker, malicious document or faulty model could redirect the agent and what barrier limits the resulting impact.

Threats to include

  • Goal hijacking: instructions in a prompt, web page, email or retrieved document redirect the agent from its approved objective.
  • Tool misuse: a valid tool is called with unsafe parameters, excessive frequency or an unintended sequence.
  • Identity and privilege abuse: an agent inherits, confuses or escalates permissions.
  • Supply-chain compromise: a model, package, plug-in, connector, prompt template or tool update is poisoned.
  • Unexpected code execution: generated code or a tool output reaches an interpreter, shell or deployment pipeline without a safe boundary.
  • Memory and context manipulation: poisoned long-term memory or retrieved content persists across users or tasks.
  • Sensitive-data disclosure: prompts, outputs, logs or tool calls expose secrets or personal data.
  • Cascading failure: retries, delegation or agent-to-agent calls amplify an error or create a denial-of-service loop.
  • Human-agent trust exploitation: convincing output causes an operator to approve an unsafe action or overlook missing evidence.

Test realistic attack paths

Include direct and indirect prompt injection, malicious retrieved content, poisoned tools or packages, cross-tenant access, confused-deputy behavior, unsafe output-to-tool transitions and denial-of-service loops. Test both a cooperative agent and one exposed to adversarial inputs; a refusal in a simple chat does not demonstrate safe behavior when the same content arrives through a connector or memory store.

Map evidence to AISVS verification

Map each applicable OWASP AISVS requirement to a control owner and evidence location. Evidence can include architecture diagrams, identity policies, code review records, configuration exports, test transcripts, red-team findings, approval records and incident exercises. Record the AISVS verification level used and the date of the last successful test so an auditor can distinguish a designed control from an operating one.

4. Data, privacy and memory controls

Document the data classification an agent may receive, infer, retain and export. The policy should cover prompts, conversation history, retrieved documents, tool responses, generated files, embeddings and long-term memory, not just the primary database.

Enforce purpose and tenant boundaries

  • Allow access only to data needed for the stated business purpose.
  • Apply tenant, residency, retention and export restrictions at the data and tool layers.
  • Use identity-aware retrieval and memory stores; never rely on the model to remember a tenant boundary.
  • Prevent prompt, memory, retrieval and tool outputs from bypassing data-loss-prevention controls.
  • Define deletion, correction, legal hold and retention behavior for every memory and index.

Test deletion and correction requests end to end. Removing a source record is insufficient if a copied passage remains in a cache, vector index, conversation transcript or tool-generated file. Record which stores were searched, what was removed and any documented retention exception.

Protect telemetry before storage

Redact secrets, authentication material and sensitive personal data before logs or traces are written. Apply the same rule to prompts, model responses, retrieved passages, tool parameters and error messages. Keep a controlled, access-limited diagnostic path for cases where investigators need additional content, with a documented approval and retention period.

5. Monitoring, logging and audit evidence

An agent audit needs to reconstruct both intent and effect. Log enough context to connect a human request to an agent decision, a tool authorization and the resulting change, while preventing the log itself from becoming a data-leak channel.

Minimum event fields

  • Human requester, agent identity, service or tool identity and tenant.
  • Task, session or prompt identifier where policy and privacy rules allow it.
  • Model, orchestrator, prompt-policy and tool versions.
  • Requested action, tool call, parameters or a safely redacted representation.
  • Authorization decision, policy rule, approval or rejection and approver.
  • Data classes and repositories accessed, including retrieval and memory stores.
  • Policy violations, refusals, retries, errors, latency and resource-consumption indicators.
  • External effects such as records changed, messages sent, code deployed or credentials altered.

Make logs tamper-evident, time-synchronized, access-controlled and retained for the period required by organizational policy and applicable regulation. Separate operational debugging from the authoritative audit stream, and monitor access to both.

Alert on behavior, not only failures

  • Privilege escalation or a sudden increase in granted scopes.
  • Unusual tool sequences, high-frequency retries or repeated authorization denials.
  • Attempts to bypass policy, disable monitoring or alter prompts and memory.
  • Data-exfiltration patterns, unexpected destinations or cross-tenant references.
  • Abnormal autonomy, such as actions outside approved hours or a new class of irreversible effect.

Define alert ownership, response time and escalation criteria. A dashboard without a person responsible for triage is not a functioning control.

6. Approval gates and autonomy levels

Do not approve “the agent” as a single undifferentiated capability. Approve a defined task, environment, data scope, tool set and autonomy level. Increase autonomy only when test evidence and monitoring support the additional risk.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Autonomy stage Typical behavior Approval expectation
Observe Reads approved data and proposes an action; no external side effect. Human reviews output before any separate system performs the action.
Assist Prepares drafts, queries or code in a bounded workspace. Human confirms each material change; write access is limited and reversible.
Execute with confirmation Calls approved tools after presenting the proposed effect. Explicit confirmation for each high-impact or irreversible action.
Constrained automation Performs pre-approved, low-impact actions within fixed limits. Policy engine, rate limits, monitoring and sampled human review.
Higher autonomy Sequences multiple actions or delegates to other agents. Formal risk acceptance, stronger testing, continuous monitoring and a tested kill switch.

Compare two deployments using risk criticality, autonomy, data sensitivity, breadth of system access, reversibility, approval friction, identity isolation, monitoring coverage, test evidence, supplier dependence and incident-recovery time. A low-criticality internal assistant may tolerate more automation than an agent that changes customer accounts, even when both use the same model.

7. Incident response and resilience

Write the response plan before increasing autonomy. It should identify who can stop the agent, which credentials are revoked first, how affected tools are isolated and how evidence is preserved.

Required response capabilities

  1. Stop: pause new tasks and block outbound effects.
  2. Quarantine: disable a connector, memory store, tool or tenant without taking unrelated services offline.
  3. Revoke: invalidate agent credentials, delegated tokens and exposed secrets.
  4. Preserve: protect time-synchronized logs, prompts, approvals, tool calls and changed records from alteration.
  5. Recover: roll back reversible changes, restore known-good prompts and configurations, and validate data boundaries.
  6. Learn: notify affected owners, assess regulatory or contractual duties and update controls, tests and training.

Maintain and regularly exercise a kill switch for each production agent. Measure how long it takes to stop execution and how long credentials remain usable after revocation. Test partial failure: the organization should be able to disable one unsafe tool or agent without shutting down unrelated business processes.

8. Third-party and supplier governance

Contracts and onboarding reviews should assign responsibilities instead of assuming that a hosted model provider controls the entire risk. Require suppliers to disclose the model and versioning approach, tool and connector boundaries, data handling and residency, logging fields, identity integration, vulnerability management, incident notification and recovery commitments.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Verify whether supplier updates can change model behavior, prompts, tools, retrieval components or safety policies without customer approval. Require notice and a reassessment trigger for material changes. Keep an exit plan that covers data export, memory and index deletion, credential revocation and migration to a replacement service.

9. Lifecycle review schedule

Use the same governance record throughout the lifecycle:

  • Design: define purpose, owner, risk tier, data flows, threat model and proposed autonomy.
  • Pre-production: complete AISVS-mapped tests, identity review, privacy checks, logging validation and incident exercises.
  • Material change: reassess whenever models, tools, connectors, prompts, retrieval corpora, permissions or environments change.
  • Periodic operation: review alerts, access, test coverage, supplier updates, incidents and whether the original purpose remains valid.
  • Retirement: disable schedules and endpoints, revoke credentials, remove memory and indexes according to policy, preserve required evidence and record the closure decision.

Putting the checklist into an auditable workflow

  1. Create the inventory record and assign the accountable owner and security reviewer.
  2. Classify data, map tools and identities, and document the permitted business purpose.
  3. Set the autonomy stage and enumerate actions that require human confirmation.
  4. Complete the threat model, including agent-specific attack paths and cascading failures.
  5. Implement least-privilege identities, credential lifecycle controls and tenant boundaries.
  6. Configure redacted, tamper-evident logging and alerts before production access is granted.
  7. Run security, privacy and resilience tests; map results to applicable AISVS requirements.
  8. Approve, reject or conditionally approve the deployment with dated evidence and remediation owners.
  9. Exercise the stop, quarantine, revocation and recovery procedures.
  10. Reassess at each material change and at the scheduled periodic review.

The approval packet should let an independent reviewer reproduce the decision: scope, owner, architecture, permissions, test results, unresolved risks, monitoring, emergency controls and expiry or review dates. That evidence turns a policy statement into a control that can be audited and improved.
