The iPhone is still needed to pair each new sensor and to start or stop a Direct to Watch session; the sensor-to-watch connection itself does not require an Apple Watch cellular plan.
What Dexcom G7 Direct to Watch changes
In the traditional smartwatch arrangement, the Apple Watch displays information relayed by the Dexcom G7 app on the iPhone. Direct to Watch adds a separate Bluetooth path: the G7 sensor communicates with the Apple Watch while continuing to send data to the iPhone.
That means you can leave the iPhone elsewhere and still see glucose readings on the watch, provided the sensor and watch remain within Bluetooth range and the watch meets Dexcom’s requirements. The phone remains part of setup and session control rather than disappearing from the system entirely.
Compatible Apple Watch, iPhone and app versions
Dexcom’s documented minimum setup is an Apple Watch Series 6 or later running watchOS 10 or later, an iPhone running iOS 17 or later, and Dexcom G7 app version 2.2.1 or later. Dexcom’s supported-family list includes the following models:

The Dexcom G7 App Store listing has continued to change; it recorded version 2.10.0 on July 29, 2025. Before buying or upgrading a watch, use Dexcom’s live compatibility checker because supported models, operating-system requirements and regional availability can change.
Where the feature is available
DexCom, Inc. announced Direct to Apple Watch availability on June 5, 2024, for the United States, United Kingdom and Ireland, with additional markets planned later that month. Availability is market-specific, so check Dexcom’s current regional support information before attempting setup.
DexCom’s announcement called G7 “the first and only CGM available with Direct to Apple Watch.” That is the company’s description of its product offering, not an independent clinical-effectiveness comparison.

How to connect a G7 sensor to Apple Watch
Keep the iPhone and watch together for the initial connection. The phone performs the compatibility check and authorizes the Direct to Watch session.
- Install or update the Dexcom G7 app on the compatible iPhone and make sure the iPhone runs iOS 17 or later.
- Update the Apple Watch to watchOS 10 or later and keep it paired to the iPhone.
- Open the Dexcom G7 app on the iPhone.
- Go to Connections > Direct to Watch.
- Let the app run its compatibility check, then follow the onboarding prompts to connect the sensor and watch.
- Keep watch notifications enabled and leave iPhone alert mirroring turned on so Dexcom alerts can appear as configured.
A new G7 sensor still has to be paired through the compatible smartphone. The iPhone also starts and stops Direct to Watch sessions, so do those actions while the phone and watch are together.
What happens when the iPhone is not nearby
Direct to Watch mode
The sensor sends readings to the Apple Watch over Bluetooth. The watch can therefore show glucose values and trend information while the iPhone is elsewhere, as long as the sensor-to-watch Bluetooth connection is maintained.

Standard smartwatch mode
Without Direct to Watch, the watch is only mirroring data delivered by the iPhone. If the phone is out of range, the watch cannot receive new readings through that relay.

Do you need an Apple Watch cellular plan?
No cellular subscription is needed for the Bluetooth link between a G7 sensor and Apple Watch. GPS-only and cellular-and-GPS watches differ in what they can do with data beyond the local display:

The cellular capability therefore affects data uploading and connectivity beyond the immediate sensor-to-watch link; it is not a prerequisite for viewing readings on the watch itself.

Recognizing a lost direct connection
If the watch loses its direct sensor link, an orange phone icon indicates that it may be copying readings from the iPhone instead. In that state, bring the iPhone close to the watch to restore the phone relay or re-establish the direct connection according to the Dexcom app’s prompts. Leaving the phone behind will not restore a broken sensor-to-watch Bluetooth path.
Practical limits when leaving the iPhone at home
- Readings: You can check current glucose information on the watch through Direct to Watch while the sensor and watch remain connected.
- New sensors: Pairing a replacement sensor still requires the compatible iPhone.
- Session controls: Start and stop commands are performed in the iPhone app.
- Alerts: Keep watch notifications and iPhone alert mirroring configured; changing notification permissions can prevent expected alerts.
- Clarity data: Upload behavior follows the watch’s GPS-only or cellular-and-GPS capability described by Dexcom.
Troubleshooting checklist
The Direct to Watch option is missing
- Confirm the Apple Watch is Series 6 or newer, SE (2nd generation), Ultra or Ultra 2.
- Check that the watch runs watchOS 10 or later, the iPhone runs iOS 17 or later, and the Dexcom G7 app is version 2.2.1 or later.
- Verify that Direct to Watch is supported in your country or region.
The watch shows an orange phone icon
The watch is likely receiving a phone-relayed reading rather than a direct sensor reading. Bring the iPhone nearby, check Bluetooth and notification permissions, and reopen Connections > Direct to Watch in the iPhone app if the direct session needs to be re-established.
Readings appear on the watch but not in Dexcom Clarity
Check whether the watch is GPS-only. Dexcom documents direct display for GPS-only models but Clarity uploading for cellular-and-GPS models. Also allow the watch or phone to reconnect as required by your configuration.

A replacement sensor will not start
Use the compatible iPhone to pair the new sensor and start the session. Direct to Watch does not remove that phone-based activation step.
Choosing a watch for G7 use
Apple Watch Series 9 is one of the explicitly listed supported families, but the model name alone is not enough: the watch must run the required watchOS version, the paired iPhone must meet its iOS requirement, and Direct to Watch must be available in your market. Confirm all three conditions in Dexcom’s current compatibility checker before purchasing a watch or changing regions.
