Caffeine—the central acute ingredient in many pre-workout formulas—can make exercise feel more energetic and improve some performance measures for some people. Choose based on the activity, how quickly you want an effect, your caffeine tolerance, and whether a stimulant could disrupt sleep.

The evidence supports creatine as a training supplement and caffeine as a potential acute aid. “Pre-workout” is a product category, not one standardized formula, so the name alone does not establish what a particular blend will do. NIH Office of Dietary Supplements and the International Society of Sports Nutrition (ISSN) position stand describe benefits that vary with ingredient, activity, and individual response.
What creatine does for performance
Where it can help
Creatine helps muscles replenish adenosine triphosphate (ATP), a rapid energy source used during short, intense efforts. It is most relevant when exercise involves repeated hard bouts—such as lifting sets or sprints—rather than one long, steady effort. NIH ODS summarizes evidence of improvements in strength, power, repeated maximal contractions, sprinting, and soccer performance. Results vary; factors such as diet and muscle-fiber composition may affect response. People who do not eat meat may respond more because their baseline muscle creatine can be lower.
How to take it
Creatine is not a stimulant and should not be expected to create an immediate, noticeable surge of energy before a workout. The adult protocols described by NIH ODS are either a loading phase of 20 g per day, divided into four 5 g servings for 5–7 days, followed by 3–5 g per day, or about 3–6 g per day without loading for 3–4 weeks. These are general protocols summarized from research, not individualized medical advice.

Creatine monohydrate is the most widely used and studied form. NIH ODS says other forms have not been shown superior for raising muscle creatine, digestibility, stability, or safety. A higher-priced alternative form therefore does not have established superiority over plain monohydrate in these respects.
Limits and trade-offs
Creatine appears to offer little value for distance running or swimming, and extra body mass may hinder some endurance performance. Several studies summarized by NIH ODS reported a 1–2 kg increase in total body weight over a month of creatine monohydrate use during strength training; that is a study finding, not a guaranteed result. Commonly reported side effects center on weight gain associated with water retention. Nausea, diarrhea, cramps, and heat intolerance are also reported, though some complaints are anecdotal.

What caffeine in pre-workout does
Acute energy and performance
Caffeine blocks adenosine activity. It can increase arousal and vigor while reducing perceived pain and exertion, which may make a workout feel easier or more energetic. NIH ODS reports that many studies find possible performance benefits at 2–6 mg per kilogram of body weight before exercise. ISSN identifies 3–6 mg/kg as a range consistently associated with performance improvement, while noting the minimum effective dose remains uncertain and may be as low as 2 mg/kg. The best amount for an individual cannot be inferred from those study ranges alone.
Effects are not guaranteed. Across a review of 33 caffeine trials summarized by NIH ODS, performance changes ranged from a 0.7% decrease to a 17.3% increase, and half of the changes were not statistically significant. Responses differ by person and by exercise type. Very high doses such as 9 mg/kg are associated with more side effects and are not needed to produce an ergogenic effect.

Timing and sleep
NIH ODS describes 15–60 minutes before exercise as a possible timing window; ISSN says 60 minutes is the most common timing, with the delivery form affecting absorption. NIH ODS reports that caffeine blood levels peak within about 45 minutes and that caffeine has a half-life of around 4–5 hours. Because caffeine can remain in the body for hours, late-day use may interfere with sleep, depending on timing and individual sensitivity.
Why the product label matters
Pre-workout formulas may combine caffeine with other ingredients, but evidence for caffeine does not prove that every ingredient or blend improves performance. NIH ODS describes citrulline research as limited and conflicting, and evidence for several other ingredients as mixed or limited. Check the serving size, caffeine amount per serving, and complete ingredient list rather than relying on the product category or branding.

Safety considerations before choosing
Caffeine amount and sensitivity
NIH ODS reports FDA guidance that 400 mg of caffeine per day does not usually cause dangerous adverse effects in healthy adults. That general amount is not a recommended target, does not establish a safe amount for every person, and includes caffeine from all sources. Reported adverse effects include insomnia, restlessness, nausea, vomiting, tachycardia, and arrhythmia. Avoid concentrated pure caffeine powder; measuring a safe amount from highly concentrated powder can be difficult.
Creatine, health conditions, and younger users
The National Center for Complementary and Integrative Health advises people at risk of kidney problems to consult a health professional and be monitored if they use creatine. It also notes that data documenting creatine safety in children or adolescents are absent, and cites pediatric and sports-medicine organizations that warn teens against performance-enhancing supplements. NCCIH cautions that bodybuilding supplements may contain hidden or adulterated ingredients; discuss supplement use and possible interactions with a health care provider. Tested athletes
Rules depend on the governing organization. The NIH ODS fact sheet says the World Anti-Doping Agency does not prohibit or limit caffeine, while Olympic and NCAA rules described there use different urine thresholds. Athletes should verify current rules with their own governing body before using a product; policies can change.
Which one should you choose?
- Choose creatine monohydrate if your main goal is better capacity for repeated high-intensity work, such as lifting or sprinting, and you are comfortable taking it consistently rather than expecting a pre-session boost.
- Consider caffeine-containing pre-workout if you want an acute stimulant effect and caffeine suits your tolerance, schedule, and exercise. Evaluate the total caffeine you already consume and the formula’s full label.
- Consider neither, or get professional advice first, if stimulant effects threaten your sleep, you have relevant health concerns, or the product’s ingredients and amounts are unclear.
- Do not assume they are alternatives in every sense. They work differently, and a person may use one without the other; whether combining them is appropriate depends on total caffeine, individual circumstances, and professional advice where needed.
For athletes competing under anti-doping rules, check the current requirements of the applicable organization before choosing either a supplement or a pre-workout product.
