Cloud AI inference is the production step that runs a trained model for each user request. A service queues and tokenizes the prompt, processes it on accelerator-backed servers, and streams generated tokens back. GPUs provide the parallel math and high-bandwidth memory needed to hold model weights and the growing key-value (KV) cache. Providers usually bill language-model inference by input and output tokens, while performance is best understood through time to first token (TTFT), inter-token latency (ITL), and end-to-end latency.
What cloud AI inference means
Training changes a model’s parameters using a large dataset. Inference uses those fixed parameters to answer a request, classify text, create an embedding, or generate another prediction. Cloud inference is the same serving phase delivered through a provider’s API or through GPU virtual machines that you operate.
For a text-generation request, the service performs several linked operations:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- It accepts the request and places it in a scheduler queue.
- It tokenizes the prompt, converting text into the model’s vocabulary units.
- It runs a prompt-processing pass, often called prefill, across the complete context.
- It generates output one token at a time, updating the KV cache after each step.
- It detokenizes the result and returns it, often as a stream so the first tokens appear before the answer is complete.
Tokens are not identical to words. A token can be a whole short word, part of a longer word, punctuation, or whitespace. The tokenizer and language determine the count, so the same visible text can consume different numbers of tokens in different models.

Why accelerators matter
Inference repeatedly performs large tensor operations that are highly parallel. GPUs can execute those operations far more efficiently than a general-purpose CPU for many model sizes. They also provide high-bandwidth memory for the model weights and the KV cache, which grows as the prompt and generated conversation become longer. A deployment can run on CPUs for small or lightly used models, but latency, throughput, and memory requirements usually make GPUs the practical choice for modern large language models.
How one request uses the cloud
Queueing and tokenization
Before model computation starts, a request may wait behind other work. Queueing varies with traffic, quotas, batching policy, and the provider’s available capacity. Tokenization then determines the actual input length that the model must process; character count is not a reliable substitute.
Prefill creates the first-token delay
During prefill, the server reads the complete input context and builds the KV cache. Longer prompts generally increase this work and therefore increase time to first token. A request with a large retrieved document can have a noticeably slower first response even when it ultimately generates only a few words.

Decode streams the answer
After prefill, the model decodes one new token at a time. The interval between those tokens determines how quickly a streaming answer appears. The service may combine several users’ decode steps in a batch; that improves hardware utilization but can make each user share memory bandwidth and scheduling time.
Completion and accounting
The final token marks the end of generation, but network transfer and gateway processing can add to the observed completion time. Billing systems typically count the tokenized input and generated output separately, even when the API returns a single response object.
Why APIs charge for input and output tokens
Token pricing maps usage to the main computational work. Input tokens require prompt processing and occupy context memory; output tokens require repeated decode steps. Providers therefore commonly publish separate input and output rates.

| Billable component | What it represents | Questions to ask |
|---|---|---|
| Input tokens | Prompt, system instructions, conversation history, and retrieved context sent to the model. | Is the count measured before or after provider-side formatting? Are cached prefixes discounted? |
| Output tokens | Tokens generated for the completion, including any reasoning or hidden output the service bills. | Is there a maximum-output setting, and does the provider charge for truncated or filtered output? |
| Cached tokens | Previously processed prompt material reused by an implicit or explicit cache. | What is the cache-hit definition, retention period, and hit discount? |
| Batch requests | Asynchronous jobs submitted for lower-priority, higher-throughput processing. | What completion-time window applies, and does the model qualify for the batch rate? |
| Regional or service-tier charges | Premium routing, regional processing, provisioned capacity, or other delivery options. | Does the selected region or latency tier add a surcharge? |
Use this model for an initial estimate:
cost = (input_tokens / 1,000,000 × input_rate) + (output_tokens / 1,000,000 × output_rate) + cache, batch, regional, storage, network, and platform charges

The rates and model catalogs change frequently by model, region, account tier, and date. AWS Bedrock publishes model-specific input and output rates and lists batch, cache, and latency-optimized options; its current pricing material says select batch-inference offerings can be 50% lower than on-demand pricing, but the applicable model and region must be verified. OpenAI documents a 10% regional-processing uplift for eligible models released on or after March 5, 2026. Google Vertex AI documents implicit caching and provisioned-throughput scale units; a cache hit can reduce both cost and latency. None of these figures is a universal cloud-inference price.
Do you need a GPU for inference?
You need enough memory and compute for the chosen model, context, concurrency, and latency target. A GPU is not automatically required, but it is often the most economical way to serve a sizeable generative model at interactive speed.

| Decision factor | Why it changes the hardware requirement |
|---|---|
| Model size | More parameters require more weight memory and generally more compute per token. |
| Context length | Longer prompts enlarge the KV cache and increase prefill work; memory can become the limiting resource. |
| Quantization | Lower-precision weights reduce memory use and may improve throughput, with a possible quality trade-off that must be tested. |
| Concurrency | Simultaneous requests need additional KV-cache space and scheduling capacity. |
| Target latency | Strict TTFT or ITL objectives usually require faster accelerators, more replicas, or both. |
| Utilization pattern | Steady traffic can justify dedicated instances; sporadic traffic often favors an elastic API that avoids idle hardware. |
Managed API
A managed API is attractive when a team needs rapid integration, elastic capacity, and access to several models without maintaining servers. The provider handles accelerator placement, runtime updates, autoscaling, and much of the observability. Trade-offs include provider-specific model availability, quotas, regional restrictions, token pricing, retention policies, and limited control over batching or kernel settings.
Rented or self-managed GPU instances
Dedicated cloud GPU instances provide control over model placement, quantization, runtime version, batching, networking, and data locality. They can be cheaper at sustained utilization because you pay for a machine rather than every token. You also assume capacity planning, deployment, autoscaling, patching, monitoring, incident response, and the cost of idle capacity. Google Cloud publishes GPU SKU prices separately from machine configuration and provides a Billing Catalog API and Pricing Calculator, so a GPU-hour quote must include the attached host and all ancillary services.
How providers should be evaluated
NVIDIA’s provider-selection guidance says organizations should evaluate “the underlying hardware, per user interactivity measured in tokens per second per user, cost per million tokens, latency and time-to-first-token, model diversity, and whether the provider has capacity to support workload growth.” Treat that as a checklist rather than a single score.

Understanding cloud AI latency
| Metric | Definition | What it tells you |
|---|---|---|
| Time to first token (TTFT) | Time from request submission until streaming begins. | Interactive responsiveness; includes queueing, prefill, and network effects. |
| Inter-token latency (ITL), or time per output token | Average delay between consecutive generated tokens. | How smoothly the answer continues after the first token. |
| End-to-end latency | TTFT + generation time, plus any final transfer or gateway overhead observed by the client. |
Total wait until the final token arrives. |
| Time to last token (TTLT) | Azure’s gateway measure from prompt submission to the last token. | Useful for completion time, but not sufficient without output length. |
A five-second TTLT can represent a very different experience for a 50-token answer than for a 2,000-token answer. Report prompt length, output length, and percentile values with every latency result. Averages hide queue spikes that users experience as stalls, so production SLOs normally track at least p50 and p95 TTFT and ITL, plus p95 end-to-end latency.
Throughput, batching, and concurrency
GenAI-Perf defines system-level tokens per second (TPS) as total output tokens divided by the elapsed benchmark interval. Aggregate TPS usually rises as concurrency increases until the GPU reaches a resource limit. Beyond saturation, queueing grows and throughput can fall. Per-user TPS generally declines earlier because requests share compute and memory bandwidth.
Do not compare a benchmark run that uses short prompts and short completions with production traffic that has long retrieval contexts. Measure at the prompt and output distributions you actually expect. Record successful requests per second, output TPS, TTFT, ITL, p95 completion latency, GPU utilization, and cache-hit rate at each concurrency level.

| Axis | Measurements or questions |
|---|---|
| Interactive latency | p50/p95 TTFT and ITL under realistic prompt and output lengths. |
| Throughput | Output tokens per second and successful requests per second at target concurrency. |
| Economics | Delivered-token cost, including idle GPU time, cache hits, batch discounts, egress, and platform fees. |
| Capacity and reliability | Quotas, regional availability, autoscaling behavior, retry semantics, and failure handling. |
| Model and data constraints | Required model, context window, data residency, private networking, retention, and training-use policy. |
| Operational control | Managed simplicity versus control of GPU type, runtime, quantization, batching, and deployment topology. |
A practical method for sizing and pricing inference
Start with a workload sheet instead of a provider’s headline rate. Include:
- Requests per minute and peak requests per minute.
- Input-token and output-token distributions, not just averages.
- Maximum and typical concurrency.
- Required model, context window, region, and uptime target.
- TTFT, ITL, and end-to-end latency SLOs.
- Expected cache-hit rate and the percentage of traffic eligible for batch processing.
- Define representative traffic. Assemble anonymized prompts and expected completion limits that match production. Include long-context and worst-case requests.
- Test several concurrency levels. Run the same workload at low load, normal load, and above the expected peak until saturation is visible.
- Capture comparable metrics. Record p50 and p95 TTFT, ITL, end-to-end latency, successful requests per second, output TPS, GPU utilization, cache hits, errors, and total spend.
- Convert all options to one denominator. For an API, calculate cost per delivered token. For rented GPUs, divide instance, storage, network, orchestration, and idle costs by the tokens actually delivered while meeting the SLO.
- Add capacity headroom. Size replicas or provisioned units from the saturation point plus growth and failure headroom, not from the average load alone.
- Recheck volatile inputs. Confirm current model prices, quotas, GPU SKUs, regional availability, and service-tier terms immediately before committing.
Comparing a provider’s advertised GPU-hour rate directly with an API’s per-token rate is misleading. The meaningful denominator is delivered tokens under the required latency, quality, reliability, and region constraints.
Quick Recap
Common failure modes and fixes
First-token delays are high
- Check queue time and quota throttling before changing models.
- Reduce unnecessary prompt and retrieval tokens; long contexts increase prefill work.
- Measure cache hits and ensure repeated prefixes are actually eligible for caching.
- Test a lower-concurrency or latency-optimized tier if batching is creating head-of-line waiting.
Streaming is smooth but completion is slow
- Inspect ITL and output length separately; a long answer can have acceptable TTFT but high total time.
- Set an output-token limit appropriate to the task.
- Compare quantized or smaller models only after checking quality on representative evaluations.
The GPU is expensive but underused
- Plot utilization and delivered TPS over the full day, including idle periods.
- Consolidate compatible workloads with continuous batching where your runtime supports it.
- Compare the resulting cost per delivered token with a managed API, including operations and failover capacity.
Costs exceed the estimate
- Separate input and output token counts in billing exports.
- Look for longer-than-expected conversation history, retrieval payloads, or hidden reasoning output.
- Check whether regional processing, provisioned capacity, storage, egress, or platform fees were omitted.
- Verify that the assumed cache and batch discounts apply to the selected model and region.
What to remember
- Inference is the serving phase: accelerators execute the model while tokens are the main language-model accounting unit.
- Input and output tokens are often priced differently, and cache, batch, region, and service tier can change the bill.
- TTFT, ITL, and end-to-end latency describe different parts of the user experience.
- Concurrency can raise system throughput until GPU saturation while worsening each user’s latency.
- A GPU-hour price is not an inference cost; compare cost per delivered token at a stated workload.
- Managed APIs favor speed and elasticity, while GPU instances favor control and can win when utilization is sustained and predictable.
